SUBJECT: PRICE LIST UPDATE/PRODUCT DELETIONS

The price list for Welch Allyn has been updated to reflect the deletion of all AED 20 series products,
as well as, all PIC 30, 40 and 50 MED products and the Propaq LT Monitors. Welch Allyn will continue to
sell all AED 10 series products.
The following clarifying information has been provided by Welch Allyn:
Welch Allyn has created a strategic alliance with ZOLL Medical to provide close cooperation in areas
of product development, quality/regulatory, service, sales, and the distribution of existing and future
defibrillator and monitor products.
The following changes will also occur:
•
Welch Allyn will be transferring portions of its defibrillator business to ZOLL, including research and
development, production, and quality and service.
•
The AED 20, PIC 30, PIC 40, and PIC 50 products will be discontinued on November 30, 2008. Welch
Allyn will honor existing and new orders of AED 20 and PIC products until November 30, 2008;
shipments are expected to be completed within 30 days of the final receipt of orders.
•
ZOLL will distribute the Propaq LT monitors to the emergency medical service markets in the U.S.,
Canada, and select international locations through its direct sales organization.

Welch Allyn will continue to provide technical support and maintenance
for discontinued Welch Allyn defibrillation products until those services
transfer to ZOLL in 2009. After the transfer, ZOLL will provide the
following for a minimum of 5 years after the transition or until
component obsolescence: sale of essential supplies and accessories,
product technical support, and factory service for discontinued Welch
Allyn defibrillation products. The firm states that all warranties will
continue to be covered. Welch Allyn issued a letter dated October 24,
2008 to this effect which is being retained by OGS for future reference.
AED 10 customers should continue to contact Welch Allyn for service, supplies and accessories.
Action Needed: Authorized Users should verify that they have received the October 24, 2008, letter from
Welch Allyn. Authorized Users should identify any affected product in their inventory. ZOLL Medical will
work with Welch Allyn customers affected by the November 30, 2008, discontinuation of the AED 20 and
PIC products. Authorized Users can contact the Welch Allyn representative as listed in the contract Award
document.

The Contractor for PC60006, Physio-Control, Inc. has issued the following statement on May 5, 2008:
Dear Customers and Partners of Physio-Control:
We have received a number of inquiries over the past few days from customers who have read in the press that
Physio-Control is "permanently enjoined" or "prohibited from shipping products." I want to provide a clear
description of our situation to avoid any potential confusion.
Physio-Control has signed an agreement with the FDA called a consent decree, which is a legal document filed with
the court. To initiate the legal process, the FDA filed a complaint and, at the same time, filed the agreement we
reached to settle the issues raised in the complaint. The agency also issued a press release about the consent decree.
In all of these documents, the FDA uses language such as "permanent injunction" and "the consent decree prohibits
the manufacture, distribution and export of specified AEDs…". These restrictions are in place until Physio-Control
satisfies various requirements outlined in the consent decree. Significantly, these restrictions do not apply in certain
circumstances. In fact, the consent decree includes very specific language which allows Physio-Control to
manufacture and ship products for customers globally under limited and specific conditions.

To be specific, the section of the decree which imposes a "permanent injunction" on Physio-Control is paragraph 4.
It states that the injunction will be in effect until the requirements listed in sections A through F of paragraph 4 have
been satisfied. We have already completed many of these requirements and are working with the FDA to expedite
the completion of the others. Significantly, the injunction in Paragraph 4 is qualified in Paragraph 5 of the consent
decree which begins "paragraph 4 shall NOT apply to" and then lists the exceptions over the next five pages of the
document. The consent decree will be publicly available as soon as it is entered with the court.
In summary, Physio-Control is clearly permitted to manufacture and ship a limited amount of product for emergency
providers under certain conditions according to the consent decree, a document signed by both Physio-Control and
the FDA. I assure you that the decree also allows for Physio-Control to provide disposables, accessories and service
to our customers globally.
We are pleased to reach this stage of the process with the FDA, as it prescribes a formal process for the remaining
actions that need to be taken for us to resume unrestricted shipping. The best people in our company are working
full-time on the additional improvements to our quality system as their top priority.
At Physio-Control, there is nothing more important to us right now than clear communication with our customers.

