Die Implantation kann transgingival 1-phasig oder gedeckt 2-phasig durchgeführt werden. Bei der Implantation transgingival 1-phasig entfällt der chirurgische Zweiteingriff. Für die Implantation 2-phasig gedeckt muss drei Wochen vor der Abformung ein Gingivaformer zur Weichgewebekonditionierung in das Implantat eingeschraubt werden. Achten Sie bei der Auswahl der Instrumente auf die Farbcodierungen, die mit der Farbcodierung des entsprechenden Implantats übereinstimmen müssen. 7.1.1 Präparation des Implantatbetts Da die Langzeitprognose eines Implantats und das ästhetische Ergebnis mit der optimalen Positionierung steigt, wird die Verwendung einer Bohrschablone empfohlen. Wichtiger Hinweis: Die Präparation des Implantatbetts für CONELOG® SCREW-LINE Implantate erfolgt mit den chirurgischen Instrumenten der SCREW-LINE Implantate. Verwenden sie Bohrer in aufsteigendem Durchmesser. Deutsch 1Produktbeschreibung CONELOG® SCREW-LINE Implantate, sind enossale Implantate, erhältlich in verschiedenen Durchmessern und Längen. Sie werden chirurgisch in den Knochen des Ober- und/oder Unterkiefers gesetzt zur Verankerung von funktionellen und ästhetischen oralen Rehabilitationen bei teilbezahnten- und zahnlosen Patienten. Die prothetische Versorgung erfolgt mit Einzelkronen, Brücken oder Totalprothesen, die durch entsprechende Elemente mit den CONELOG® SCREW-LINE Implantaten verbunden werden. Prinzipiell gibt es keine bevorzugten Einsatzbereiche für die CONELOG® SCREW-LINE Implantate. Die äußere Oberfläche der CONELOG® SCREW-LINE Implantate ist sandgestrahlt und säuregeätzt (Promote®-Oberflächenstruktur). Die Implantatschulter der CONELOG® SCREW-LINE Implantate ist maschiniert. CONELOG® SCREW-LINE Implantate sind in der Innenkonfiguration mit einem Konus zur Rotationssicherung und drei Nuten zur Positionierung von CONELOG® Abutments versehen. Die CONELOG® Abutments sind apikal mit einem Konus und drei Nocken versehen und greifen in die Konusverbindung und die drei Nuten des Implantats ein. Die Implantatschulter wird dabei nicht vom C ONELOG® Abutment abgedeckt. ® ® Für die CONELOG SCREW-LINE Implantate stehen eigene CONELOG Komponenten wie CONELOG® Verschlussschrauben, CONELOG® Gingivaformer, CONELOG® Abformpfosten sowie CONELOG® Prothetik komponenten zur Verfügung. Wichtiger Hinweis: Nur diese Komponenten sind mit der konischen Innenkonfiguration der CONELOG® SCREW-LINE Implantate kompatibel! CONELOG® SCREW-LINE Implantate sind Bestandteil des CAMLOG® Implantatsystems. Das CAMLOG® Implantatsystem beinhaltet chirurgische, prothetische, labortechnische Komponenten und Instrumente. 2Indikationen Implantate des CONELOG® Implantatsystems sind als Sofort- oder verzögertes Sofortimplantat im Knochen des Ober- oder Unterkiefers vorgesehen. Abutments für das CONELOG® Implantatsystem sind für die Versorgung von Kronen-, Brücken- und Vollprothesen vorgesehen. Beim einzeitigen chirurgischen Vorgehen können die Implantate sofort belastet werden, falls eine gute Primärstabilität erzielt wurde und die funktionale Belastung angemessen ist. Der Chirurg muss bei der Planung genaue Kenntnisse über das verwendete Messsystem verfügen und einen angemessenen Sicherheitsabstand zu den Zähnen und vitalen Strukturen einhalten. Wird die tatsächliche Bohrtiefe in Relation zur Röntgenaufnahme nicht richtig ermittelt und über die beabsichtigte Tiefe hinausgebohrt, kann dies dauerhafte Verletzungen von Nerven oder anderen vitalen Strukturen verursachen. Ein Sicherheitsabstand von 1.5 mm zum Nervus mandibularis bzw. Nervus alveolaris inferior muss eingehalten werden. Jeder Bohrer verfügt über Tiefenmarkierungen die unbedingt beachtet werden müssen. Beim Glätten der koronalen Knochenkante wird die zur Verfügung stehende Knochenhöhe geringer. Die Implantatlänge muss dann erneut überprüft werden. Warnung: Wird bei einer Bohrung im Oberkiefer die Kieferhöhle perforiert, wird empfohlen, die Behandlung mit Hilfe von Augmentationsmaßnahmen weiterzuführen. • Schnittführung • Optional: Anphasen/planieren des Alveolarkamms an der gewünschten Implantatposition mit dem Rosenbohrer. • Entsprechenden Tiefenstopp auf dem Bohrer anbringen, um ein zu tief gebohrtes Implantatbett zu vermeiden. Pilot- und Vorbohrer ohne Tiefenstopp sind für Implantatlängen von 16 mm geeignet. Wird eine Bohrschablone benutzt können die Tiefenstopps nach den Markierungsbohrungen aufgesetzt werden. • Intermittierende Bohrtechnik für Pilot-/Vorbohrer (für Bohrung zur Erreichung der gewünschten Tiefe) und Formbohrer (für Erweiterung des Bohrlochs bis zum gewünschten Durchmesser): Knochen 2 bis 3 Sekunden anbohren. Anschließend Bohrer nach oben aus dem Knochen ziehen ohne den Handmotor zu stoppen. Vorgang wiederholen bis die gewünschte Tiefe erreicht ist. • Gegebenenfalls bei Pilot- oder Vorbohrungen die Tiefe und Achsrichtung mit einem Parallelisierungspfosten kontrollieren. • Bohrer in aufsteigendem Durchmesser verwenden bis der gewünschte Durchmesser für das Implantatbett erreicht ist. • In den Knochenqualitäten 1 und 2 (Lekholm & Zarb 1985*) kann unter Umständen ein Vorschneiden des Gewindes mit dem Gewindeschneider erforderlich werden, wenn sich bei der Implantatbett gestaltung zeigt, dass die Kortikalis sehr kompakt ist. Gewindeschneider maximal bis zur Oberkante des schneidenden Arbeitsteiles eindrehen. • Für CONELOG® SCREW-LINE Implantate kann bei Knochenqualitäten 1 und 2 der apikale Teil des Implantatbetts mit den Formbohrern SCREW-LINE Cortical Bone aufgeweitet werden, was zu einer Reduktion des Eindrehmoments der Implantate führt. Für CONELOG® Implantate mit einem Durchmesser von Ø 3.3 mm gelten folgende zusätzliche spezifische Indikationen: Diese Implantate sind eine Alternative bei beschränkter Kieferkammbreite von 5 – 6 mm. Aufgrund der geringeren mechanischen Festigkeit, verglichen mit den Implantaten größerer Durchmesser, dürfen Sie nur wie folgt angewendet werden: • Einzeln nur zum Ersatz von Incisivi im Unterkiefer und/oder lateralen Incisivi im Oberkiefer. • Bei einer Stegverblockung von mindestens vier Implantaten mit Ø 3.3 mm ohne distale Verlängerungen können zahnlose Kiefer prothetisch versorgt werden. • Im teilbezahnten Kiefer eignen sich Implantate mit Ø 3.3 mm in Kombination mit Implantaten grösserer Durchmesser bei verblockten Suprastrukturen. Voraussetzung ist jedoch, dass die begrenzte Festigkeit der Implantate mit Ø 3.3 mm berücksichtigt wird. • Bei Verwendung von Kugelaufbauten in Verbindung mit Ø 3.3 mm Implantaten muss eine zu starke mechanische Belastung der Implantate vermieden werden. Für CONELOG® Implantate mit einer Länge von 7 mm gelten folgende zusätzliche spezifische Indikationen: Diese Implantate sollen nur dann verwendet werden, wenn für längere Implantate nicht ausreichend Platz zur Verfügung steht. Diese Implantate sind für die verzögerte Belastung bei Einzelzahnrestaurationen indiziert. Bei ungünstigem Verhältnis von Kronenlänge zu Implantatlänge müssen die bio mechanischen Risikofaktoren berücksichtigt werden und die notwendigen Maßnahmen vom Fachexperten getroffen werden. Die endgültige Positionierung immer manuell durchführen. Ansonsten könnte das Implantataußen gewinde im Knochen überdreht werden. 3Kontraindikationen Ungenügendes Knochen- und Weichgewebsangebot und/oder inadäquate Knochenqualität, lokale Wurzelreste, Erkrankung des Knochens und Wundheilungsstörungen, lokale Infektion der Implantationsstelle, schwerwiegende therapieresistente Funktionsstörungen, unkontrollierte Diabetes Mellitus, Langzeit immunosuppressive Therapie, Bindegewebeserkrankung/Kollagenosen, Blutkrankheiten (z. B. Leukämie, Hämophilie), Intraorale Infektion oder Malignome, unkontrollierte parafunktionelle Gewohnheiten, Behandlungsunfähige Okklusal- oder Artikulationserkrankungen, Schwerwiegende psychische Erkrankungen, Xerostomie und Titanallergie. Doppelkronenkonstruktionen sind auf Ø 3.3 mm Implantaten nicht zulässig. Bei Einzelzahnversorgungen auf 7 mm langen Implantaten wird eine sofortige Belastung nicht empfohlen. Vorsichtsmaßnahmen Es ist wichtig vor der chirurgischen Implantation eine sorgfältige Patientenanamnese und sofern nötig eine Fremdanamnese durch den Hausarzt zu erheben, um abzuklären, ob 1) eine erschwerte Implantation aufgrund der anatomischen Situation, 2) ein ernsthaftes chirurgisches Problem oder ein allgemeines Risiko, 3) eine beeinträchtigte Wundheilung und/oder Osseointegration oder 4) eine Verschlechterung der ordentlichen Hygiene und/oder Pflege von Implantat, Abutment und Prothese auftreten könnte. Im Folgenden sind aus jeder Kategorie Beispiele aufgeführt, welche berücksichtigt werden sollten. Einige können auf mehr als einer Kategorie zutreffen. Falls irgend eine dieser Fälle oder eine Kombination dieser Fälle schwerwiegend oder unkontrolliert ist, soll auf ein Kieferimplantat verzichtet werden. Anatomische Verhältnisse Nicht abgeschlossenes Kieferwachstum, ungünstige anatomische Knochenverhältnisse, vorbestrahlter Knochen, temporomandibuläre Gelenkserkrankungen und behandlungsfähige pathologische Kiefer erkrankungen. Chirurgische und allgemeine Risiken Schwerwiegende systematische Erkrankungen, reduzierte Immunabwehr und Leukozytendysfunktionen, die das Infektionsrisiko erhöhen, endocrine Erkrankungen, medikamentöse Antikoagulation/hämorrhagische Diathesen, Arteriosklerose und Schlaganfall, Hypertonie, Herzinfarkt, Erkrankungen mit periodischem Gebrauch von Steroiden, Hepatitis, Diabetes mellitus und Schwangerschaft. Gestörte Wundheilungsfähigkeit Knochenstoffwechselstörungen, alle Erkrankungen, welche die Knochenregeneration oder die Mikro zirkulation des Bluts beeinflussen, Erkrankungen des rheumatischen Formenkreises und Drogen-, Alkohol- oder Tabakabusus. Pflege Verweigerte Patientencompliance, ungenügende Mundhygiene, Parodontitis, Bruxismus, parafunktionelle Gewohnheiten und Mundschleimhautveränderungen. Implantate mit einem Durchmesser von Ø 3.3 mm sind nicht für Einzelzahnversorgungen von zentralen Incisivi im Oberkiefer sowie Canini, Prämolaren und Molaren im Unter- und Oberkiefer geeignet. Implantate mit einem Durchmesser von Ø 3.8 mm dürfen im Molarenbereich nicht mit abgewinkelten Abutments verwendet werden. 