SIMVASTATIN-ELUTING CORONARY STENT SYSTEM
Cordynamic® IRIST SMALL STENT
INSTRUCTIONS FOR USE
Sterilized with ethylene oxide gas. Non Pyrogenic. For single use. Do not re-sterilize. Inspect package integrity. Use before the expiry date.
Store in a dry, cool place, protected from light.
1. Description of device
The simvastatin-eluting coronary stent system, IRIST SMALL, is a drug-eluting stent expandable by premounted balloon, that includes:
• A medical-grade 316 L Apolo Small stainless steel stent including a drug (a formulation of simvastatine contained in a polymer coating). The coating
is made of a polymer carrier loaded with 0.36 µg/mm2 of simvastatina in a slow-release system, with a maximum nominal amount of 56 µg in the
largest stent (2,75 mm x 36 mm).
• Semi-distensible, fast-exchange dilation balloon catheter, with high-pressure support.
• The balloon contains radiopaque markers for a precise stent placement, and is designed to provide a segment expandable to known diameter
and length, with application of a specific pressure.
• The stent is supplied in various lengths (9, 14, 19, 23, 28 and 36 mm) and diameters (2.0, 2,25, 2,50, 2,75 mm).
Description of the drug
The stent that is part of the IRIST SMALL coronary stent system is a stent coated with a drug formulation and a polymer containing simvastatin (drug
substance) and polymer carrier P-5 (non-active component).
Simvastatin
The active pharmaceutical compound of the IRIST SMALL stent is simvastatin. Simvastatin is a white or off-white crystalline powder with molecular
formula C25H38O5 and a molecular weight of 418.6. It is a drug the use of which has been approved for the treatment of hypercholesterolaemia and
for cardiovascular prevention.
P-5 polymer carrier
The polymer P-5 is an inactive ingredient of the IRIST SMALL stent. It is an elastomeric acrylic copolymer, highly flexible and hydrophobic. The polymer
is mixed with the drug simvastatin and then is applied to the stents, obtaining a coating with the drug and polymer throughout the luminal and abluminal
surface of the stent.
2. Indications and use
The IRIST SMALL simvastatin-eluting stent is indicated for improving luminal coronary diameter and for reducing the re-stenosis occurring inside and
at the edges of the stent during the treatment of de novo lesions in small coronary arteries with a reference vessel diameter of 2.0 mm to 2.75 mm.
The stent is indicated for lesions of lengths below the nominal length of the stent used (9, 14, 19, 23, 28 or 36 mm).
General inclusion criteria
1. Eligible for percutaneous transluminal coronary intervention (PTCI).
2. Candidate acceptable for coronary artery by-pass graft (CABG).
Angiographic inclusion criteria
1. Target lesion to be treated is located inside the native coronary vessel.
2. Stenosis of the diameter of the lesion to be treated 50% by visual estimation, or 50% by estimation using automatic quantitative angiographic
analysis (QCA).
3. Contraindications
The implantation of the IRIST SMALL stent system is contraindicated and must be excluded in patients meeting any of the following criteria:
General exclusion criteria:
• Known sensitivity to simvastatin.
• Known allergy to stainless steel.
• Allergy to drugs and/or materials used during the procedure.
• Contraindications to anticoagulation and/or antiplatelet therapy.
• The function of the left ventricle of the patient is seriously affected (< 30%).
• Direct stent implantation.
• The patient suffered a myocardial infarction in the 48h prior to the procedure with IRIST SMALL.
• Cardiogenic shock.
• Known or probable presence of intraluminal thrombus.
• Intra-stent re-stenosis.
• Bifurcation lesions.
• Implantation of a stent in saphenous vein grafts.
Angiographic exclusion criteria:
• Lesions over 50% located in the unprotected left common coronary tree.
• The lesion to be treated or the proximal vessel are severely calcified.
• Complete chronic occlusion of the vessel to be treated.
• The lesion to be treated shows a severe tortuosity, not adequate for stent insertion and deployment.
4. Warnings and precautions
Warnings
• The use of this product involves the risks associated with subacute thrombosis, vascular complications or bleeding events. Therefore, patients
should be carefully selected, and double antiplatelet treatment (ASA and clopidogrel or ticlopidine) be prescribed. In the clinical trial the antiplatelet
treatment was administered for 6 months post-procedure; therefore the treatment can never last less than 6 months, and should conveniently last at
least 12 months following the procedure in patients with low risk of bleeding. Aspirin should be administered indefinitely.
• It is very important to provide the appropriate information to the patient on the antiplatelet advice subsequent to the procedure. The early
discontinuation of the antiplatelet drug prescribed will increase the risk of thrombosis followed by myocardial infarction or death.
• This device must not be applied to patients with confirmed hypersensitivity to simvastatin or to 316 L stainless steel.
• The occurrence of subsequent restenosis can require new dilation of the artery segment containing the stent. For the moment, the result of repeated
stent dilation is unknown.
• It has not been evaluated the overlapping of Irist Small stent. Even so, when more than one stent is required, if the placement involves contact
between them, the material of both stents must be of similar composition.
• Do not use stents of different compositions to expand the same vessel, since the risk of thrombosis increases particularly in small arteries
o The stent implantation must be only performed at hospitals with emergency means or hospitals with easy access to other hospitals that can perform
open heart surgery in case of complications.
• The implantation must be only performed by physicians highly acquainted and trained on the technique of percutaneous transluminal coronary
angioplasties and stent deployment.
Precautions
Handling the stent - Precautions
• Single-use device. DO NOT RE-STERILISE. DO NOT RE-PROCESS. Respect the expiry date of the package.
• Before use, choose the correct size of the stent to be used and examine it.
• Do not remove the stent from the carrier balloon in which it is supplied. The stent is not designed to be mounted on another balloon. Removing
the stent from its insertion balloon can damage the stent and/or its coating or cause an embolism in it.
• Inadequate handling can loosen stent contact with balloon and also damage the coating.
• Carefully remove the catheter from the package, place it on the guide and advance it through the hemostatic valve adapter and the guide catheter's
connector, avoiding manipulating the stent or displacing it in any way from its position in the insertion device.
• Do not use air or gas media to inflate the balloon.
• Do not use organic solvents or oil-based (such as contrast mediums Ethiodol or Lipiodol) or highly viscous products.
• Contact between the stent and fluids before placement is not recommended, as drug release could occur. However, if completely necessary, irrigate
the stent with isotonic / sterile saline, with a maximum contact time of 1 minute.
• If the stent cannot be deployed, remove the entire system carefully and do not handle the stent with bare hands.
Placing the stent - Precautions
• Do not prepare or pre-inflate the balloon before stent deployment in a manner different than that indicated. Follow the balloon purging technique
described in the Section Instructions for the operator.