The contractor for PC 60008 WBE, National Instruction for Cardiovascular Education (N.I.C.E.), has issued the
following information regarding a voluntary recall.
Defibtech Announces a Worldwide Voluntary Recall of
Lifeline and ReviveR Automated External Defibrillators
FOR IMMEDIATE RELEASE – Guilford, CT -- March 6, 2007 – Defibtech, LLC, is initiating a voluntary worldwide
recall of the Lifeline AED® and ReviveR AEDTM (semi-automatic external defibrillators). This recall affects all Lifeline
and ReviveR AEDs with software versions 2.002 and earlier. The self-test software for these devices may allow a selftest to clear a previously detected low battery condition. If this situation occurs, the operator may be unaware of the low
battery, and the device may be unable to deliver a defibrillation shock, which could result in failure to resuscitate a
patient.
Defibtech initiated notification of its distributors and customers by letter on February 22, 2007. Defibtech determined
the need for this recall after learning of three reports of malfunctions from end users. The company has provided a
maintenance procedure that can be used to verify functionality of the device until the software upgrade has been
installed, allowing the device to remain in service. A copy of this maintenance procedure is being mailed to customers.
This procedure, as well as instructions on determining the software version of a unit, can be found on the

Defibtech will provide customers with a free software upgrade to address this issue for all affected AEDs. This software
upgrade will be able to be installed in the field where the unit is located. The software upgrade is expected to be
available within the next 10 weeks.
Defibtech has distributed approximately 42,000 units worldwide through distributors to end users including: schools,
fire & EMS, businesses, health clubs and hospitality companies. The products can be identified by the words “Lifeline
AED®” and “ReviveR AEDTM” on the front of the device.
The Food and Drug Administration (FDA) has determined that this action is a Class I recall. Class I recalls are the most
serious type of recall and involve situations in which there is a reasonable probability that, should this particular
malfunction occur, use of the affected product may cause serious injury or death if the problem is not corrected. Any

What is the problem?
The AED software (version 2.002 and earlier), under some conditions, detects a low battery, but then mistakenly clears
the low battery warning.
Why is this an issue?
The AED’s active status indicator (ASI) light can flash green and the voice prompt will report a battery pack ‘OK’
when the rescue battery is actually low or depleted. If the battery is low or depleted the unit may not be able to provide
enough power to deliver a shock during a rescue.
What is Defibtech doing about this?
We are implementing a field corrective action. This means that we are releasing a mandatory software upgrade that
must be applied to all Defibtech AEDs software version 2.002 and earlier. This applies to all Defibtech AEDs that
were shipped before February 15, 2007. This upgrade can be performed in the field where the AED is currently located.
Is this a recall?
Specifically, this is a “field corrective action” which means that AEDs can be upgraded in the field without having to
ship the AEDs back. However, the FDA still classifies this as a type of recall.
What is meant by a “field corrective action”?
A field corrective action means that the correction to the device can be made in the field where the unit is located. This
issue will be corrected by doing a simple software upgrade to the unit where it is located.
Is this Defibtech’s first corrective field action or recall?
Yes, this is Defibtech’s first corrective field action. Defibtech has been in business since 1999.
Is Defibtech the only AED manufacturer with a corrective field action or recall?
No, all other defibrillator manufacturers have had numerous corrective actions or recalls. For example, last year there
were at least 11 recalls for external defibrillators by other manufacturers. This is Defibtech’s first corrective action. To

Did the FDA make you do this?
No. Defibtech discovered this issue and decided on its own to voluntarily institute this action. We have told the FDA
about it and we are working with them to ensure that the issue is corrected.
Is Defibtech issuing a Press Release announcing this field action?
Yes, Defibtech has issued a press release to publicly announce this field action.
Does this mean your customer’s AED won’t work properly?
No. This issue just affects how the software detects a low battery condition. If the battery is not depleted the unit will
function normally. Until your customers unit is upgraded, customers should follow the recommended maintenance
procedure found in the customer notification letter and on www.defibtech.com/fa2007.