4Nebenwirkungen In einzelnen Fällen findet keine Osseointegration statt. Bitte wenden Sie sich in diesem Fall an die lokale Vertretung. Unmittelbar nach der Insertion von Dentalimplantaten sollten Aktivitäten, bei denen der Körper hohen physischen Belastungen ausgesetzt ist, vermieden werden. Mögliche Komplikationen nach der Insertion von Zahnimplantaten können sein: Vorübergehende Beschwerden Schmerzen, Schwellungen, Sprechschwierigkeiten, Zahnfleischentzündungen. Länger anhaltende Beschwerden Chronische Schmerzen in Verbindung mit dem Dentalimplantat, permanente Parästhesie, Dysästhesie, Nervschädigung, Exfoliation, Hyperplasie, lokalisierte oder systemische Infektionen, Oroantral- oder Oronasalfisteln, Verlust von Oberkiefer-/ Unterkieferkammknochen, ungünstig beeinflusste Nachbarzähne, irreversible Schäden an Nachbarzähnen, Implantat-, Kiefer-, Knochen-, oder Zahnersatzfrakturen, ästhetische Probleme. 5 Allgemeine Sicherheits- und Warnhinweise • Unsachgemäßes Vorgehen bei Chirurgie und Prothetik kann zu Schäden am Implantat oder zu Knochenverlust führen. Das CAMLOG® und das CONELOG® Implantatsystem sollte nur durch mit dem System ausgebildete Zahnärzte, Ärzte und Chirurgen angewendet werden. Die Anwendung des Implantatsystems erfordert spezielle Kenntnisse und Fertigkeiten über Implantologie. Jeder Patient muss gründlich untersucht und bezüglich seines röntgenologischen, psychischen und physischen Status beurteilt werden, einschliesslich der Zähne und der dazugehörigen Hart- und Weichgewebedefizite, die Einfluss auf das Endresultat haben können. Die enge Zusammenarbeit zwischen Chirurg, Prothetiker und Zahntechniker ist für den Erfolg unentbehrlich. Das CAMLOG® und das CONELOG® Implantatsystem und die entsprechenden Verfahren wurden von Fachexperten entwickelt und klinisch geprüft. Detaillierte Informationen zur Auswahl der geeigneten Implantate, der prothetischen Komponenten, zur Behandlungsplanung und Anwendung von CAMLOG® und CONELOG® Implantaten, sind in den Anwenderinformationen und unter www.camlog.com der CAMLOG Biotechnologies AG ersichtlich. Verwenden Sie deshalb ausschliesslich chirurgische, prothetische labortechnische Komponenten und Instrumente von CAMLOG. Alle Bestandteile des CAMLOG® und des CONELOG® Implantatsystems sind optimal auf einander abgestimmt und Teil des Gesamtsystems. • Bohrer, Instrumente und Systemkomponenten sind für bestimmte Implantate und Implantatdurchmesser bestimmt. Die Verwendung für andere Implantate oder anderen Durchmessern kann zu mechanischem Versagen von Systemkomponenten, Gewebeschädigungen oder zu unbefriedigenden ästhetischen Ergebnissen führen. CONELOG® SCREW-LINE Implantate dürfen nur in Verbindung mit Bohrern, Instrumenten der SCREW-LINE und CONELOG® Prothetikkomponenten verwendet werden. Beachten Sie die Farbmarkierungen zur Auswahl der Instrumente für die benötigten Implantatdurchmesser. • Aufgrund der geringen Größe, kann es zum Verschlucken und zur Aspiration eines CAMLOG®/ CONELOG® Produktes kommen. Die Aspiration kann zu Atemnot und im schlimmsten Fall zur Erstickung führen. Aus diesem Grund sind die Produkte bei intraoraler Anwendung mit einem Faden bzw. Zahnseide vor Verschlucken und Aspiration zu sichern. • Bei den zu den jeweiligen Produkten aufgeführten Indikationen ist zu beachten, dass alle Indikationen die nicht aufgeführt wurden, kontraindiziert sind. • Implantate mit kleinem Durchmesser und abgewinkelte Abutments werden nicht für den Einsatz im posterioren Bereich empfohlen. • Verwendung bei Magnet-Resonanz-Untersuchungen: Das CONELOG® Implantatsystem ist nicht auf Sicherheit und Kompatibilität bei Magnet-Resonanz-Untersuchungen geprüft worden. Das CONELOG® Implantatsystem ist nicht auf Erhitzung oder Migration bei Magnet-Resonanz-Untersuchungen geprüft worden. 6 Vorbereitung der Implantate und Instrumente 6.1 Vorbereitung des Patienten Voraussetzungen für eine erfolgreiche Implantation sind: Lokale und systemische Anforderungen Normale Wundheilungskapazität, effiziente Mundhygiene, saniertes Restgebiss, abgeschlossenes Oberund Unterkieferwachstum, guter allgemeiner Gesundheitszustand, ausreichendes Angebot an gesundem Kieferknochen. Lokalbefund Anatomie des Kieferkammes, intermaximilläre Beziehungen wie Tiefbiss, Qualität und Dicke der Mukosa, Studienmodelle und Bissregistrierung im Artikulator, Röntgenbefund. Mängel in der Patientenevaluation, präoperativen Diagnostik und Therapieplanung können einen Implantatverlust verursachen. Dem chirurgischen Teil der Implantatversorgung müssen eine umfassende Patientenevaluation, präoperative Diagnostik und Therapieplanung vorausgehen. Implantatdurchmesser und Implantatlänge sind so zu bestimmen, dass um das Implantat ausreichend Knochen (mindestens 1 mm) vorhanden ist. Ein Mindestabstand von 1.5 mm zu einem angrenzenden natürlichen Zahn und 3 mm zu einem angrenzenden Implantat ist einzuhalten. 6.2 Vorbereitung der Instrumente Die Instrumente des CAMLOG® Implantatsystems werden, wenn sie nicht ausdrücklich als steril gekennzeichnet sind, nicht steril geliefert. Sie müssen vor der ersten und jeder weiteren Anwendung am Patienten gereinigt, desinfiziert und sterilisiert werden (siehe Abschnitt 8.2 Aufbereitung der Instrumente und prothetischen Komponenten). 6.3 Vorbereitung der Implantate und Gingivaformer CAMLOG®/CONELOG® Implantate und Gingivaformer sind steril verpackt. Sie müssen trocken, vor direkter Sonneneinstrahlung geschützt und bei Raumtemperatur aufbewahrt werden. Die Verpackung muss vor dem Öffnen auf Beschädigungen und auf ihr Verfalldatum geprüft werden und darf erst unmittelbar vor dem Einsetzen der Produkte geöffnet werden. 7.1.2 Insertion CONELOG® A) Bissregistrierung mit CONELOG® Bissregistrierpfosten und aufgesteckten Kappen für Bissnahme • CONELOG® Bissregistrierpfosten in die zuvor gereinigten Implantate stecken. • Die Halteschrauben mit dem Schraubendreher, Inbus, handfest anziehen. • Entsprechend dem Farbcode, die Kappen für Bissnahme auf die Bissregistrierpfosten bis in Endposition aufstecken und die Okklusion prüfen. • Registrierung der habituellen Kieferrelation mit den üblichen Materialien. • Bissregistrat, Kappen für Bissnahme, und Bissregistrierpfosten (durch Lösen der Halteschrauben) entfernen und an das Dentallabor geben. • CONELOG® Bissregistrierpfosten mit den im Modell befindlichen farbcodierten CONELOG® Laborimplantaten verschrauben und Kappen für Bissnahme bis in Endposition aufstecken. • Bissregistrat auf die Kappen aufsetzen. • Gegenkiefermodell mit dem Bissregistrat verbinden und Modelle einartikulieren. B) Bissregistrierung mit Bissregistrat mit fest verbundenen CONELOG® Bissregistrierpfosten Die Herstellung eines Bissregistrats mit fest verbundenen CONELOG® Bissregistrierpfosten kann direkt auf den Implantaten im Mund oder auf dem Arbeitsmodell erfolgen. Wir empfehlen die Herstellung eines Bissregistrats auf dem Arbeitsmodell: • Bissregistrierpfosten in die im Modell befindlichen CONELOG® Laborimplantate stecken. • Die Halteschrauben mit dem Schraubendreher, Inbus, handfest anziehen. • Umlegung der Bissregistrierpfosten mit geeignetem Kunststoff, unter Gewährleistung der Zugänglichkeit zur Halteschraube. • Nach dem Aushärten Halteschrauben lösen, Bissregistrat vom Modell entfernen und an die Praxis übergeben. • Bissregistrat in die zuvor gereinigten Implantate stecken und die Halteschrauben mit dem Schraubendreher, Inbus, handfest anziehen. • Registrierung der habituellen Kieferrelation mit den üblichen Materialien. • Halteschrauben lösen, Bissregistrat mit integrierten Bissregistrierpfosten entfernen und wieder an das Dentallabor geben. • Bissregistrat mit integrierten Bissregistrierpfosten auf die im Modell befindlichen CONELOG® Labor implantate aufsetzen und handfest verschrauben. • Gegenkiefermodell mit dem Bissregistrat verbinden und Modelle einartikulieren. 7.2.3Prothetikherstellung Die Herstellung von Kronen- und Brückenversorgungen sowie abnehmbaren Prothesen erfolgt im zahntechnischen Labor. Anleitungen zur Verarbeitung unserer Produkte finden Sie in den jeweiligen CONELOG® Anwenderinformationen zur prothetischen Versorgung. 7.2.4 Eingliederung der prothetischen Versorgung • Gingivaformer entfernen. • Den Innenraum des Implantats reinigen. • Ausgewähltes CONELOG® Abutment in das Implantat einsetzten. Achtung! Das Abutment muss richtig im Implantat sitzen. Es darf kein Weichgewebe zwischen Implantat und Abutment eingeklemmt werden. • Abutmentschraube mit dem vorgegebenen Drehmoment festdrehen (siehe Produktkatalog). • Nach mindestens 5 Minuten mit dem gleichen Drehmoment nachziehen. 