• Implanting a stent can cause dissection of the vessel distal or proximal to the stenosed area and can cause acute closure of the vessel requiring
additional intervention (e.g. CABG, greater dilation, placement of additional stents, etc.)
• It is recommended to sufficiently predilate the lesion to be treated before stent implantation. The direct implantation of stents has not been evaluated.
• If an unusual resistance is noticed at any time during access to the lesion before stent implantation, the stent and catheter guide system should
be removed as a single piece.
• The device must be handled under high-quality fluoroscopic observation. Do not advance or take back the catheter unless the balloon is completely
deflated under vacuum. If resistance is found during handling, find out the cause of it before continuing.
• Do not deploy the stent until it has been adequately positioned in the stenosis. Observe the position through the radiopaque marks.
• Stent placement can affect the patency of a lateral branch.
o Balloon pressure should not exceed the rated burst pressure. The use of a pressure control device is recommended to prevent exceeding the
balloon burst pressure. At least 99.9% of the balloons (with a reliability of 95%) will not burst when they reach the level (or below) of the maximum
recommended pressure. The use of higher pressures than specified can cause balloon rupture and possible damage in the intima and dissection.
• The stent removal methods (additional guides, loops, forceps) can cause an additional vascular injury. The complications include bleeding, bruising
or pseudoaneurysm.
Removal of the stent system - Precautions
• If the stent could not be deployed, remove the entire system carefully, particularly when removing to the guide catheter to prevent the stent from
getting trapped in the tip of the guide catheter.
• If any resistance is noticed, remove the entire system, including the guide catheter, as a single unit. This must be done under direct fluoroscopic
control view. In this case, follow these steps:
- Refrain from removing the positioning system inside the guide catheter.
- Position the proximal radiopaque mark of the balloon facing the distal edge of the guide catheter.
- Advance the guide in the coronary anatomy as distant as possible.
- Tighten the rotary hemostatic valve and remove the entire set carefully as a single unit.
- If these steps are not followed or an excessive strength is applied to the stent system, loss or damage can occur in the stent or the components of
the stent system, such as the balloon.
Post- implant - Precautions
• Caution should be exerted when crossing a stent just deployed with an intravascular ultrasound catheter or a balloon catheter, as the geometry
of the stent or its coating can be affected.
• After recent implantation of a stent, avoid magnetic resonance procedures in the patient to reduce the possibility of stent movement. Wait for 2-3
months to ensure stent endothelization.
• It is necessary to prescribe a double antiplatelet treatment (AAS and clopidogrel or ticlopidine) for at least 6 months after the procedure and
preferably for 12 months in patients with low risk of bleeding, in order to reduce the risk of thrombosis in the stent area. Aspirin should be administered
indefinitely.
Drug interactions
• The potential interactions of simvastatin with the drugs administered concomitantly have not been investigated. Drug interactions at the systemic
levels of simvastatin are detailed in the product data sheet but, considering that the IRIST SMALL stent has simvastatin levels that are 1000 times
below the doses used in systemic treatments of the drug, these interactions are not to be expected.
• In the specific case of the joint oral administration of a statin, the calculations performed show no foreseeable increase in the risk of toxicity in
patients treated with the IRIST SMALL Stent who are also receiving oral statins.
Pregnancy
• This product has not bee assessed in pregnant women or in men intending to father children, so recipients of the device are recommended to
avoid pregnancy. Although there are no contraindications, the risks and effects on the reproduction are still unknown.
5. Potential adverse events
The following adverse events can be associated, amongst others, with the placement of a coronary stent in the native coronary arteries:
• Angina
• Arrhythmia, including ventricular fibrillation (VF) and ventricular tachycardia (VT)
• Blood pressure changes
• Cardiogenic shock
• Pulmonary shock or oedema
• Abrupt stent closure
• Emergency coronary artery by-pass surgery (CABG)
• Pericardial effusion
• Dissection, perforation, rupture or lesion of the vessels

Other potential adverse events, not above indicated, that can be caused solely by the coating with simvastatin are:
• Myalgia
• Increase in serum transaminases and CK levels
• Blood and lymphatic system disorders (anaemia)
• Nervous system disorders (headaches, dizziness, peripheral neuropathy)
• Gastrointestinal disorders (constipation, abdominal pain, dyspepsia)
• Hepatobiliary disorders
• Skin and subcutaneous tissue disorders
6. Supply form
Sterile: This product is sterilized with ethylene oxide and is non-pyrogenic. For single use. Do not use if the package is not integer.
Content: A simvastatin-eluting stent system, IRIST SMALL.
A handbook with instructions for use.
A table with balloon compliance chart.
Storage: Keep at room temperature, in a dry, cool place, protected from light.
Instructions for disposal: After using it, dispose of the device and its package following hospital, administrative or local government rules.
7. Instructions for the operator
Inspection prior to use
• Inspect the package carefully before opening. Do not use after the “expiry date”. If any possible damages are seen in the package, do not use
the product and contact Iberhospitex to obtain information on how to proceed. Do not use if any defect is seen.
• At any time during its use, if you see that the proximal stainless steel body has bended or kinked, do not continue using the catheter.

Preparation
• Prepare the inflation device with the recommended contrast medium according to the manufacturer's instructions. Purge the system of air.
• Select adequately the stent in terms of length and diameter. The balloon diameter (stent) must not exceed the diameter of the artery to be dilated
before and after the stenosis.
• Leave the catheter inside the protection tube. Remove the distal edge of the protection ring to see the stent.
• Check that the stent is secure between the balloon marks, do not touch with the fingers as it could be damaged.
• Prepare the dilation catheter for purging. Fill the 20 cc syringe with luer closure with 3 cc of the appropriate contrast solution.
WARNING: Do not use the pre-inflation technique for purging the balloon lumen. Do not use air or any other gas media to inflate the balloon. Do not
take negative pressure from the inflation device.
• Connect the syringe to the balloon lumen, position the catheter with the distal tip and the balloon faced vertically downwards.
• Apply a negative pressure and suction for 5 sec. Release the plunger slowly.
• Remove the syringe from the balloon inflation lumen and remove all air.
• Connect again the syringe to the catheter and keep a negative pressure in the balloon until no more air returns to the device. Release syringe
pressure to the neutral point and disconnect it.
• Connect the inflation device to the catheter inflation port without entering air into the system.
WARNING: Avoid applying a positive pressure to the release system; keep the inflation device at environmental pressure.
Insertion technique
• Insert the guide catheter by standard techniques.
• Predilate the lesion with a balloon of an adequate diameter, with a 1:1 ratio with the vessel diameter. After predilating the lesion, leave the guide
in position and remove the balloon catheter.
Stent positioning
• Advance the release system through the guide to positioning at an appropriate place. Place the stent on the lesion using the radiopaque marks
of the release balloon. IF RESISTANCE IS FOUND DURING STENT INSERTION, do not continue until the cause is established.