How do customers know if their battery is ok?
The new software upgrade will correct the issue. Until then, customers can ensure that their battery pack is rescue ready
by performing a manual self-test as described in the customer notification letter.
Do customers have to take their AED out of service?
No. Until the software upgrade is available, customers should perform the manual self-test as described in the customer
notification letter to ensure that their AED is rescue-ready.
What if the manual self-test shows that the battery is low, and the voice prompt recommends replacing the battery
pack?
If the manual self-test shows that the battery is low, and the voice prompt recommends replacing the battery pack, then
the battery pack should be replaced (stock # DCF-200 for 5-year battery pack or DCF-210 for seven-year battery pack).
What is my distributor responsibility to communicate this field action to my customers? What is my responsibility to
facilitate this field action for my customers?
You must provide Defibtech with your latest tracking data for units sold to customers. Defibtech will directly mail a
notification to your customers after you supply us with this information.
When the software upgrade is available, you will have the option of handling your customers’ upgrades directly or
Defibtech will do it for you. The Software Upgrade Responsibility Document lists three field corrective options offered
to distributors:
o Option A – Distributor manages field software upgrade;
o Option B – Defibtech manages the field software upgrade;
o Option C – Defibtech and Distributor share the field software upgrade responsibility;
When will the software upgrade be available?
Defibtech is targeting to have upgrade cards available to distributors (to upgrade their own AEDs, as well as customer
AEDs if they choose to facilitate their customer upgrades themselves) in late March or April. Of course, this schedule
may change due to unforeseen circumstances.
How long will it take to upgrade an AED?
The actual upgrade process is quite simple and can be done in less than five minutes at the location where the AED is
located.
Does this mean my battery is bad?
No. This issue applies to the software in the unit that detects that the battery is low.
Is this covered by the battery warranty?
No. This action relates to the software detecting that the battery is low. The battery can become low for all kinds of
reasons: actual unit use, demonstration use of the unit, frequent manual testing, normal battery aging, etc. If there is an
actual defect in the battery within three years, that would be covered under the battery warranty.

Does this mean that Defibtech AED batteries do not really last five years (or seven years for the DCF-210) as
marketed?
No. This is a software issue that causes the software to indicate that the battery is low when it actually has capacity left
in it.
How do I know what software version I currently have?
To determine the software version of a unit, turn the unit on, wait until the unit begins speaking, and then press and hold
the on/off button for approximately five seconds until the unit begins speaking again. Prior to automatically turning
itself off, the unit will speak the version number; if the unit does not speak a version number, the software version is
1.201.
Do I have to do anything to get the new software?
No. Defibtech will provide you with new upgrade software cards when they are available.
How will the software get to customers?
If you choose not to handle your customer upgrades, Defibtech will send each customer a software upgrade package
with instructions, an upgrade data card and an upgrade reply card to fill out and return.
What has to be done to upgrade the unit?
The data card must be inserted in the unit and the upgrade procedure must be run according to the instructions. The
whole process will take less than five minutes. Then the serial number must be recorded on the reply card. Anyone
upgrading their AED, be it a distributor or an end-customer, must then mail back the data card and the reply card in the
supplied prepaid envelope.
Can I call Defibtech to help me with my upgrade?
Yes. You or your customers can call Defibtech at 1-877-453-4507, 8:30 A.M. – 5:30 P.M. (Eastern), Monday-Friday.
Defibtech customer service representatives will gladly guide you through the process over the phone.
I have more than one unit; do I need an upgrade card for each unit?
No. Each data card can be used to upgrade up to 50 units.
How many AED upgrade software cards will I get if I choose to facilitate the upgrade process myself with my
customers (Option A)?
Your Defibtech Account Manager will work with you to send you the appropriate number of cards so that your
customer AEDs can be upgraded.
My address has changed; what should I do?
Please fax your new address and a list of AED serial numbers that you control to 203-738-1072.
What AHA protocol will be in the new software?
The new software upgrade will automatically bring the upgraded unit up to the latest 2005 AHA rescue protocol for
free.

Will I have to do this for new units I buy?
No. All units being shipped from now on from the Defibtech factory will already have the new software installed.
When should I expect to receive the current AED order(s) I already placed with Defibtech but have not yet
received?
Defibtech expects to be shipping current AED orders in our system between March 15-April 1, 2007. Of course, this
schedule may change due to unforeseen circumstances.
What is the expected lead time for AED orders that I place today?
Expect that new AED orders you place today or over the next few weeks will ship between April 1 - April 15. Of
course, this schedule may change due to unforeseen circumstances.
Does this action affect AED accessories?
No, this field action does not affect Defibtech AED accessories, and orders can be placed for accessories as normal
through [email protected] or via fax at 203-738-1026.
Should I still continue promoting Defibtech AED sales to my customers during this field action process?
YES, Defibtech expects to be shipping AEDs and have the software upgrade complete and ready for mailing to existing
AED customers soon. By all means continue your Defibtech AED marketing efforts as you would normally. As usual,
please let your Defibtech Account Manager or Defibtech Customer Service know if we can assist you in any way to

We are writing to notify you that Physio-Control, a wholly-owned subsidiary of Medtronic, Inc.,
has initiated a voluntary suspension of U.S. shipments of LIFEPAK external defibrillators and
certain associated products manufactured at its facility in Redmond, WA. The decision to suspend
U.S. product shipments was made to address issues identified in Physio-Control’s quality system
that were identified by Physio-Control, Medtronic and the FDA. This action is not directly related
to a specific product issue and does not impact Physio-Control products currently in use worldwide.
We have informed the FDA of our decision and intend to work with them to address these issues so
that these products can be returned to the market.
Physio-Control continues to be committed to providing the highest quality products into the
marketplace. We are confident this voluntary action will allow us to focus the appropriate resources
on rectifying these issues to ensure we produce utmost quality products.