8 Instandhaltung und Pflege 8.1 Pflege der Implantate • Sterilverpackung des Implantats öffnen. • Implantat nur am blauen Handgriff aus der Verpackung nehmen. Warnung: Vor dem Inserieren des Implantats müssen der Silikonstopfen und die Verschlussschraube im blauen Handgriff entfernt werden! • Implantat von Hand in das Implantatbett einführen und bis zum ersten Halt einschrauben. • Handgriff abziehen. • Implantat endgültig von Hand mittels Eindrehinstrument und der Ratsche oder dem kardanischen Eindrehinstrument eindrehen. Das Implantat ist protokollgerecht eingesetzt, wenn es 0.4 mm über der Knochenkante steht und eine der Cam-Markierungen nach bukkal zeigt. Hinweis: Optional können bei Bedarf Level) gesetzt werden. CONELOG® SCREW-LINE Implantate auf Knochenniveau (Bone Bei geringer Primärstabilität Einbringpfosten mit Universal-Ringschlüssel fixieren/kontern bevor die Halteschraube gelöst wird. • Halteschraube des Einbringpfostens aus dem Implantat schrauben und Einbringpfosten entfernen. • Innengewinde des Implantats reinigen. • Verschlussschraube einsetzen. Die Wundränder werden mit atraumatischem Nahtmaterial dicht verschlossen. Die Nähte nicht zu straff knüpfen. Sie müssen so gelegt werden, dass die Wundränder über der Verschlussschraube spannungsfrei anliegen. Anstelle der Verschlussschraube kann ein Gingivaformer mit der entsprechenden Weichgewebehöhe eingesetzt werden. Dies ermöglicht eine transgingivale 1-phasige Einheilung. Der Gingivaformer muss zum Implantatdurchmesser passen und wird von Hand eingedreht. Auf exakten Sitz des Gingivaformers achten. Die Gingiva muss dicht am Gingivaformer anliegen. 7.1.3 Pflege nach der Implantation Eine einwandfreie Mundhygiene des Patienten ist eine wesentliche Voraussetzung für den Langzeiterfolg dentaler Implantate. Direkt nach der Implantation muss das Operationsgebiet von mechanischen Einflüssen weitestgehend freigehalten werden. Durch Kühlung soll eine Schwellungsprophylaxe betrieben werden. Der Patient sollte sich bei jedem für ihn unnormalen Zustand nach der Operation unverzüglich mit seiner Praxis in Verbindung setzen. 7.1.4 Provisorische Versorgung Eine temporäre prothetische Versorgung kann nur erfolgen, nachdem sichergestellt wurde, dass kein mechanischer Reiz auf das Implantat bzw. auf die Naht erfolgt. Wird eine provisorische Versorgung vorgenommen ist darauf zu achten, dass die Dentalimplantate während der Einheilphase nicht belastet werden. 7.1.5Einheilphase Die Einheilphase sollte bei guter Knochenqualität mindestens 6 Wochen, bei spongiöser Knochenqualität 12 Wochen betragen. Für Implantate mit einem Durchmesser von 3.3 mm beträgt die Einheilphase mindestens 12 Wochen. Die Werte gelten sowohl für den Ober- als auch Unterkiefer. 7.1.6 Postoperative Kontrollen Kontrollen sollten am nächsten Tag bis zu einer Woche postoperativ erfolgen. Zu beachten ist die Dichtheit der Naht und Anzeichen einer eventuell beginnenden Entzündung. Die Nahtentfernung kann nach 7-10 Tagen erfolgen. 7.1.7 Freilegen des Implantats und Weichgewebemanagement Alle Gingivaformer sind steril verpackt und sind vor dem vermerkten Verfallsdatum zu verwenden. Unsterile Gingivaformer, die keinen Kontakt mit Speichel und Blut des Patienten hatten, können resterilisiert werden. • Implantat freilegen. • Verschlussschraube entfernen. • Den Innenraum des Implantats reinigen. • Gingivaformer von Hand eindrehen. Der Gingivaformer muss zum Implantatdurchmesser und zur Weichgewebedicke des Patienten passen. • Auf exakten Sitz des Gingivaformers achten. Die Gingiva muss dicht am Gingivaformer anliegen. 7.2Prothetik Für CONELOG® SCREW-LINE Implantate stehen eigene CONELOG® Prothetikkomponenten zur Verfügung. Nur diese Komponenten sind mit der konischen Innengeometrie der CONELOG® SCREW-LINE Implantate kompatibel. Weitere Informationen zur prothetischen Versorgung mit CONELOG® Prothetikkomponenten sind in der „Gebrauchsanweisung CONELOG® Prothetikkomponenten“, Art.-Nr. J8000.0122, ersichtlich. Bei der Auswahl der Abutments muss auf den Durchmesser und die Angulation des Implantats sowie die Höhe der Gingiva geachtet werden. Es dürfen keine Angulationskorrekturen von mehr als 20° durchgeführt werden. An ein freistehendes Einzelimplantat darf kein Freiendglied angehängt werden. Achtung: Die Kontaktflächen von Abutments oder Aufbauteilen zum Implantat dürfen weder abgestrahlt noch bearbeitet werden! Folgende Punkte sind bei der Herstellung der prothetischen Versorgung zu beachten: • Günstige Belastungsverteilung, • spannungsfreier Sitz der prothetischen Versorgung auf den Abutments, • korrekte Okklusion. Zur Gerüstherstellung dürfen nur für diesen Zweck bestimmte Materialien verwendet werden. Instrumente Die Instrumente des CONELOG® Implantatsystems werden, wenn sie nicht ausdrücklich als steril gekennzeichnet sind, nicht steril geliefert. Sie müssen vor der ersten und jeder weiteren Anwendung am Patient gereinigt, desinfiziert und sterilisiert werden. Prothetische Komponenten Die prothetischen Komponenten des CONELOG® Implantatsystems werden, wenn sie nicht ausdrücklich als steril gekennzeichnet sind, nicht steril geliefert. Sie dürfen nur einmalig bei einem einzigen Patienten verwendet werden. Sie müssen vor und nach der Anwendung am Patient (z. B. zur Weitergabe an das Dentallabor) gereinigt und desinfiziert werden. Wir empfehlen eine zusätzliche Sterilisation. Ausnahmen: Die Kunststoffteile für die Abformung und Bissnahme dürfen nicht sterilisiert werden. Wichtiger Hinweis: Ausführliche Informationen zur Aufbereitung der Instrumente und prothetischen Komponenten des CAMLOG®/CONELOG® Implantatsystems sind in der „Aufbereitungsanweisung für das CAMLOG®/CONELOG® Implantatsystem“, Art.-Nr. J8000.0032, beschrieben und müssen beachtet werden! 9 Technische Daten CONELOG® CONELOG® Implantate werden aus Titan (Titan Grade 4) hergestellt. Der Einbringpfosten und die CONELOG® Verschlussschraube bestehen aus Titanlegierung (Ti6Al4V ELI). English 1 Product Description CONELOG® SCREW-LINE Implants are endosseous implants available in different lengths and diameters. They are surgically placed in the bone of maxillary and/or mandibular arch to provide support for functional and aesthetic oral fixtures in partially or fully edentulous patients. The prosthetic treatment is then completed with single crowns, bridges or full dentures, which are fastened to the CONELOG® SCREW-LINE Implants with adequate attachments. Generally there are no preferred sites for the use of CONELOG® SCREW-LINE Implants. The external surface of CONELOG® SCREW-LINE Implants is sand-blasted and acid-etched (Promote® Surface structure). The implant shoulder of CONELOG® SCREW-LINE Implants is machined. CONELOG®SCREW-LINE Implants are equipped with a cone in the inner configuration for anti-rotation and three grooves for positioning of CONELOG® abutments. CONELOG® abutments have a cone and three cams apically which fit and lock into the tapered connection and the three grooves of the implant. The CONELOG® abutment does not cover the implant shoulder. For CONELOG® SCREW-LINE Implants, specific CONELOG® components are available such as CONELOG® cover screws, CONELOG® healing caps, CONELOG® impression posts and CONELOG® prosthetic components. Important note: Only these components are compatible with the conical inner configuration of the CONELOG® SCREW-LINE Implants! CONELOG® SCREW-LINE Implants are part of the CAMLOG® Implant System. The CAMLOG® Implant System contains surgical, prosthetic, and dental laboratory components and instruments. 2Indications CONELOG® Implant System Implants are intended for immediate or delayed placement in the bone of the maxillary or mandibular arch. CONELOG® Implant System Abutments are intended for use as support for crowns, bridges or overdentures. When a one-stage surgical approach is applied, the implant may be immediately loaded when good primary stability is achieved and the functional load is appropriate. CONELOG® Implants with 3.3 mm diameter have the following additional specific indications: These are an alternative in cases where the alveolar ridge width is only 5 – 6 mm. Because of their lower mechanical strength compared with larger diameter implants, they should only be used under the following conditions: • As single implants, they should be used only to replace mandibular incisors and/or maxillary lateral incisors. • An edentulous arch can only be restored with a bar retained superstructure with at least four implants of 3.3 mm diameter without distal extensions. • Implants of Ø 3.3 mm are suitable for a partially edentulous arch when combined with implants of larger diameter for splinted superstructures. However, the limited strength of the implants with Ø 3.3 mm must be taken into account. • Avoid excessive mechanical stressing of the implants when using ball abutments in combination with Ø 3.3 mm implants. The healing time for Ø 3.3 mm implants is at least 12 weeks. CONELOG® Implants with 7 mm length have the following additional specific indications: These Implants should only be used when there is not enough space for a longer implant. Delayed loading in single tooth replacement is indicated with these implants.If the ratio of crown length to implant length is unfavorable the biomechanical risk factors have to be considered and appropriate measures have to be taken by the dental professional. 3Contraindications Precautions It is important to obtain a thorough medical history from the patient and if necessary from the general practitioner prior to conducting implant surgery in order to determine if conditions exist that will 1) make implant placement difficult because of anatomical conditions, 2) create a significant surgical or general risk, 3) impair healing capacity and/or osseointegration, or 4) lessen the likelihood of proper hygiene and/ or maintenance of the implant, abutments and fixtures. Following are examples of conditions in each category that should be considered. Some of these conditions may be relevant to more than one category. If any of these conditions or combination of conditions are severe or uncontrolled, dental implants should not be used. Achtung: Schraube nicht entfernen! • Abformung inklusive Abformpfosten entfernen. • Implantat wieder mit CONELOG® Gingivaformer oder CONELOG® Verschlussschraube verschließen. • Das CONELOG® Laborimplantat wird mit dem CONELOG® Abformpfosten verbunden. Die Cams müssen spürbar in die Nuten des Laborimplantates einrasten. Die Halteschraube des Abformpfostens mit Hilfe eines Schraubendrehers, Inbus, von Hand festdrehen. • Abformung mit Modellmaterial ausgießen. • Modellmaterial aushärten lassen. • Abformpfosten entfernen. • CONELOG® Laborimplantat mit CONELOG® Abutment versorgen. 