• Inflate the stent system expanding the stent to a pressure that ensures the adequate stent apposition against the artery wall. Use the deployment
pressure table. The recommended maximum pressure should not be exceeded.
• When the stent has been inflated and positioned, deflate the balloon and remove it carefully through the guide.
• Confirm the adequate stent position, performing an angiography through the guide catheter. To obtain optimum results, the entire stenosed arterial
segment must be covered by the stent. An optimum expansion requires that the stent is fully positioned in contact with the arterial wall. All appropriate
measures should be adopted to check that the stent is not sub-dilated.
Post-dilation
• Stent post-dilation may be required once it is positioned. For this, use the release catheter or any balloon of appropriate size and inflate it at the
stent site. Confirm the results by angiography. Dilate the balloon as many times as required until perfect stent dilation is obtained.
• Once the stent is post-dilated, deflate the balloon and remove it carefully. Remove the guide and the guide catheter.
8. Warnings on re-use
The content is supplied STERILE by ethylene oxide (EO). Do not use if the sterile barrier is damaged. If damages are found, call Iberhospitex agent.
For single use in a single patient. Do not re-use, re-process or re-sterilise. Re-use, re-processing or re-sterilisation can affect the structural integrity
of the device and/or cause its failure, which in turn could cause injuries in the patient, disease or death. The re-use, re-processing or re-sterilisation
can also create a risk of contamination of the device and/or cause infection or cross infection in the patient, including but not limited to the transmission
of infectious diseases from one patient to another. The contamination of the device can cause injuries, disease or death of the patient.
9. Warranty
Iberhospitex is not liable for the damages caused to patients by misuse of the catheter or the stent; therefore, no claims will be accepted except for
those stated herein. Under no circumstances will Iberhospitex be liable for any damage caused, except for those specifically established by the law.

1. Descripción del dispositivo
El sistema de stent coronario con elución de simvastatina IRIST SMALL, es un stent de elución de fármacos expansible por balón premontado que
consta de:
• Un stent Apolo Small de acero inoxidable 316 L grado médico que incluye un fármaco (una formulación de simvastatina contenida en un revestimiento
polimérico). El revestimiento consta de un portador polimérico cargado con 0,36 µg/mm2 de simvastatina en un sistema de liberación lenta, con una
cantidad nominal máxima de 56 µg en el stent de mayor tamaño (2,75 mm x 36 mm).
• Catéter balón de dilatación de rápido intercambio semidistensible con soporte de alta presión.
• El balón tiene marcadores radiopacos para la colocación precisa del stent, y está diseñado para proporcionar un segmento expansible a un
diámetro y longitud conocidos con aplicación de una presión específica.
• El stent se suministra en diferentes longitudes (9, 14, 19, 23, 28 y 36mm) y diámetros (2,0; 2,25; 2,50 y 2,75 mm).
Descripción del fármaco
El stent que forma parte del sistema de stent coronario Irist Small es un stent revestido con una formulación de fármaco y polímero compuesta de
simvastatina (ingrediente activo) y polímero portador P-5 (ingrediente no activo).
Simvastatina
El componente farmacéutico activo del stent Irist es la simvastatina. La simvastatina es un polvo cristalino blanco o casi blanco, con fórmula molecular
C25H38O5 y peso molecular 418,6. Es un fármaco de uso aprobado para el tratamiento de la hipercolesterolemia y para la prevención cardiovascular.
Portador polimérico P-5
El polímero P-5 es un ingrediente inactivo del stent Irist Small. Es un copolímero acrílico elastomérico, altamente flexible e hidrófobo. El polímero se
mezcla con el fármaco simvastatina y después se aplica en los stents, obteniendo un revestimiento de fármaco y polímero en toda la superficie luminal
y abluminal del stent.
2. Indicaciones y uso
El stent eluyente de simvastatina IRIST SMALL está indicado para mejorar el diámetro luminal coronario y reducir la reestenosis que se produce en
el interior y en los bordes del stent durante el tratamiento de lesiones de novo en arterias coronarias de pequeño calibre, con diámetro del vaso de
referencia de 2,0 mm a 2,75 mm.
El stent está indicado para lesiones con longitudes inferiores a la longitud nominal del stent utilizado (9, 14, 19, 23, 28 o 36 mm).
Criterios de inclusión general
1. Elegible para intervención coronaria transluminal percutánea (ICTP).
2. Candidato aceptable para injerto de derivación de arteria coronaria (IDAC).
Criterios de inclusión para angiografía
1. La lesión a tratar está situada dentro del vaso coronario nativo.
2. Estenosis del diámetro de la lesión a tratar 50% por estimación visual o 50% por estimación mediante análisis angiográfico cuantitativo
automático (QCA).
3. Contraindicaciones
La implantación del sistema de Stent IRIST SMALL está contraindicada y se debe excluir en pacientes si cumplen alguno de los siguientes criterios:
Criterios de exclusión general:
• Sensibilidad conocida a la simvastatina.
• Alergia conocida al acero inoxidable.
• Alergia a los medicamentos y/o materiales utilizados durante el procedimiento.
• Contraindicaciones a la terapia de anticoagulación y/o antiplaquetaria.
• La función del ventrículo izquierdo del paciente se encuentra gravemente comprometida (< 30%).
• Implantación directa de un stent.
• El paciente ha sufrido infarto de miocardio en las 48h anteriores al procedimiento con IRIST Small.
• Choque cardiogénico.
• Presencia conocida o probable de trombo intraluminal.
• Reestenosis intra-stent.
• Lesiones en bifurcación.
• Implantación de un stent en injertos de vena safena.
Criterios de exclusión para angiografía:
• Lesiones superiores al 50% situadas en el tronco coronario común izquierdo no protegido.
• La lesión a tratar o el vaso proximal están severamente calcificados.
• Oclusión total crónica del vaso a tratamiento.
• La lesión a tratar tiene una tortuosidad severa, no adecuada para la introducción y despliegue del stent.
4. Advertencias y precauciones
Advertencias
• La utilización de este producto conlleva los riesgos asociados de trombosis subaguda, complicaciones vasculares o episodios de hemorragia.
Por lo tanto, debe seleccionarse a los pacientes cuidadosamente y prescribir el tratamiento antiplaquetario doble (AAS y clopidogrel o ticlopidina).
En el ensayo clínico se administró el tratamiento antiplaquetario durante 6 meses post-procedimiento, por lo que el tratamiento nunca podrá ser inferior
a 6 meses, y convenientemente al menos durante 12 meses tras el procedimiento en pacientes con bajo riesgo de sangrado. La aspirina se deberá
administrar indefinidamente.
• Es muy importante proporcionar la información adecuada al paciente sobre las recomendaciones antiplaquetarias posteriores a la intervención.
El cese prematuro del medicamento antiplaquetario recetado aumentará el riesgo de trombosis seguida de infarto de miocardio o muerte.
