To receive your product, print out and mail your Activation Form, and send it via certified mail, along with the Activation, and Shipping & Handling Fee, to Customer Choice Rewards post-‐marked within 21 days of the Issue Date located on your certificate. 7. Is rush shipping available? No, unfortunately rush shipping is not available on either of these products. 8. Does my certificate ever expire? Yes, your certificate expires exactly 21 days after the Issue Date located on your certificate. 9. Do you accept personal checks for payment of any of the required fees?