7.2.2Bissregistrierung Das implantatgestützte Erfassen der Kieferrelation und deren Übertragung auf die Modellsituation erfolgt mit CONELOG® Bissregistrierpfosten inklusive aufsteckbaren Kappen für Bissnahme oder nur mit den CONELOG® Bissregistrierpfosten in Verbindung mit einem fest verbundenen Bissregistrat. Die Komponenten sind entsprechend dem Implantatdurchmesser farbcodiert, nicht modifizierbar und dürfen nur einmal verwendet werden. Anatomical Conditions Incomplete jaw development, difficult anatomical bone relationships, previously irradiated bone, temporomandibular joint disease and treatable jaw conditions. Surgical and General Risks Severe systemic diseases, reduced immune response and leukocytic disorders that increase the risk of infection, endocrine diseases, anticoagulation therapy/hemorrhagic diathesis, arteriosclerosis and CVA, hypertension, cardiac infarct, diseases requiring periodic uses of steroids, hepatitis, diabetes mellitus, and pregnancy. Impaired Healing Capacity Bone metabolism disorders, any disease that affects bone regeneration or microcirculation of the blood, rheumatic diseases and abuse of drugs including alcohol and tobacco. Maintenance Inadequate patient compliance, inadequate oral hygiene, periodontitis, bruxism, loss of proper functionality and oral mucosal changes. Implants with 3.3 mm diameter These are not suitable for single tooth replacements of central maxillary incisors, or of canines, premolars or molars in the maxilla and mandible. Implants with 3.8 mm diameter These are not to be used with angled abutments in the molar region. 4 Side Effects, Complications and Adverse Reactions In rare cases, it is possible that osseointegration does not occur. In such an event, consult your local representative. Immediately after dental implant insertion, activities that expose the body to high physical stress should be avoided. Possible complications after dental implant insertion may include: Temporary symptoms Pain, swelling, speech difficulties, gingival inflammation. Prolonged symptoms Chronic pain associated with the dental implant, permanent paresthesia, dysesthesia, nerve damage, exfoliation, hyperplasia, localized or systemic infection, oroantral or oronasal fistulas, loss of maxillary/ mandibular ridge bone, negative impact on adjacent teeth, irreversible damage to adjacent teeth, fractures of the implant, jaw, bone, or restoration, aesthetic problems. 5 General Safety Instructions and Warnings • An improper procedure in surgery or prosthetics can lead to implant damage or bone loss. The CAMLOG® and the CONELOG® implant system should be used only by dentists, doctors and surgeons trained in the implant system. Use of the implant system requires specialized knowledge and skills in implantology. Every patient must be thoroughly examined and evaluated on radiographic, psychological and physical parameters, including the condition of the teeth and deficits in the related hard and soft tissue that might affect the final outcome. Close collaboration between surgeon, prosthodontist and dental technician is essential for success. The CAMLOG® and the CONELOG® implant system and the related procedures were developed and clinically tested by experts in the field. The surgical component of implant treatment must be preceded by a comprehensive patient evaluation, preoperative diagnostics and treatment planning. CAMLOG®/CONELOG® products intended for single use may not be reused because safe preparation and/or functional safety cannot be ensured. 7Application Thermal trauma can prevent healing of the dental implant. Because of this, temperature elevation must be minimized to the extent possible. Take note of the maximum rotational speeds for drills and taps as follows: Round bur 800 rpm Drill Ø 4.3 mm 400 rpm Drill Ø 1.7 – 2.8 mm 600 rpm Drill Ø 5.0 mm 350 rpm Drill Ø 3.3 mm 550 rpm Drill Ø 6.0 mm 300 rpm Drill Ø 3.6 – 3.8 mm 500 rpm Taps all Ø 15 rpm During planning the surgeon must have exact knowledge of the measurement system utilized and maintain an appropriate safety margin from teeth and vital structures. Permanent damage to nerves or other vital structures can be caused if the actual drilling depth is not correctly determined relative to the radiograph and extends beyond the planned depth. Maintain a safety margin of 1.5 mm from the mandibular nerve or inferior alveolar nerve respectively. Each drill has depth markings, which must be fully observed. When smoothing the coronal bone margin, the available bone height diminishes. The implant length must be rechecked. • Incision • Optional: Chamfering/smoothing of the alveolar ridge at the selected implant position with the round bur. • Install the appropriate depth stop on the pilot and pre-drill to prevent over drilling the implant bed. Pilot drilling and pre-drilling without depth stops are suitable for implant lengths of 16 mm. If a drilling guide is being used, the depth stops can be placed after the marking procedure is complete. • Use intermittent drilling technique for pilot drilling / pre-drilling (for drilling to achieve the required depth) and form drilling (to enlarge the drilled hole to the required diameter): prepare the bone for 2 – 3 seconds, then withdraw the drill straight upward from the bone without stopping the motor. Repeat the procedure until the desired depth is obtained. • If necessary, check the depth and axial direction with a paralleling pin when performing pilot drilling and pre-drilling. • Use form drills of increasing diameter until the planned diameter for the implant bed is reached. • If the bone quality is 1 or 2 (Lekholm & Zarb 1985*), you may need to cut the thread with the tap whenever the cortical bone appears to be very dense during preparation of the implant bed. Insert the tap maximal to the upper edge of the cutting part of the instrument into the prepared implant bed. • For CONELOG® SCREW-LINE Implants, if the bone quality is 1 or 2, you can widen the apical portion of the implant bed with the form drills for SCREW-LINE Cortical Bone, which will reduce the inserting torque of the implant. m The primary packaging of the implants contains a label with the lot number, which must be recorded in the patient’s documentation or attached to it. The healing phase should be at least 12 weeks for implants with a 3.3 mm diameter. Preparation of the Patient W n ng To create optimal conditions for successful healing of the implants, the hard and soft tissue must be treated very gently. Double crown constructions are not allowed on Ø 3.3 mm implants. Immediate loading in single tooth replacement is not recommended with 7 mm length implants. They must be cleaned, disinfected, and sterilized before first use and every further use thereafter (see Section 8.2 Preparation of Instruments and Prosthetic Components). During intraoral use therefore, products should be secured with a ligature thread or dental floss to prevent them being swallowed or aspirated. • Where indications are listed for a particular product, it should be noted that any indications that are not listed are in fact contraindicated. • Small diameter implants and angled abutments are not recommended for the posterior region of the mouth. • Use in MR Environment: The CONELOG® Implant System has not been evaluated for safety and compatibility in the MR environment. La restauration prothétique définitive de l’implant ne doit avoir lieu, au plus tôt, que lorsque les tissus mous sont parfaitement cicatrisés et sains. Il est indispensable de réaliser un contrôle radiographique avant de commencer la restauration prothétique, après 6 à 12 semaines de cicatrisation. 7.2.1 Ouverture et prise d’empreinte Le transfert de la situation buccale au maître-modèle se fait à l’aide de piliers d’empreinte CONELOG® directement via l’épaulement de l’implant. La prise d’empreinte peut avoir lieu, au choix, avec un système d’empreinte fermé ou ouvert. Des piliers de prise d’empreinte CONELOG® adaptées sont fournis pour les deux variantes. Un capuchon de prise d’empreinte doit être utilisé avec le pilier de prise d’empreinte CONELOG® pour la prise d’empreinte fermée. Tous les composants présentent un code couleur correspondant au diamètre de l’implant. Le silicone ou le polyéther constituent des matériaux d’empreinte adaptés. On doit pouvoir sentir l’enclenchement de la tige dans les cames de l’implant. • Serrer à la main la vis de maintien du pilier de prise d’empreinte à l’aide du tournevis à six pans. • Placer le capuchon de positionnement sur le pilier de prise d’empreinte. Noter la position finale correcte. • Appliquer le matériau de prise d’empreinte à la seringue autour du pilier de prise d’empreinte et du capuchon de positionnement. • Remplir le porte-empreinte de matériau de prise d’empreinte et l’insérer. • Laisser durcir le matériau de prise d’empreinte. • Démouler l’empreinte, le capuchon de positionnement reste dans la masse. • Dévisser la vis de fixation et retirer le pilier de prise d’empreinte. • Fermer à nouveau l’implant avec le pilier de cicatrisation CONELOG® ou la vis de fermeture CONELOG®. • Avant le repositionnement dans l’empreinte, le pilier de prise d’empreinte CONELOG® est monté avec la vis de fixation sur l’analogue de laboratoire CONELOG®. On doit sentir l’enclenchement de la tige dans les cames de l’analogue de laboratoire. • Repositionner le pilier de prise d’empreinte avec l’analogue de laboratoire relié à lui dans l’empreinte. • Couler le matériau de modelage sur l’empreinte. • Laisser durcir le matériau de modelage. • Retirer le pilier de prise d’empreinte. • Équiper