• Este dispositivo no debe aplicarse a pacientes con hipersensibilidad confirmada a la simvastatina o al acero inoxidable 316 L.
• La aparición de reestenosis posteriores puede requerir volver a dilatar el segmento de arteria que contiene el stent. Se desconoce por el momento
el resultado de la dilatación repetida del stent.
• No se ha evaluado el solapamiento de stents IRIST Small. Aún así, cuando es necesario más de un stent, si la colocación implica contacto entre
ellos, el material de ambos stents debe ser de composición similar.
• No usar stents de diferentes composiciones para expandir el mismo vaso porque el riesgo de trombosis aumenta especialmente en arterias de
pequeño calibre.
• La implantación del stent debe realizarse únicamente en hospitales equipados con medios de emergencia ó en hospitales con facilidad de acceso
a otros hospitales que puedan realizar operación a corazón abierto en caso de complicaciones.
• La implantación sólo debe ser realizada por médicos altamente familiarizados y entrenados con la técnica de realización de angioplastias coronarias
trasluminales percutáneas y despliegue de stent.
Precauciones
Manejo del stent - Precauciones
• Dispositivo de un solo uso. NO REESTERILIZAR. NO REPROCESAR. Respetar la fecha de caducidad del envase.
• Previamente a su uso elegir correctamente la talla del stent a utilizar y verificarla.
• Abstenerse de retirar el stent del balón portador en el que se suministra. El stent no está diseñado para ser montado en otro balón. Extraer el
stent de su balón introductor puede dañar el stent y/o su revestimiento o producir una embolización del mismo.
• La manipulación inadecuada puede aflojar el contacto del stent con el balón y además dañar el recubrimiento.
• Prestar especial atención al retirar el catéter del envase, colocarlo sobre la guía y hacerlo avanzar a través del adaptador de la válvula hemostática
y el conector del catéter guía, para no manipular el stent ni desplazarlo de cualquier forma de su posición en el dispositivo introductor.
• No utilizar aire ni medios gaseosos para inflar el balón.
• No usar solventes orgánicos ni productos fabricados con base oleosa (tal como los medios de contraste Ethiodol o Lipiodol) ni de elevada viscosidad.
• No se recomienda el contacto del stent con ningún fluido antes de la colocación, ya que podría producirse liberación de fármaco. Sin embargo,
si es absolutamente necesario, irrigar el stent con solución salina isotónica / estéril, siendo el tiempo máximo de contacto 1 minuto.
• Si el stent no pudiera ser desplegado retirar cuidadosamente todo el sistema y no manipular el stent con las manos.
Colocación del stent - Precauciones
• No preparar o preinflar el balón antes del despliegue del stent de manera diferente a la indicada. Seguir la técnica de purgado del balón descrita
en la Sección de Instrucciones para el operador.
• El implante de un stent puede conducir a la disección del vaso distal o proximal a la porción estenosada y puede causar un cierre agudo del vaso
que requiera de una intervención adicional (ej. IDAC, mayor dilatación, colocación de stents adicionales, etc.)
• Se recomienda predilatar suficientemente la lesión a tratar antes de la implantación del stent. No se ha evaluado el implante directo de stent.
• Si se sintiera una resistencia inusual en cualquier momento durante al acceso a la lesión antes de la implantación del stent, el sistema de stent
y catéter guía deben extraerse como una sola unidad.
• El dispositivo debe manipularse bajo observación fluoroscópica de alta calidad. No hacer avanzar ni retroceder el catéter a menos que el balón
esté totalmente desinflado bajo vacío. Si durante la manipulación se encuentra resistencia, determinar la causa de la misma antes de proseguir.
• No desplegar el stent hasta haber conseguido posicionarlo correctamente en la estenosis. Observar la posición a través de las marcas radiopacas.
• La colocación del stent puede comprometer la permeabilidad de una rama lateral.
• La presión del balón no debería exceder la presión estimada de ruptura. Se recomienda el uso de un dispositivo de control de la presión para
evitar sobrepasar la presión de estallido del balón. Por lo menos el 99,9% de los balones (con una fiabilidad del 95%) no se romperán al llegar al
nivel (o por debajo), de la máxima presión recomendada. El uso de presiones más altas que las especificadas, puede causar rotura del balón y
posibles daños en la íntima y disección.
• Los métodos de extracción del stent (guías adicionales, lazos, fórceps), pueden producir un traumatismo vascular adicional. Entre las complicaciones
se incluyen hemorragia, hematoma o pseudoaneurisma.
Extracción del sistema de stent - Precauciones
• Si el stent no pudiera ser desplegado retirar cuidadosamente todo el sistema, especialmente cuando se retire hacia el catéter guía para evitar
que el stent se quede enganchado en la punta del catéter guía.
• Si se percibe alguna resistencia, retirar todo el sistema incluyendo el catéter guía, como una sola unidad. Esto debe hacerse bajo visualización
fluoroscópica directa. En este caso, seguir los siguientes pasos:
- Abstenerse de retraer el sistema de posicionamiento dentro del catéter guía.
- Posicionar la marca radiopaca proximal del balón encarada con el extremo distal del catéter guía.
- Hacer avanzar la guía en la anatomía coronaria lo más distante posible.
- Apretar la válvula hemostática rotatoria y retirar todo el conjunto cuidadosamente como una sola unidad.
- Si no se siguen estos pasos o se aplica una fuerza excesiva al sistema de stent, se puede provocar pérdida o daño del stent o los componentes
del sistema de stent, como por ejemplo, el balón
Post- implante - Precauciones
• Tener el máximo cuidado al cruzar un stent recién desplegado con un catéter de ultrasonido intravascular ó un catéter balón, ya que se puede
alterar la geometría del stent ó del recubrimiento.
• Tras la implantación reciente de un stent evitar procedimientos de resonancia magnética al paciente para reducir la posibilidad de movimiento
del stent. Debe esperarse 2-3 meses para asegurar la endotelización del stent.
• Es necesario prescribir un tratamiento doble antiplaquetario (AAS y clopidogrel o ticlopidina) al menos durante 6 meses tras el procedimiento y
recomendable durante 12 meses en pacientes con bajo riesgo de sangrado, para reducir el riesgo de trombosis en la zona del stent. La aspirina se
deberá administrar indefinidamente.
Interacciones medicamentosas
• No se han investigado las posibles interacciones de la simvastatina con las medicaciones administradas de manera concomitante. Las interacciones
con medicamentos de los niveles sistémicos de simvastatina están detallados en la ficha técnica del producto pero, si atendemos a que el Stent IRIST
SMALL tiene unos niveles de simvastatina 1000 veces inferiores a las dosis administradas sistémicamente, no son previsibles dichas interacciones.
• En el caso específico de administración conjunta de una estatina por vía oral, los cálculos realizados demuestran que no es previsible el incremento
del riesgo de toxicidad en pacientes tratados con el Stent IRIST SMALL que además reciban estatinas por vía oral.