FDA Announces Guidant’s Class I Pacemaker Recall
The U.S. Food and Drug Administration (FDA) is notifying health care providers and patients that Guidant Corporation
is voluntarily recalling certain pacemakers. A seal within the devices can leak, allowing moisture to affect the electronic
circuits. This defect can cause the pacemakers to fail to provide pacing or can cause a rapid heart rate. Other unexpected
device behaviors are also possible. The problems may occur without warning and can lead to loss of consciousness, and
possibly heart failure and death.
Only the following models are affected by this recall. All were manufactured between November 25, 1997 and October
26, 2000.

Guidant announced the initiation of a voluntary recall on July 18, 2005. The recall action consisted of a letter to
physicians that describes the problem and provides recommendations about how to minimize the risk of pacemaker
failure. These products, which are of an earlier generation design, have not been sold or implanted for the last four
years.
Guidant has determined that a hermetic sealing component used in the subset of devices listed above may experience a
gradual degradation, resulting in a higher than normal moisture content within the pacemaker case late in the device's
service life.
The FDA has classified Guidant's action as a Class I recall. Recall classifications can fall into one of three categories,
with Class I being the most serious. These numerical classifications are based on the probability that the device failure
could lead to adverse health effects. In a Class I recall, there is a reasonable probability that the malfunctioning device
will cause serious adverse health consequences or death.
The FDA concurs with Guidant's proposed recommendation to patients.

FDA Updates Consumers on Guidant Corporation’s Implantable Defibrillators
As the Food and Drug Administration (FDA) continues to evaluate the safety and performance of certain implantable
defibrillators manufactured by Guidant Corporation, the Agency has now classified the recalled devices. This additional
information on the relative health risks of the devices will help patients and doctors take appropriate action, if
necessary.
Classifications can fall into three categories, with Class I being the most serious. These numerical classifications are
based on the probability that the device failure could lead to adverse health effects.
Below is a full list of the 11 devices affected by this recall classification.

VENTAK PRIZM AVT, VITALITY AVT, and RENEWAL AVT Devices- Class II
FDA has classified the previous actions taken by Guidant for these devices as a Class II recall. For a Class II recall, the
malfunctioning product may cause temporary or medically reversible adverse health consequences, however the
probability of serious adverse health consequences is remote.
These Guidant devices are subject to a memory error which, in rare cases, may limit available therapy. Of the 21,000
devices implanted worldwide (18,000 in the U.S.), two incidents have been confirmed, neither of which resulted in
death or injury. The defect can be detected by medical evaluation of the device and Guidant is recommending the device
be reprogrammed during the patient’s next doctor visit. Guidant is developing an additional non-invasive software
solution for this problem, which is expected by the end of the year.
CONTAK RENEWAL 3 and 4, RENEWAL 3 and 4 AVT, and RENEWAL RF Devices- Class II
FDA has classified the previous actions taken by Guidant regarding these devices as a Class II recall.
These devices are subject to a component failure that in rare cases may limit available therapy. A magnetic switch in
these devices may become stuck in the closed position, which in some cases inhibits the device’s ability to treat
ventricular or atrial tachyarrhythmias (abnormally fast heart rhythms) and also accelerates battery depletion. Four
occurrences have been confirmed out of approximately 46,000 devices; a fifth occurrence is suspected but cannot be
confirmed. In the four confirmed cases, patients and/or physicians were alerted to the condition by audible device tones
signaling that the magnetic switch was closed. Based on this information, it is important that patients who hear tones
from their device immediately contact their physician or go to the hospital emergency room.
As a precautionary measure, Guidant has recommended that physicians discontinue implanting these devices until