l’analogue de laboratoire CONELOG® avec le pilier CONELOG®. Prise d’empreinte avec porte-empreinte ouvert Pour cette méthode de prise d’empreinte, il existe des piliers de prise d’empreinte CONELOG®, porteempreinte ouvert. La vis de fixation du pilier de prise d’empreinte est pourvue d’un point de rupture et peut être raccourcie de 3 mm en occlusal par cassure à l’aide d’un tournevis. Avertissement: la vis de fixation doit être raccourcie avant l’insertion! Pour cette méthode de prise d’empreinte, on emploie des porte-empreintes individuels munis de perforations dans les zones de sortie des vis de fixation des piliers de prise d’empreinte, porte-empreinte ouvert. Les piliers de prise d’empreinte y compris les vis de fixation ne doivent pas toucher le porteempreinte ou les bords des perforations. • Retirer le pilier de cicatrisation ou la vis de fermeture, puis nettoyer l’intrados de l’implant. • Insérer le pilier de prise d’empreinte CONELOG® pour le porte-empreinte ouvert dans l’implant. On doit pouvoir sentir l’enclenchement de la tige dans les cames de l’implant. • Serrer à la main la vis de maintien du pilier de prise d’empreinte à l’aide du tournevis à six pans. • Appliquer le matériau de prise d’empreinte à la seringue autour du pilier de prise d’empreinte. • Remplir le porte-empreinte de matériau de prise d’empreinte et l’insérer. • Laisser durcir le matériau de prise d’empreinte. • Dévisser la vis de fixation et la tirer de la longueur du tube de guidage hors du pilier de prise d’empreinte. Attention: ne pas retirer la vis! • Démouler l’empreinte, y compris le pilier de prise d’empreinte. • Fermer à nouveau l’implant avec le pilier de cicatrisation CONELOG® ou la vis de fermeture CONELOG®. • On relie l’analogue de laboratoire CONELOG® au pilier de prise d’empreinte CONELOG®. On doit sentir l’enclenchement de la tige dans les cames de l’analogue de laboratoire. Serrer à la main la vis de maintien du pilier de prise d’empreinte à l’aide du tournevis à six pans. • Couler le matériau de modelage sur l’empreinte. • Laisser durcir le matériau de modelage. • Retirer le pilier de prise d’empreinte. • Équiper l’analogue de laboratoire CONELOG® avec le pilier CONELOG®. 7.2.2 Enregistrement de l’occlusion La prise implanto-portée de la relation maxillaire et son transfert au modèle a lieu avec des piliers d’enregistrement de l’occlusion CONELOG® équipés de capuchons d’enregistrement de l’occlusion ou seulement avec des piliers d’enregistrement de l’occlusion CONELOG® reliés à une empreinte de l’occlusion fixe. Les composants ont un code couleur correspondant au diamètre de l’implant. Ils ne peuvent être modifiés et sont à usage unique. CONELOG® A) Enregistrement de l’occlusion avec piliers d’enregistrement de l’occlusion et capuchons d’enregistrement de l’occlusion • Placer les piliers d’enregistrement de l’occlusion CONELOG® dans les implants nettoyés préalablement. • Serrer à la main les vis de fixation à l’aide d’un tournevis à six pans. • Poser les capuchons d’enregistrement de l’occlusion sur les piliers en respectant le code couleur sur les piliers d’enregistrement de l’occlusion et contrôler l’occlusion. • Enregistrement de la relation maxillaire habituelle avec les matériaux courants. • Enlever l’empreinte de l’occlusion, les capuchons pour la prise d’empreinte et les piliers d’enregistrement de l’occlusion (en desserrant les vis de fixation) et les confier au laboratoire dentaire. • Visser les piliers d’enregistrement de l’occlusion CONELOG® avec les analogues de laboratoire à code couleur CONELOG® se trouvant dans le modèle et enficher à fond les capuchons pour l’enregistrement de l’occlusion. • Placer l’empreinte de l’occlusion sur les capuchons. • Relier le modèle du maxillaire antagoniste à l’empreinte de l’occlusion et articuler les modèles entre eux. B) Enregistrement de l’occlusion avec empreinte de l’occlusion au moyen de piliers d’enregistrement de l’occlusion CONELOG® fixes La fabrication d’une empreinte de l’occlusion au moyen de piliers d’enregistrement de l’occlusion CONELOG® fixes peut être effectuée sur les implants directement en bouche ou sur le modèle de travail. Nous recommandons la fabrication d’une empreinte de l’occlusion sur le modèle de travail: • Insérer les piliers d’enregistrement de l’occlusion dans les analogues de laboratoire CONELOG® se trouvant dans le modèle. • Serrer à la main les vis de fixation à l’aide d’un tournevis à six pans. • Entourer les piliers d’enregistrement de l’occlusion avec une résine adaptée tout en préservant l’accès à la vis de fixation. • Après durcissement desserrer les vis de fixation, enlever l’empreinte de l’occlusion du modèle et la transmettre au cabinet dentaire. • Insérer l’empreinte de l’occlusion dans les implants préalablement nettoyés et serrer à la main les vis de fixation avec le tournevis à six pans. • Enregistrement de la relation maxillaire habituelle avec les matériaux courants. • Desserrer les vis de fixation, retirer l’empreinte de l’occlusion avec les piliers d’enregistrement de l’occlusion intégrés et les confier à nouveau au laboratoire dentaire. • Poser l’empreinte de l’occlusion avec les piliers d’enregistrement de l’occlusion intégrés sur les analogues de laboratoire CONELOG® se trouvant dans le modèle et la visser à la main. • Relier le modèle du maxillaire antagoniste à l’empreinte de l’occlusion et articuler les modèles entre eux. 7.2.3 Fabrication de prothèses La fabrication de restaurations prothétiques par couronnes et bridges ainsi que de restaurations amovibles est réalisée au laboratoire dentaire. Vous trouverez des instructions pour la mise en œuvre de nos produits dans les informations CONELOG® correspondantes pour restaurations prothétiques. 7.2.4 Insertion de la restauration prothétique • Retirer le pilier de cicatrisation. • Nettoyer l’intérieur de l’implant. • Insérer le pilier CONELOG® sélectionné dans l’implant. Attention! L’assise du pilier dans l’implant doit être correcte. Ne pas pincer de tissu mou entre l’implant et le pilier. • Fixer la vis du pilier en appliquant le couple prescrit (voir catalogue produit). • Après au moins 5 minutes, resserrer avec le même couple. 8 Entretien et soin 8.1 Soin de l’implant CONELOG®, Les instruments du système d’implants sauf mention contraire expresse, sont livrés non stériles. Ils doivent être nettoyés, désinfectés et stérilisés avant la première utilisation sur le patient, et avant toute utilisation ultérieure. Composants prothétiques CONELOG®, Les composants prothétiques du système d’implants sauf mention contraire expresse, sont livrés non stériles. Elles sont destinées à un usage unique sur un seul patient. Ils doivent être nettoyés et désinfectés avant et après toute utilisation sur le patient (par exemple pour le transfert au laboratoire dentaire). Nous recommandons une stérilisation supplémentaire. Exception: les capuchons en plastique pour la prise d’empreinte et le capuchon pour l’occlusion ne doivent pas être stérilisés. Misure precauzionali Prima dell’impianto chirurgico è fondamentale rilevare un’accurata anamnesi del paziente e, se necessario, richiedere un’anamnesi esterna al medico curante, allo scopo di chiarire se potrebbe verificarsi 1) una complicanza durante l’intervento implantare a causa delle condizioni anatomiche, 2) un grave problema chirurgico o un rischio di natura generale, 3) un problema di cicatrizzazione e/o di osteointegrazione, oppure 4) una compromissione della comune igiene orale e/o della cura dell’impianto, dell’abutment e della protesi. Si riportano di seguito esempi relativi a ciascuna di queste categorie, che occorre tenere in debita considerazione. Alcuni esempi possono riferirsi a più categorie. Se si verifica uno di questi casi o una combinazione degli stessi in proporzioni rilevanti o incontrollate, occorre rinunciare all’intervento implantare. Condizioni anatomiche Crescita mascellare non ancora completata, condizioni ossee anatomicamente sfavorevoli, ossa sottoposte a precedente irradiazione, patologie articolari temporomandibolari e patologie mascellari curabili. Rischi chirurgici e di natura generale Gravi patologie sistemiche, immunodepressione e disfunzioni leucocitarie che aumentano il rischio di infezioni, endocrinopatie, terapia anticoagulante/diatesi emorragiche, arteriosclerosi e ictus, ipertonia, infarto cardiaco, patologie con assunzione periodica di steroidi, epatite, diabete mellito e gravidanza. Problemi di cicatrizzazione Disturbi del metabolismo osseo, tutte le patologie che influenzano la rigenerazione ossea o la microcircolazione del sangue, patologie reumatiche e abuso di stupefacenti, alcol e tabacco. Cura Non conformità del paziente: insufficiente igiene orale, parodontite, bruxismo, abitudini parafunzionali e alterazioni della mucosa orale. Impianti con diametro di Ø 3.3 mm Sono indicati per ricostruzioni di denti singoli nella regione degli incisivi centrali del mascellare superiore, dei canini, premolari o molari sia nel mascellare superiore che inferiore. Impianti con diametro di Ø 3.8 mm Non devono essere utilizzati con abutments angolati nella regione molare. 4 Effetti collaterali In alcuni casi non avviene l’osteointegrazione degli impianti. In tal caso, rivolgersi al proprio referente locale. Subito dopo l’inserimento di impianti dentali occorre evitare attività che richiedono un elevato sforzo fisico. Possibili complicanze dopo l’inserimento di impianti dentali possono essere: Disturbi provvisori Dolori, gonfiori, difficoltà di fonazione, infiammazioni gengivali. Disturbi prolungati Dolore cronico associato all’impianto dentale, parestesia permanente, disestesia, danni nervosi, esfoliazione, iperplasia, infezioni sistemiche o localizzate, fistole oroantrali o oronasali, atrofia della cresta alveolare mascellare/mandibolare, ripercussioni sfavorevoli sui denti adiacenti, danni irreversibili ai denti adiacenti, fratture dell’impianto, del mascellare, dell’osso o della protesi, problemi estetici. 