Embarazo
• Este producto no ha sido evaluado en mujeres embarazadas o en hombres que deseen tener hijos, por lo que se recomienda a los receptores
del dispositivo evitar el embarazo. Aunque no haya contraindicaciones, los riesgos y efectos reproductivos siguen siendo desconocidos.
5. Posibles episodios adversos

7. Instrucciones para el operador
Inspección previa a su uso
• Inspeccione cuidadosamente el envase antes de abrirlo. No usar después de la “fecha de caducidad”. Si se observan posibles daños en el embalaje
no utilizar el producto, y ponerse en contacto con Iberhospitex para obtener información sobre como resolverlo. No utilizar si se observa algún defecto.
• En cualquier momento durante su empleo, si se observa que el cuerpo proximal de acero inoxidable se ha doblado o acodado, no continuar usando
el catéter.

Preparación
• Preparar el dispositivo de inflado con el medio de contraste recomendado según instrucciones del fabricante. Purgar el aire del sistema.
• Seleccionar correctamente el stent en cuanto a longitud y diámetro. El diámetro del balón (stent) no debe exceder el diámetro de la arteria a dilatar
antes y después de la estenosis.
• Dejar el catéter dentro de su tubo de protección. Retirar el extremo distal del aro de protección para ver el stent.
• Comprobar que el stent se encuentra asegurado entre las marcas del balón, no tocar con los dedos ya que podría dañarse.
• Preparar el catéter de dilatación para el purgado. Llenar la jeringuilla de 20 cc con cierre luer con 3 cc de la solución de contraste apropiada.
ADVERTENCIA: No utilice la técnica de inflado previo para purgar el lumen del balón. No usar aire ni ningún otro medio gaseoso para inflar el balón.
No extraiga presión negativa del dispositivo de inflado.
• Conectar la jeringuilla al lumen del balón, orientar el catéter con la punta distal y el balón apuntando verticalmente hacia abajo.
• Aplicar presión negativa y aspirar durante 5 seg. Soltar el émbolo despacio.
• Retirar la jeringuilla del lumen de inflado del balón y retira todo el aire.
• Volver a conectar la jeringuilla al catéter y mantener presión negativa en el balón hasta que deje de retornar aire al dispositivo. Liberar la presión
de la jeringuilla hasta el punto neutro y desconectarla.
• Conectar el dispositivo de inflado al puerto de inflado del catéter sin introducir aire al sistema.
ADVERTENCIA: Evitar dar presión positiva al sistema de liberación; mantener el dispositivo de inflado a la presión ambiente.
Técnica de inserción
• Introducir el catéter guía mediante técnicas Standard.
• Predilatar la lesión con un balón de diámetro adecuado que tenga una relación de 1:1 con el diámetro del vaso. Una vez predilatada la lesión,
dejar la guía en posición y retirar el catéter balón.
Posicionamiento del stent
• Avanzar el sistema de liberación a través de la guía hasta posicionamiento en el lugar adecuado. Poner el stent sobre la lesión utilizando las
marcas radiopacas del balón de liberación. EN CASO DE ENCONTRAR RESISTENCIA DURANTE LA INSERCIÓN DEL STENT, no proseguir hasta
determinar la causa.
• Inflar el sistema de stent expandiendo el stent hasta una presión tal que se asegure la correcta aposición del stent contra la pared arterial. Utilizar
la tabla de presiones de despliegue. No exceder la presión máxima recomendada.
• Cuando el stent haya sido inflado y posicionado, desinflar el balón y retirarlo cuidadosamente a través de la guía.
• Confirmar la posición correcta del stent, realizando una angiografía a través del catéter guía. Para obtener resultados óptimos, la totalidad del
segmento arterial estenosado debe estar cubierto por el stent. Una óptima expansión requiere que el stent quede totalmente posicionado en contacto
con la pared arterial. Deben tomarse todas las medidas necesarias para comprobar que el stent no esté subdilatado.
Postdilatación
• Puede ser necesario postdilatar el stent una vez ha sido posicionado. Para ello utilizar el catéter de liberación u otro balón de la talla adecuada e
inflarlo en el lugar del stent. Confirmar los resultados mediante angiografía. Dilatar el balón tantas veces como sea necesario hasta conseguir la
perfecta dilatación del stent.
• Una vez postdiladado el stent, desinflar el balón y retirarlo cuidadosamente. Retirar la guía y el catéter guía.
8. Advertencias sobre la reutilización
El contenido se suministra ESTÉRIL mediante óxido de etileno (OE). No usar si la barrera estéril está dañada. Si se encuentran daños, llamar al
representante de Iberhospitex.
Para uso en un solo paciente. No reusar, reprocesar o reesterilizar. La reutilización, el reprocesamiento o la reesterilización pueden comprometer
la integridad estructural del dispositivo y/o causar su fallo, lo que a su vez puede resultar en lesiones al paciente, enfermedad o la muerte. La
reutilización, reprocesamiento o reesterilización pueden también crear riesgo de contaminación del dispositivo y/o causar infección o infección cruzada
al paciente, incluyendo pero no limitándose a la transmisión de enfermedades infecciosas de un paciente a otro. La contaminación del dispositivo
puede causar lesiones, enfermedad o la muerte del paciente.
9. Garantía
Iberhospitex no se hace responsable de los daños causados a pacientes por una utilización errónea del catéter o del stent, por lo tanto no aceptará
reclamaciones a excepción de las especificadas aquí. Bajo ninguna circunstancia Iberhospitex se hará responsable de ningún daño causado a

1. Beschreibung der Vorrichtung
Das simvastatin eluierende koronare Stent-System IRIST SMALL ist ein mit einem Wirkstoff beschichteter Stent, der durch einen vormontierten Ballon
expandierbar ist. Die Vorrichtung besteht aus:
• einem Apolo-Small-Stent aus rostfreiem medizinischen Edelstahl 316 L, der einen Wirkstoff enthält (eine simvastatin-Formel, die in einer polymeren
Beschichtung enthalten ist). Die Beschichtung besteht aus einem polymeren Träger, der 0,36 μg/mm2 Simvastatin in einem langsamen FreisetzungsSystem aufweist, mit einer maximalen Nennmenge von 56 μg in einem der größten Stents (2,75 mm x 36 mm).
• einem Dilatations-Ballonkatheter mit schnellem semi-complianten Wechsel und Hochdruck-Support.
• Der Ballon besitzt radioopake Markierungen zur präzisen Platzierung des Stents und ist so ausgelegt, dass ein expandierbares Segment von
bekanntem Durchmesser und Länge bei Anwendung eines spezifischen Druckes zur Verfügung gestellt wird.
• Der Stent wird in unterschiedlichen Längen (9, 14, 19, 23, 28, 36 mm) und Durchmessern (2,0; 2,25; 2,50; 2,75 mm) geliefert.