Medtronic, Inc. has announced that it is voluntarily recalling a limited number of LIFEPAK® 500 automated external
defibrillators (AEDs). Only certain monophasic LIFEPAK 500 AEDs manufactured in 1997 are included in this action.
The FDA has been apprised of this action. The U.S. Food and Drug Administration (FDA) classified this action as a
Class I recall. The FDA defines Class I as a situation in which there is reasonable probability that the use of or exposure
to the product will cause serious adverse health consequences or death.
The AED may continue to display a "connect electrodes" message and may not analyze the patient's heart rhythm even
when the electrodes are properly connected. Failure to analyze the patient's heart rhythm will inhibit defibrillation, if it
is needed. This action affects first-generation LIFEPAK 500 AEDs that were manufactured in 1997, which represents
approximately 1 percent of LIFEPAK 500 AEDs currently in use worldwide. This action does not affect any other
LIFEPAK 500 AEDs currently produced, or any other LIFEPAK product.

determine if your device is included in the action. Attached is a .pdf file with a list of serial numbers.
Medtronic has received 54 incident reports with this specific group of LIFEPAK 500 AEDs, including eight instances
where it may have prevented patient resuscitation, which corresponds to less than 1 percent of patient uses. In addition,
a recently completed theoretical engineering analysis estimates that this issue may occur on up to 8 percent of patients.
Medtronic began distributing notices via certified mail on Feb. 3, 2005, to customers who purchased AEDs in this group
of devices. The company will update or upgrade their device at no charge by May 31, 2005. The affected AEDs may
remain in service and customers are currently being contacted with recommendations for use and replacement
schedules.

Access Cardiosystems, Inc. Issues a Worldwide Recall of Automated External Defibrillators (AED'S)
The Office of General Services is pursuing information regarding any recourse available for contract users who may
have acquired the recalled products and are getting no response from the company. We ask that any contract user who
has acquired such products please contact this office so we may keep you informed about future developments. No
new orders should be placed with this company.
The following information in italics was received as part of the notice from Access Cardiosystems regarding the FDA
recall.
Contact: Recall Coordinator - 1-978-405-1057
Access CardioSystems, Inc., Concord, Massachusetts, initiated a voluntary recall of all of its Automated External
Defibrillators (AED's). Additionally, it appears the Company is no longer doing business.
We have identified two potential problems with certain AED's that warrant your immediate attention:
1. Potential Failure of the Shock Delivery Circuit:
The Company is aware of a situation involving certain of its AEDs in which the device may experience a catastrophic
failure of the shock delivery circuit. The Company's investigation indicates to date that this failure mode is restricted to
a specific batch of one device component. To date, the company has received 11 complaints of this occurrence in
devices containing the component shown to be associated with this failure mode (representing a 0.8% complaint rate
within the affected units). When this potential problem occurs, it is not possible to deliver additional defibrillation
shocks.

Although the investigation of this issue is still ongoing, the Company has determined that AEDs with the following
serial numbers may have this problem: 075690 - 077140.
2. Potential of the AED to Turn on Unexpectedly:
The Company is aware of a situation involving certain of its AEDs in which the "ON/OFF" button of the device may
become inoperative after the device turns on unexpectedly. The Company's investigation indicates to date that this
failure mode is related to a specific manufacturer of a specific device component. To date, the Company has received
33 complaints of this occurrence in devices containing the component (representing a 0.3% complaint rate within the
affected units), none of which have involved a patient treatment. If this potential problem occurs, the device may not
defibrillate.
Although the investigation of this issue is still ongoing, the Company has determined that AEDs with the following
serial numbers may have this problem: 075180 - 084760.
USERS ARE ADVISED TO IMMEDIATELY DISCONTINUE USE OF AND TO REMOVE
FROM SERVICE ACCESS CARDIOSYSTEMS AED DEVICES WITH THE ABOVE
REFERENCED CATALOG AND SERIAL NUMBERS: 075690 - 077140 OR 075180 - 084760.
The Company discontinued manufacturing and marketing ALL models of its AEDs and discontinued supporting its
AEDs that are currently in service. The Company cannot accept orders for new AEDs, for consumable components
(specifically disposable battery packs and electrode sets) used with its AEDs, and cannot service, repair or answer
technical questions for existing AEDs.
For units not affected by the recall Access Cardiosystems is no longer accepting orders for disposable parts used with
our AEDs. Therefore, when your supply of disposable parts is depleted, please immediately discontinue use of and
remove from service all of the Company's AEDs that you have in your possession. Customers should consider replacing
the AED's as soon as possible. It is your responsibility to equip yourself with AEDs that meet your medical needs.
Access CardioSystems, Inc. has notified affected customers on November 3, 2004 by registered mail return receipt
requested mail. All interested parties with any questions should contact the Recall Coordinator at 1 978 405-1057.

This recall is being conducted with the knowledge of the US Food & Drug Administration.