5 Avvertenze di sicurezza e di natura generale • Una procedura impropria nella tecnica chirurgica e protesica può provocare danni all’impianto o atrofia dell’osso. Il sistema implantare CAMLOG®/CONELOG® deve essere utilizzato esclusivamente da dentisti, medici e chirurghi esperti del sistema. L’utilizzo del sistema implantare richiede speciali conoscenze e abilità in implantologia. Ogni paziente deve essere sottoposto ad un’accurata visita di controllo e ad esami radiologici, psichici e fisici, che tengano conto, oltre che dello stato dei denti, anche di eventuali deficit dei tessuti duri e molli tali da influenzare l’esito finale dell’impianto. È indispensabile la stretta collaborazione fra chirurgo, protesista e odontotecnico per il successo dell’impianto. Il sistema implantare CAMLOG®/CONELOG® e le relative procedure d’impianto sono state sviluppate e clinicamente testate da personale esperto. Gli impianti CONELOG® SCREWLINE devono essere utilizzati esclusivamente in abbinamento a frese e strumenti della linea SCREWLINE e a componenti protesici CONELOG®. Per selezionare gli strumenti adatti al rispettivo diametro d’impianto si prega di fare attenzione alla codifica cromatica. • Date le dimensioni ridotte dei prodotti CAMLOG®/CONELOG®, fare attenzione al rischio di deglutizione o aspirazione. L’aspirazione potrebbe causare dispnea e, nel caso più grave, soffocamento. Per questo motivo, in caso di impiego intraorale si raccomanda di prevenire l’eventuale deglutizione o aspirazione dei prodotti, fissandoli con un semplice filo o filo interdentale. • Per quanto riguarda le indicazioni elencate per i rispettivi prodotti si rammenta che tutte le indicazioni non espressamente menzionate rappresentano controindicazioni. • Si sconsiglia l'utilizzo di impianti di piccolo diametro e di abutment angolati nell'area posteriore della bocca; • Utilizzo in Ambito RM: il sistema CONELOG® non è stato valutato in termini di sicurezza e compatibilità nell'ambito RM. Il Sistema CONELOG® non è stato per quanto riguarda il riscaldamento o la migrazione nell'ambito dell'imaging a risonanza. 6 Preparazione degli impianti e degli strumenti 6.1 Preparazione del paziente I presupposti per il successo dell’impianto sono: Requisiti locali e sistemici Normale cicatrizzazione, efficace igiene orale, edentulia parziale guarita, crescita mascellare e mandibolare conclusa, buono stato di salute generale, sufficiente disponibilità di osso mascellare sano. Diagnosi locale Anatomia della cresta mascellare, rapporti intermascellari come morso profondo, qualità e spessore della mucosa, modelli di studio e registrazione del morso in articolatore, referti radiografici. Eventuali mancanze nella valutazione del paziente, nella diagnosi pre-operatoria e nella pianificazione della terapia possono causare la perdita dell’impianto. La fase chirurgica della ricostruzione implantare deve essere preceduta da un’esauriente valutazione del paziente, diagnosi pre-operatoria e pianificazione della terapia. 6.2 Preparazione degli strumenti Salvo diversa esplicita indicazione, gli strumenti del sistema implantare CAMLOG® non sono sterili alla consegna. Prima del primo impiego e di ogni impiego successivo sul paziente, occorre quindi pulirli, disinfettarli e sterilizzarli (vedere il paragrafo 8.2 Ricondizionamento degli strumenti e dei componenti protesici). Preparazione degli impianti e delle cappette di guarigione Gli impianti CAMLOG®/CONELOG® e le cappette di guarigione sono confezionati sterili. Devono essere conservati in luogo asciutto, al riparo dai raggi solari diretti e a temperatura ambiente. Prima dell’apertura, la confezione deve essere controllata per verificarne l’integrità e la data di scadenza e deve essere aperta solo subito prima dell’impiego del prodotto. Avvertenza: i prodotti CAMLOG®/CONELOG® forniti sterili, come gli impianti e le cappette di guarigione, non devono essere utilizzati se: • è stata superata la data di scadenza (riportata sull’etichetta); • la confezione è stata danneggiata prima dell’utilizzo o era già aperta. CAMLOG®/CONELOG® Attenzione: tutti i prodotti confezionati non sterili non devono essere sterilizzati nella confezione originale CAMLOG®. Avvertenza: Salvo diverse indicazioni nelle istruzioni per l’uso, i prodotti CAMLOG®/CONELOG® riutilizzabili possono essere ricondizionati con una frequenza corrispondente a quella superata con successo dal controllo prescritto nelle istruzioni per l’uso e/o nelle istruzioni di ricondizionamento. Attenzione: I prodotti CAMLOG®/CONELOG® monouso non devono essere riutilizzati, poiché in tal caso non è garantita la sicura disponibilità e/o sicurezza funzionale dei prodotti stessi. 7.1Chirurgia Per creare le condizioni ottimali per un’efficace guarigione dell’impianto, occorre assolutamente gestire i tessuti duri e molli con tecniche conservative. Il letto implantare deve essere preparato con la massima cura. Per l’intervento chirurgico devono essere disponibili le mascherine precedentemente preparate e tutta la documentazione diagnostica. L’inserimento dell’impianto può essere eseguito con modalità transgengivale in un unico tempo oppure con modalità sommersa in due tempi. Se l’intervento implantare viene eseguito con modalità transgengivale in un unico tempo, non è necessaria la seconda fase d’intervento. Per l’intervento implantare con modalità sommersa in due tempi, prima della presa d’impronta occorre avvitare sull’impianto e lasciare in situ per tre settimane una cappetta di guarigione per il condizionamento dei tessuti molli. Nella selezione degli strumenti fare attenzione che la codifica cromatica corrisponda alla codifica cromatica del rispettivo impianto. 7.1.1 Preparazione del letto implantare Poiché la prognosi a lungo termine di un impianto e il risultato estetico migliorano con un posizionamento ottimale dell’impianto, si raccomanda di utilizzare una guida di fresaggio. Nota importante: la preparazione del letto implantare per gli impianti CONELOG® SCREW-LINE avviene con gli strumenti chirurgici degli impianti SCREW-LINE. 11 Explication des symboles Voir au verso. Italiano 1 Insufficiente volume osseo e copertura dei tessuti molli e/o qualità inadeguata dell’osso, residui radicolari locali, patologie ossee e cicatrizzazione patologica, infezione locale del sito implantare, gravi disturbi funzionali refrattari a terapia, diabete mellito non controllato, terapia immunosoppressiva a lungo termine, connettiviti/collagenosi, patologie ematiche (ad es. leucemia, emofilia), infezioni o patologie maligne intraorali, abitudini parafunzionali non controllate, disturbi occlusali o articolari non curabili, gravi disordini psicologici, xerostomia e allergia al titanio. Si sconsigliano le ricostruzioni a doppia corona su impianti di Ø 3.3 mm. Si consiglia di non effettuare il carico immediato nella sostituzione di denti singoli con impianti di lunghezza 7 mm. 7Applicazione Les implants CONELOG® sont fabriqués en titane (titane de degré 4). Les implants CONELOG® SCREW-LINE sont disponibles en différents diamètres et longueurs: Implant 3Controindicazioni 6.3 Préparation des instruments et des composants prothétiques Instruments 9 Impianti CONELOG® con lunghezza 7 mm Si raccomanda di utilizzare gli impianti CONELOG® SCREW-LINE esclusivamente quando non è presente spazio sufficiente per inserire un impianto di lunghezza superiore. Si consiglia di effettuare il carico tardivo nella sostituzione di denti singoli con questi impianti. Se il rapporto fra la lunghezza della corona e la lunghezza dell’impianto non è adeguato, il dentista deve considerare i fattori di rischio biomeccanici e adottare opportune misure. Il diametro e la lunghezza dell’impianto devono essere stabiliti in modo da prevedere una sufficiente quantità d’osso intorno all’impianto (almeno 1 mm). Occorre rispettare una distanza minima di 1.5 mm da un dente naturale adiacente e di 3 mm da un impianto adiacente. Tous les implants sont conditionnés dans un double emballage stérile et doivent être utilisés avant leur date de péremption. La restérilisation par le fabricant est également interdite. 8.2 Impianti CONELOG® con diametro Ø 3.3 mm Costituiscono un’alternativa in presenza di cresta mascellare larga solo 5 – 6 mm. Data la ridotta resistenza meccanica rispetto agli impianti di diametro maggiore, questi impianti possono essere utilizzati esclusivamente alle seguenti condizioni: • Impiego singolo limitato alla ricostruzione di incisivi nel mascellare inferiore e/o di incisivi laterali nel mascellare superiore. • In caso di bloccaggio a barra di almeno quattro impianti di Ø 3.3 mm senza prolunghe distali è possibile eseguire la protesizzazione di mascellari edentuli. • Nei mascellari parzialmente edentuli sono indicati impianti con diametro di Ø 3.3 mm in associazione ad impianti di diametro maggiore per sovrastrutture bloccate. È indispensabile, tuttavia, che si tenga conto della resistenza limitata degli impianti con diametro di Ø 3.3 mm. • Se si utilizzano pilastri a sfera in associazione ad impianti di Ø 3.3 mm, occorre evitare un eccessivo carico meccanico degli impianti. Il tempo di guarigione per gli impianti di Ø 3.3 mm è di almeno 12 settimane. Descrizione del prodotto Gli impianti CONELOG® SCREW-LINE sono impianti endossei, disponibili in diverse lunghezze e diversi diametri. Vengono posizionati chirurgicamente nell’osso del mascellare superiore e/o inferiore per ancorare restauri destinati alla riabilitazione orale, funzionale ed estetica in pazienti parzialmente e completamente edentuli. La ricostruzione protesica avviene con corone singole, ponti o protesi totali, che vengono ancorati agli impianti CONELOG® SCREW-LINE con appositi elementi. In linea di principio non esistono campi di applicazione preferenziali per gli impianti CONELOG® SCREW-LINE. La superficie esterna degli impianti CONELOG® SCREW-LINE è sabbiata e mordenzata con acido (struttura superficiale Promote®). La spalla degli impianti CONELOG® SCREW-LINE è macchinata. La configurazione interna degli impianti CONELOG® SCREW-LINE presenta un cono con protezione anti-rotazione e tre scanalature per il posizionamento degli abutments CONELOG®. Gli abutments CONELOG® sono provvisti a livello apicale di un cono e tre camme, che si incastrano nella connessione conica e nelle tre scanalature dell’impianto. La spalla dell’impianto non viene quindi coperta dall’abutment CONELOG®. Per gli impianti CONELOG® SCREW-LINE sono disponibili adeguati componenti CONELOG®, quali viti-tappo CONELOG®, cappette di guarigione CONELOG®, transfer da impronta CONELOG®, nonché componenti protesici CONELOG®. Nota importante: solo questi componenti sono compatibili con la configurazione interna conica degli impianti CONELOG® SCREW-LINE! Gli impianti CONELOG® SCREW-LINE sono parte integrante del sistema implantare CAMLOG®. Il sistema implantare CAMLOG® contiene componenti e strumenti chirurgici, protesici e da laboratorio. 