Erläuterung des Wirkstoffs
Der Stent, der Bestandteil des koronaren Stent-Systems „IRIST SMALL“ ist, ist ein mit einem Wirkstoff und einem Polymer beschichteter Stent, der
aus Simvastatin (aktiver Bestandteil) und dem Trägerpolymer P5 (nicht aktiver Bestandteil) besteht.
Simvastatin
Der aktive pharmazeutische Wirkstoff des Stents IRIST SMALL ist Simvastatin. Simvastatin ist ein weißes oder fast weißes kristallines Pulver mit der
Molekularformel C25H38O5 und einem Molekulargewicht von 418,6. Es handelt sich um ein zugelassenes Arzneimittel zur Behandlung der
Hypercholesterinämie und der kardiovaskulären Prävention.
Polymerer Träger P5
Das Polymer P5 ist ein inaktiver Bestandteil des Stents „IRIST SMALL”. Es handelt sich hier um ein hochflexibles und hydrophobes, elastomeres AcrylCopolymer. Das Polymer vermischt sich mit dem Wirkstoff Simvastatin und kommt danach in den Stents zur Anwendung, indem ein Überzug des
Wirkstoffs und des Polymers auf der gesamten luminalen und abluminalen Stentoberfläche erhalten wird.
2. Hinweise und Benutzung
Der Simvastatin eluierende Stent „IRIST SMALL” ist geeignet, den luminalen koronaren Durchmesser zu verbessern und die Restenose zu reduzieren,
die im Inneren und an den Rändern des Stents bei der Behandlung von De-Novo-Läsionen in den Koronararterien mit einem Gefäßdurchmesser von
2,0 mm bis 2,75 mm auftritt.
Der Stent ist für Läsionen mit Längen geeignet, die unter der Nennlänge des verwendeten Stents (9, 14, 19, 23, 28 o 36 mm) liegen.
Kriterien des allgemeinen Einsatzes
1. Zu wählen bei der perkutanen transluminalen koronaren Intervention (PTCI).
2. Annehmbare Option für den Koronararterien-Bypass (IDAC).
Kriterien des Einsatzes in der Angiografie
1. Die zu behandelnde Läsion befindet sich im nativen Herzkranzgefäß.
2. Die Stenose des Durchmessers der zu behandelnden Läsion ist nach visueller Einschätzung ≥ 50 % oder gemäß Einschätzung nach einer
quantitativen Analyse der Koronarangiografie (QCA) ≥ 50%.
3. Gegenanzeigen
Die Implantation des Stent-Systems „IRIST SMALL“ ist für Patienten nicht geeignet und auszuschließen, wenn einige der nachfolgenden Kriterien
zutreffen:
Allgemeine Ausschlusskriterien:
• Bekannte Empfindlichkeit für Simvastatin.
• Bekannte Allergie gegen rostfreien Edelstahl.
• Allergie gegen die während des Verfahrens verwendeten Medikamente und/oder Materialien.
• Gegenanzeigen bezüglich der Antikoagulations- und/oder Antithrombozytentherapie.
• Die Funktion der linken Herzkammer des Patienten ist ernsthaft gefährdet (< 30 %).
• Direkte Implantation eines Stents.
• 48 Stunden vor dem Einsatz des IRIST SMALL-Stents, hat der Patient einen Herzinfarkt erlitten.
• Kardiogener Schock.
• Bekanntes oder wahrscheinliches Vorhandensein eines intraluminalen Thrombus.
• Intra-Stent-Restenose
• Läsionen im Bypass.
• Implantation eines Stents in Vena-saphena-Implantaten.
Angiografie - Ausschlusskriterien:
• Über 50 % große Läsionen, die sich in der ungeschützten linken Hauptkoronararterie befinden.
• Die zu behandelnde Läsion oder das proximale Gefäß sind ernsthaft verkalkt.
• Totale chronische Okklusion des zu behandelnden Gefäßes.
• Die zu behandelnde Läsion hat eine ernsthafte Tortuosität, die für die Einführung und Entfaltung des Stents ungeeignet ist.
4. Warnhinweise und Vorsichtsmaßnahmen
Warnhinweise
• Die Verwendung dieses Produktes birgt die Risiken einer subakuten Thrombose, vaskulärer Komplikationen bzw. hämorrhagischer Gefahren. Aus
diesem Grund sind die Patienten sorgfältig auszuwählen und die doppelte Antithrombozytenbehandlung (AAS und Clopidogrel oder Ticlopidin) zu
verschreiben. In der klinischen Studie wurde die Antithrombozytentherapie 6 Monate lang nach dem Verfahren verabreicht, weshalb die Behandlungsdauer
niemals unter 6 Monaten liegen darf und bei Patienten mit einem geringen Blutungsrisiko zweckmäßigerweise mindestens 12 Monate lang nach dem
Verfahren durchzuführen ist. Aspirin ist auf unbestimmte Zeit zu.
• Es ist sehr wichtig, den Patienten über die Empfehlungen der Antithrombozytenbehandlung nach dem Eingriff entsprechend zu informieren. Ein
vorzeitiger Abbruch des verschriebenen Antithrombozytenmedikaments erhöht die Gefahr einer Thrombose bzw. eines anschließenden Herzinfarkts
oder Todes.
• Diese Vorrichtung darf nicht bei Patienten angewandt werden, die eine bestätigte Überempfindlichkeit gegen Simvastatin oder 316L-Edelstahl
haben.
• Das Auftreten einer anschließenden Restenose kann eine erneute Aufdehnung des Arteriensegmentes erforderlich machen, das den Stent enthält.
Zum heutigen Zeitpunkt ist das Ergebnis einer mehrmaligen Aufdehnung des Stents nicht bekannt.
• Die Überlappung wurde an IRIST Small stent nicht evaluiert. Wenn mehr als ein Stent notwendig ist und die Implantation eine Berührung beider
Stents erforderlich macht, muss das Material beider Stents eine ähnliche Zusammensetzung aufweisen.
• Keine Stents unterschiedlicher Zusammensetzungen zur Expansion im gleichen Gefäß verwenden, weil die Thrombosegefahr insbesondere in
kleinen Arterien erhöht ist.
• Die Implantation darf nur in Krankenhäusern erfolgen, die mit Notfallausrüstungen ausgestattet sind oder die einen schnellen Zugang zu anderen
Krankenhäusern haben, in denen im Komplikationsfall Operationen am offenen Herzen durchgeführt werden können.
• Die Implantation darf nur von hochqualifizierten Ärzten ausgeführt werden, die mit der Technik der perkutanen transluminalen Koronarangioplastie
und der Stent-Entfaltung vertraut sind und in dieser ausgebildet wurden.