2Indicazioni Gli impianti del sistema implantare CONELOG® sono studiati per l'inserimento immediato o ritardato nell'osso del mascellare superiore o inferiore. Gli abutments del sistema implantare CONELOG® sono studiati per l’uso come supporto di corone, ponti o protesi. Se si utilizza un approccio chirurgico in un unico tempo, l’impianto può essere caricato immediatamente una volta che è stata ottenuta una buona stabilità primaria e il carico funzionale è adeguato. In fase di pianificazione il chirurgo deve possedere un’opportuna conoscenza del sistema di misura utilizzato e rispettare un’adeguata distanza di sicurezza dai denti contigui e dalle strutture vitali. Se non si calcola con precisione la profondità di fresaggio sulla base delle radiografie e, quindi, si fresa oltre la prevista profondità, si possono arrecare lesioni permanenti ai nervi o ad altre strutture vitali. Occorre pertanto rispettare una distanza di sicurezza di 1.5 mm dal nervo mandibolare e dal nervo alveolare inferiore. Ogni fresa presenta marcature di profondità che vanno assolutamente rispettate. Durante la levigatura dell'estremità coronale della cresta alveolare si riduce l'altezza dell'osso disponibile, pertanto successivamente occorre verificare di nuovo la lunghezza dell’impianto. Avvertenza: se, durante un’operazione di fresaggio nel mascellare superiore, viene perforato il seno mascellare, si raccomanda di continuare il trattamento con l’ausilio di procedure di aumento osseo. • Incisione • Opzione facoltativa: smussatura/livellamento della cresta alveolare nella prevista posizione dell’impianto con la fresa a rosetta. • Applicare sulla fresa il corrispondente stop di profondità per impedire che il letto implantare venga fresato troppo in profondità. Le frese pilota e le frese iniziali senza stop di profondità sono indicate per impianti da 16 mm di lunghezza. Se si utilizza una guida di fresaggio, gli stop di profondità possono essere applicati dopo aver contrassegnato la posizione dei fori. • Tecnica di fresaggio intermittente per fresa pilota/fresa iniziale (fresaggio per raggiungere la profondità desiderata) e fresa a forma (ampliamento del foro fino al diametro desiderato): fresare l’osso per 2 – 3 secondi. Estrarre quindi la fresa verso l’alto dall’osso senza spegnere il micromotore. Ripetere il procedimento finché non si raggiunge la profondità desiderata. • Durante l’esecuzione del foro pilota e del fresaggio con fresa iniziale controllare eventualmente la profondità e l’asse con un perno parallelizzatore. • Utilizzare frese di diametro progressivamente crescente finché non si raggiunge il diametro desiderato del letto implantare. • In caso di qualità ossea 1 e 2 (Lekholm & Zarb 1985*) potrebbe essere necessario premaschiare il foro con il maschiatore se, durante la preparazione del letto implantare, emerge che la corticale è molto compatta. In tal modo è possibile rintracciare ogni impianto in caso di necessità. 9 • Aprire la confezione sterile dell’impianto. • Estrarre l’impianto dalla confezione afferrandolo esclusivamente dall’impugnatura. Attenzione: prima di inserire l’impianto è necessario rimuovere il tappo in silicone e la vite di chiusura dall’impugnatura blu. • Inserire manualmente l’impianto nel letto implantare e avvitarlo fino ad avvertire una prima resistenza. • Staccare l’impugnatura. CONELOG® • Serrare definitivamente l’impianto a mano con l'inseritore e il cricchetto oppure con l’inseritore cardanico. L’impianto sarà inserito correttamente quando sporgerà dalla cresta alveolare per 0.4 mm e quando una delle tacche delle camme sarà rivolta in direzione vestibolare. In caso di ridotta stabilità primaria, fissare/supportare l’applicatore con una chiave ad anello universale prima di rimuovere la vite di ritenzione. • Svitare la vite di ritenzione dell’applicatore dall’impianto e rimuovere l’applicatore. • Pulire la filettatura interna dell’impianto. • Inserire la vite-tappo. Subito dopo l’intervento implantare, occorre evitare il più possibile influenze meccaniche sul sito d’intervento. Applicare del ghiaccio a scopo profilattico contro il gonfiore. Dopo l’intervento, il paziente deve contattare immediatamente il proprio studio dentistico se nota eventuali condizioni anomale. m 7 7.1.7 Esposizione dell’impianto e condizionamento dei tessuti molli Tutte le cappette di guarigione sono confezionate sterili e devono essere utilizzate entro la data di scadenza indicata sulla confezione. Le cappette di guarigione non sterili che non vengono applicate nella cavità orale possono essere risterilizzate. • Esporre l’impianto. • Rimuovere la vite-tappo. • Pulire la configurazione interna dell’impianto. • Avvitare manualmente la cappetta di guarigione. La cappetta di guarigione deve corrispondere al diametro dell’impianto ed essere adatta allo spessore dei tessuti molli del paziente. • Fare attenzione al posizionamento corretto della cappetta di guarigione. La gengiva deve aderire ermeticamente alla cappetta di guarigione. m m m No mpo CR W N m m m m 7.2.1 Esposizione e presa d’impronta Il trasferimento della situazione orale sul modello master avviene con i transfer da impronta CONELOG® direttamente sulla spalla dell’impianto. La presa d’impronta può avvenire a scelta con il porta-impronte chiuso o il porta-impronte forato. Per entrambe le tecniche sono disponibili corrispondenti transfer da impronta CONELOG®. I transfer da impronta CONELOG® per porta-impronte chiuso vengono utilizzati unitamente ad un ausilio di riposizionamento. Tutti i componenti sono provvisti di codifica cromatica corrispondente al rispettivo diametro d’impianto. I materiali idonei per la presa d’impronta sono il silicone e il polietere. Presa d’impronta con porta-impronte chiuso Per questa tecnica possono essere utilizzati porta-impronte standard. • Rimuovere la cappetta di guarigione CONELOG® o la vite-tappo e pulire la configurazione interna dell’impianto. • Inserire nell’impianto il transfer da impronta CONELOG® per porta-impronte chiuso, facendo attenzione che le camme si incastrino nelle scanalature dell’impianto. • Avvitare manualmente la vite di ritenzione del transfer da impronta con un cacciavite per viti ad esagono cavo. • Applicare l'ausilio di riposizionamento sul transfer da impronta, verificando la correttezza della posizione finale. • Iniettare il materiale da impronta intorno al transfer da impronta con ausilio di riposizionamento sovrapposto. • Riempire il porta-impronte con il materiale da impronta. • Lasciare indurire il materiale da impronta. • Rimuovere l’impronta; l'ausilio di riposizionamento rimane incorporato nell'impronta. • Svitare la vite di ritenzione e rimuovere il transfer da impronta. • Sigillare di nuovo l’impianto con la cappetta di guarigione CONELOG® o la vite-tappo CONELOG®. • Prima del riposizionamento nell’impronta, avvitare il transfer da impronta CONELOG® con la vite di ritenzione nell’analogo da laboratorio CONELOG®. Le camme devono incastrarsi nelle scanalature dell’analogo da laboratorio. • Riposizionare il transfer da impronta con l’analogo da laboratorio nell’impronta. • Colare il materiale del modello nell'impronta. • Lasciare indurire il materiale del modello. • Rimuovere il transfer da impronta. • Inserire l’abutment CONELOG® sull’analogo da laboratorio CONELOG®. Presa d’impronta con porta-impronte forato Per questa tecnica sono disponibili transfer d’impronta CONELOG® per porta-impronte forato. La vite di ritenzione del transfer da impronta è provvista di un punto di rottura prestabilito, quindi può essere accorciata di 3 mm piegandola con un cacciavite in senso occlusale. Avvertenza: la vite di ritenzione deve essere accorciata esclusivamente in sede extraorale! Per questa tecnica di presa d’impronta si utilizzano porta-impronte con fori nella porzione d’uscita della vite di ritenzione del transfer da impronta (porta-impronte forato). I transfer da impronta e le viti di ritenzione non devono toccare né il porta-impronta né i margini dei fori. • Rimuovere la cappetta di guarigione o la vite-tappo e pulire la configurazione interna dell’impianto. • Inserire nell’impianto il transfer da impronta CONELOG® per porta-impronte forato, facendo attenzione che le camme si incastrino nelle scanalature dell’impianto. • Avvitare manualmente la vite di ritenzione del transfer da impronta con un cacciavite per viti ad esagono cavo. • Iniettare il materiale da impronta intorno al transfer da impronta. • Riempire il porta-impronte con il materiale da impronta. • Lasciare indurire il materiale da impronta. • Svitare la vite di ritenzione ed estrarla lungo il tubo di guida dal transfer da impronta. Debido a la menor resistencia mecánica, comparada con la de los implantes de mayor diámetro, estos implantes sólo pueden usarse del siguiente modo. • Individualmente sólo para la sustitución de los incisivos la mandíbula inferior y/o de los incisivos laterales en el maxilar superior. • En la rehabilitación protésica de mandíbulas desdentadas con una barra para ferulización de cuatro implantes de Ø 3.3 mm sin extensiones distales. • Para las mandíbulas parcialmente desdentadas, están indicados los implantes de Ø 3.3 mm si se combinan con implantes de mayor diámetro en estructuras superiores ferulizadas. En este caso es necesario tener en cuenta la resistencia limitada de los implantes de Ø 3.3 mm. • Si se emplean pilares de bola en combinación con implantes de Ø 3.3 mm debe evitarse una carga mecánica excesiva sobre los implantes. El periodo de cicatrización en el caso de los implantes de Ø 3.3 mm es de 12 semanas como mínimo. Si consiglia, comunque, di realizzare la registrazione del morso sul modello di lavoro: • Inserire i transfer per registrazione del morso negli analoghi da laboratorio CONELOG® presenti sul modello. • Serrare saldamente a mano le viti di ritenzione con il cacciavite per viti ad esagono cavo. • Applicare sui transfer per registrazione del morso una resina adeguata, garantendo l’accessibilità alle viti di ritenzione. • Dopo l’indurimento, svitare le viti di ritenzione, prelevare la registrazione del morso dal modello e consegnarla al laboratorio. • Inserire la registrazione del morso negli impianti precedentemente puliti e serrare saldamente a mano le viti di ritenzione con il cacciavite per viti ad esagono cavo. • Registrare il rapporto intermascellare abituale con comuni materiali. • Svitare le viti di ritenzione, prelevare la registrazione del morso con i transfer per registrazione del morso incorporati e consegnare il tutto al laboratorio odontotecnico. • Applicare la registrazione del morso con i transfer per registrazione del morso incorporati sugli analoghi da laboratorio CONELOG® presenti sul modello e avvitare saldamente a mano. • Collegare il modello dell’arcata antagonista con la registrazione del morso e montare i modelli in articolatore. Se implantan quirúrgicamente en el hueso del maxilar superior y/o mandíbula inferior y sirven para la colocación de las prótesis orales funcionales y estéticas en los pacientes parcial y completamente desdentados. La restauración protésica se realiza con coronas unitarias, puentes o sobredentaduras completas que se unen a los implantes CONELOG® SCREW-LINE mediante los correspondientes aditamentos protésicos. En principio no existen campos de uso preferenciales para los implantes CONELOG® SCREW-LINE. La superficie exterior de los implantes CONELOG® SCREW-LINE está chorreada con arena y grabada al ácido (superficie de estructura Promote®). El hombro de los implantes CONELOG® SCREW-LINE es liso. Los implantes CONELOG® SCREW-LINE están provistos en su configuración interna de un cono para el bloqueo antirrotacional y tres ranuras para el posicionamiento de los pilares CONELOG®. Los pilares CONELOG® están provistos en apical de un cono y tres resaltes que se encajan en la unión cónica y las tres ranuras del implante. En la unión, el hombro del implante no está cubierto por el pilar CONELOG®. Para los implantes CONELOG® SCREW-LINE hay disponibles componentes CONELOG® propios como tornillos de cierre CONELOG®, casquillos de cicatrización CONELOG®, casquillos de toma de impresión CONELOG® y componentes protésicos CONELOG®. 5 Manutenzione e cura M mm mm mm m CONELOG® Capacidad de cura reducida Trastornos en el me m m m 7 2 4 Co o Ø Ø Ø m A) Registrazione del morso con transfer per registrazione del morso CONELOG® e cappette per la presa del morso • Inserire i transfer per registrazione del morso CONELOG® negli impianti precedentemente puliti. • Serrare saldamente a mano le viti di ritenzione con il cacciavite per viti ad esagono cavo. • In base alla codifica cromatica infilare le cappette per la presa del morso sui transfer per registrazione del morso e controllare l’occlusione. • Registrare il rapporto intermascellare abituale con comuni materiali. • Prelevare la registrazione del morso, le cappette per la presa del morso e i transfer per registrazione del morso (svitando le viti di ritenzione) e consegnare il tutto al laboratorio odontotecnico. • Avvitare i transfer per registrazione del morso CONELOG® con gli analoghi da laboratorio CONELOG® codificati per colore presenti sul modello e infilare le cappette per la presa del morso fino alla posizione finale. • Applicare la registrazione del morso sulle cappette. • Collegare il modello dell’arcata antagonista con la registrazione del morso e montare i modelli in articolatore. 7.2.4 Inserimento della ricostruzione protesica • Rimuovere la cappetta di guarigione. • Pulire la configurazione interna dell’impianto. • Inserire l’abutment CONELOG® selezionato nell’impianto. Attenzione! L’abutment deve essere posizionato correttamente sull’impianto. Fra l’impianto e l’abutment non devono rimanere intrappolate parti di tessuto molle. • Serrare la vite dell’abutment con il previsto torque di serraggio (consultare il catalogo prodotti). • Dopo almeno 5 minuti riprendere il serraggio con lo stesso torque. m m m m m D m CON OG® m m OG® m U m m C 7.2.2 Registrazione del morso L’impianto supporta la determinazione del rapporto intermascellare e il relativo trasferimento sul modello può avvenire a scelta con i transfer per registrazione del morso CONELOG® incluse le cappette per la presa del morso oppure con i transfer per registrazione del morso CONELOG® incorporati nel materiale di registrazione del morso. I componenti sono codificati per colore secondo il diametro d’impianto, non sono modificabili e devono essere utilizzati una sola volta (monouso). 7.2.3 Realizzazione della struttura protesica La realizzazione di ricostruzioni a ponte e a corona nonché di protesi mobili avviene presso il laboratorio odontotecnico. Per informazioni sulla lavorazione dei nostri prodotti si rimanda alle rispettive istruzioni per l’uso CONELOG® relative alla ricostruzione protesica. m m m La ricostruzione protesica definitiva dell’impianto può essere eseguita solo quando i tessuti molli sono guariti definitivamente. Per la realizzazione della struttura devono essere utilizzati esclusivamente i materiali previsti a tale scopo. 8.2 m m Vedere a tergo. m o d mo d d D m m 711 P p ón d D m m 12 Codifica cromatica del sistema implantare CAMLOG®/ CONELOG® secondo il diametro d’impianto Attenzione: le superfici di contatto di abutments o pilastri con l’impianto non devono essere sabbiate tanto meno modificate! 8.1 m m Nella selezione degli abutments occorre considerare il diametro e l’angolazione dell’impianto, nonché l’altezza della gengiva. Non devono essere eseguite correzioni dell’angolazione superiori a 20°. Su un impianto singolo indipendente non può essere applicato un elemento in estensione. CON m m C U Modo de emp eo 10 Informazioni supplementari OG® m m 1 m 7 1C ug Español 7.1.6 Controlli postoperatori I controlli devono iniziare il giorno successivo all’inserimento dell’impianto e durare fino ad una settimana dopo l’intervento. Occorre fare attenzione alla densità della sutura e ad eventuali segni iniziali d'infiammazione. La rimozione dei punti di sutura può avvenire dopo 7-10 giorni. m B R g o d CON OG® Quando si esegue una ricostruzione provvisoria occorre accertarsi che durante la fase di guarigione gli impianti dentali non vengano caricati. 7.1.5 Fase di guarigione In caso di buona qualità ossea, la fase di guarigione deve durare almeno 6 settimane, mentre in caso di osso spongioso deve arrivare fino a 12 settimane. Per gli impianti con diametro di 3.3 mm, la fase di guarigione deve essere almeno di 12 settimane. I valori indicati si riferiscono sia al mascellare superiore che inferiore. m m m A m A n ón o p odu o CAM OG® CON OG® d h b no pu d n u do qu no po b g n un p p ón gu o gu d d d u un on m n o 7.1.4 Ricostruzione provvisoria Può avvenire una ricostruzione protesica provvisoria solo se si esclude qualsiasi sollecitazione meccanica sull’impianto o sulla sutura. 8 m m Chiudere ermeticamente i margini dell’incisione con materiale da sutura atraumatico. Non stringere eccessivamente i punti. È opportuno eseguire i punti in modo che i margini dell'incisione appoggino sulla vite-tappo senza tensione. Al posto della vite-tappo può essere impiegata una cappetta di guarigione di altezza corrispondente ai tessuti molli. Ciò consente una guarigione transgengivale in un unico tempo. La cappetta di guarigione deve corrispondere al diametro dell’’impianto e deve essere avvitata manualmente. Fare attenzione al posizionamento corretto della cappetta di guarigione. La gengiva deve aderire ermeticamente alla cappetta di guarigione. 7.2 m N m CAM OG® CON OG® Gli impianti vengono prodotti in titanio (titanio di grado 4). Gli applicatori e la vite-tappo CONELOG® sono realizzati in lega di titanio Ti6Al4V ELI. Gli impianti CONELOG® SCREW-LINE presentano una superficie Promote® micro e macro rugosa, sabbiata e mordenzata con acido. Gli impianti CONELOG® SCREW-LINE sono disponibili in diverse lunghezze e diversi diametri: Il posizionamento definitivo dell’impianto deve sempre avvenire manualmente. In caso contrario, si rischia di rovinare la filettatura esterna dell’impianto nell’osso. Nota: se necessario, in via opzionale è possibile inserire gli impianti CONELOG® SCREW-LINE a livello osseo (Bone Level).