Vorsichtsmaßnahmen
Gebrauch des Stents - Vorsichtsmaßnahmen
• Vorrichtung für den einmaligen Gebrauch. Der Stent darf weder RESTERILISIERT noch WIEDERVERWENDET werden. Das auf der Verpackung
angegebene Verfallsdatum ist einzuhalten.
• Vor dem Gebrauch ist die korrekte Größe des zu verwendenden Stents zu wählen.
o Der Stent darf nicht vom gelieferten Trägerballon gelöst werden. Er ist nicht dafür ausgelegt, auf einen anderen Ballon montiert zu werden. Das
Lösen des Stents von seinem Einführungs-Ballon kann dem Stent und/oder seiner Beschichtung schaden bzw. dessen Embolisation verursachen.
• Eine unangebrachte Handhabung kann die Verbindung zwischen Stent und Ballon bzw. die Beschichtung beschädigen.
• Das Herausnehmen des Katheters aus der Verpackung, sein Platzieren auf der Führung und das Vorschieben über den Adapter des hämostatischen
Ventils und den Konnektor des Führungskatheters sind sehr sorgfältig durchzuführen, damit der Stent weder manipuliert, noch in irgendeiner Form
aus seiner Position auf der Einführhilfe verschoben wird.
• Für die Inflatierung des Ballons keine Luft oder gasförmigen Mittel verwenden.
• Keine organischen Lösungsmittel oder auf Ölbasis hergestellte Produkte (so wie die Kontrastmittel Ethiodol oder Lipiodol) oder solche mit erhöhter
Viskosität verwenden.
• Der Kontakt des Stents vor dessen Platzierung mit Flüssigkeiten wird nicht empfohlen, da eine Freisetzung des Medikamentes erfolgen könnte.
Dagegen ist es absolut notwendig, den Stent mit isotonischer /steriler Salzlösung zu spülen, wobei die maximale Kontaktzeit 1 Minute beträgt.
• Falls der Stent nicht entfaltet werden kann, ist das gesamte System sorgfältig zu entfernen. Der Stent darf nicht mit den Händen gehandhabt
werden.
Stent-Platzierung - Vorsichtsmaßnahmen
• Den Ballon nicht bereits vor der Entfaltung des Stents vorbereiten oder vorinflatieren, sondern nur so wie angegeben vorgehen. Befolgen Sie die
Spültechnik des Ballons, die in dem Abschnitt „Anweisungen für den Benutzer“ beschrieben ist.
• Das Stent-Implantat kann zu einer Dissektion des distal oder proximal zum Stenoseabschnitt liegenden Gefäßes führen und einen akuten Verschluss
des Gefäßes hervorrufen, der eine zusätzliche Intervention erforderlich macht (z.B. IDAC, größere Aufweitung, Platzierung zusätzlicher Stents usw.).
• Es wird empfohlen, vor der Implantation des Stents die zu behandelnde Läsion ausreichend vorzudilatieren. Die direkte Implantation des Stents
wurde nicht evaluiert.
• Falls während des Läsionszugangs vor der Implantation des Stents irgendein unüblicher Widerstand festgestellt werden sollte, ist das Stent- und
Führungskatheter-System als Einheit zu entfernen.
• Die Vorrichtung ist unter hochqualitativer Fluoreszenz-Beobachtung zu handhaben. Der Katheter darf weder vorgeschoben noch zurückgezogen
werden, sofern der Ballon nicht vollkommen mittels Vakuum entleert wurde. Falls während der Handhabung ein Widerstand festgestellt wird, ist der
Grund festzustellen, bevor man mit der Untersuchung fortfährt.
• Erst nachdem der Stent korrekt in der Stenose platziert wurde, darf er entfaltet werden. Die Position ist über die radioopaken Markierungen zu
beobachten.
• Die Platzierung des Stents kann die Permeabilität eines Seitenastes gefährden.
• Der Ballondruck darf den geschätzten Berstdruck nicht übersteigen. Es wird empfohlen, eine Druckkontrollvorrichtung einzusetzen, damit der
Berstdruck des Ballons nicht überschritten wird. Zumindest 99,9 % der Ballons (bei einer Zuverlässigkeit von 95 %) bersten nicht, wenn sie das
empfohlene maximale Druckniveau erreichen. Wenn man höhere Drücke als die vorgegebenen einsetzt, kann das zu einem Bruch des Ballons sowie
zu möglichen Schäden an Intima und Dissektion führen.
• Die Methoden der Stententfernung (zusätzliche Führungen, Führungsdrähte, Zangen) können ein zusätzliches Gefäßtrauma verursachen. Es
können Komplikationen, wie Hämorrhagien, Hämatome oder Pseudoaneurysmen auftreten.
Entfernung des Stent-Systems - Vorsichtsmaßnahmen
• Falls der Stent nicht entfaltet werden kann, ist das gesamte System sehr sorgfältig zu entfernen, insbesondere wenn in Richtung des Führungskatheters
zurückgezogen wird und verhindert werden muss, dass das Stent an der Führungskatheterspitze hängen bleibt.
• Falls irgendein Widerstand festgestellt werden sollte, ist das gesamte System, einschließlich Führungskatheter, als Einheit zu entfernen. Dies ist
bei direkter Fluoreszenz-Visualisierung zu realisieren. In diesem Fall ist wie folgt vorzugehen:
- Das Positionierungssystem im Führungskatheter darf nicht zurückgezogen werden.
- Die radioopake Markierung ist proximal des Ballons gegenüber dem distalen Ende des Führungskatheters zu positionieren.
- Die Führung ist so weit wie möglich von der koronaren Anatomie weg vorzuschieben.
- Das rotierende hämostatische Ventil ist zu betätigen, und die gesamte Einheit ist sorgfältig als einzige Einheit herauszuziehen.
- Wenn diese Schritte nicht eingehalten werden oder eine zu große Kraft auf das Stent-System ausgeübt wird, kann ein Verlust oder eine Schädigung
des Stents oder der Bestandteile des Stent-Systems, beispielsweise des Ballons, verursacht werden.
Nach der Implantation - Vorsichtsmaßnahmen
• Es ist äußerste Aufmerksamkeit walten zu lassen, wenn ein vor kurzem entfaltetes Stent an einem intravaskulären Ultraschallkatheter oder
Ballonkatheter vorbeigeführt wird, weil die Geometrie des Stents oder der Beschichtung beeinträchtigt werden könnte.
• Eine gewisse Zeit nach der Implantation eines Stents sollten Magnetresonanz-Untersuchungen des Patienten vermieden werden, um die Möglichkeit
einer Stentverschiebung zu vermeiden. Es sind 2 bis 3 Monate abzuwarten, um die Endothelisation des Stents sicherzustellen.
• Es ist notwendig, zumindest für 6 Monate nach der Intervention und empfohlenerweise 12 Monate lang, den Patienten mit geringem Blutungsrisiko
eine doppelte Antithrombozytenbehandlung (AAS und Clopidogrel oder Ticlopidin) zu verschreiben, um die Thrombosegefahr im Stentbereich zu
senken. Aspirin ist auf unbestimmte Zeit zu.
Medikamentenwechselwirkungen
• Die möglichen Wechselwirkungen des Simvastatin mit Medikamenten, die in begleitender Form verabreicht werden, sind noch nicht erforscht. Die
Medikamentenwechselwirkungen der systemischen Konzentrationen des Simvastatins sind in den technischen Daten des Produktes angegeben.
Wenn wir jedoch davon ausgehen, dass die Simvastatinkonzentration 1000 fach geringer ist als die Dosis, die bei Systemischebehandlungen eingesetzt
wird, sind die genannten Wechselwirkungen nicht zu erwarten.
• Im spezifischen Fall der gemeinsamen oralen Verabreichung von Statin belegen die durchgeführten Berechnungen, dass eine Erhöhung der
Toxizitätsgefahr bei Patienten, die zusätzlich Statine auf oralem Wege zu sich nehmen, mit dem Stent IRIST SMALL nicht voraussehbar ist.
Schwangerschaft
• Dieses Produkt ist weder bei schwangeren Frauen noch bei Männern mit Kinderwunsch getestet worden, weshalb den Empfängern der Vorrichtung
empfohlen wird, eine Schwangerschaft zu vermeiden. Obwohl keine Gegenanzeigen vorliegen, sind die Gefahren und Wirkungen in diesem Fall noch
unbekannt.
5. Mögliche unerwünschte Nebenwirkungen
Die nachfolgenden unerwünschten Nebenwirkungen können, unter anderem, mit der Platzierung eines koronaren Stents in nativen Herzkranzgefäßen
in Verbindung stehen:

6. Lieferform
Steril: Dieses Produkt wurde mit Ethylenoxid sterilisiert und ist apyrogen. Es ist für den einmaligen Gebrauch bestimmt. Nicht verwenden, wenn sich
die Verpackung nicht in einwandfreiem Zustand befindet.

7. Anweisungen für den Benutzer
Überprüfung vor dem Gebrauch
• Bevor das System geöffnet wird, ist dessen Verpackung sorgfältig zu überprüfen. Es darf nicht nach dem Verfallsdatum verwendet werden. Falls
Sie mögliche Beschädigungen an der Verpackung feststellen sollten, dürfen Sie das Produkt nicht verwenden. Setzen sich in diesem Fall mit Iberhospitex
in Verbindung, um sich über eine Lösung für diese Situation zu informieren. Wenn Sie an dem System irgendeinen Mangel feststellen sollten, darf
es nicht verwendet werden.

Vorbereitung
• Die Inflationsvorrichtung ist gemäß Anweisungen des Herstellers mit dem empfohlenen Kontrastmittel vorzubereiten. Die Luft ist aus dem System
abzulassen.
• Korrekte Wahl der Länge und des Durchmessers des Stents. Der Durchmesser des Ballons (des Stents) darf nicht größer sein als der Durchmesser
der vor und nach der Stenose zu weitenden Arterie.
• Der Katheter ist in seiner Schutzhülle zu belassen. Das distale Ende des Schutzringes ist zu entfernen, um den Stent zu sehen.
• Es ist zu überprüfen, dass der Stent sicher zwischen den Markierungen des Ballons angebracht ist. Nicht mit den Fingern anfassen, weil er
beschädigt werden könnte.
• Den Dilatations-Katheter für die Reinigung vorbereiten. Die 20-cc-Spritze mit Luer-Anschluss ist mit 3 cc des geeigneten Kontrastmittels zu befüllen.
ACHTUNG: Nicht die Vor-Inflationstechnik zum Reinigen des Ballonlumens anwenden. Zum Inflatieren des Ballons darf weder Luft noch irgendein
anderes gasförmiges Mittel verwendet werden.
• Die Spritze ist auf dem Ballonlumen anzubringen. Ausrichtung des Katheters mit der distalen Spitze und des Ballons senkrecht nach unten.
• Beaufschlagung des Unterdruckes und 5 Sek. lang saugen. Den Kolben langsam loslassen.
• Entfernen der Spritze vom Inflationslumen des Ballons und die gesamte Luft ablassen.
• Die Spritze erneut auf dem Katheter anbringen und auf den Ballon solange Unterdruck geben, bis keine Luft mehr in das System zurücktritt. Den
Druck aus der Spritze bis zum neutralen Punkt entweichen lassen und diese abnehmen.
• Die Inflationsvorrichtung an den Inflationsport des Katheters anschließen, ohne Luft in das System eintreten zu lassen.
ACHTUNG: Es ist zu vermeiden, das Freisetzungssystem mit positiven Druck zu beaufschlagen; die Inflationsvorrichtung ist auf Raumdruck zu halten.
Einführtechnik
• Einführung des Führungskatheters nach den Standardtechniken.
• Vordilatation der Läsion mit einem Ballon, der einen geeigneten Durchmesser im Verhältnis 1 : 1 zum Durchmesser des Gefäßes aufweist. Nachdem
die Läsion vordilatiert ist, die Führung in Position bringen und den Ballonkatheter entfernen.
Positionierung des Stents
• Das Freisetzungssystem über die Führung bis zur Positionierung an der geeigneten Stelle vorschieben. Den Stent auf der Läsion anordnen, wobei
die radioopaken Markierungen des Freisetzungsballons zugrunde gelegt werden. FÜR DEN FALL, DASS WÄHREND DER EINFÜHRUNG DES
STENTS AUF WIDERSTAND GESTOSSEN WIRD, darf nicht fortgefahren werden, sondern muss erst der Grund dafür ermittelt werden.
• Inflation des Stent-Systems, wobei der Stent bis zu einem solchen Druck expandiert wird, dass die korrekte Position des Stents zur arteriellen
Wand sichergestellt ist. Anwendung der Drucktabelle für die Entfaltung. Der empfohlene maximale Druck darf nicht überschritten werden.
• Wenn der Stent inflatiert und positioniert ist, ist der Ballon zu entleeren und sorgfältig über die Führung herauszunehmen..
• Die korrekte Position des Stents bestätigen, indem eine Angiografie über den Führungskatheter durchgeführt wird. Um optimale Ergebnisse zu
erzielen, muss der gesamte arterielle Stenosebereich vom Stent bedeckt sein. Eine optimale Expansion erfordert, dass der Stent ganz in Berührung
mit der arteriellen Wand positioniert wurde. Es sind alle notwendigen Maßnahmen zu ergreifen, um zu überprüfen, dass der Stent nicht subdilatiert
ist.
Nachdilatation
• Nach der Positionierung des Stents kann dessen Nachdilatation erforderlich werden. Dafür ist der Freisetzungs-Katheter oder ein anderer Ballon
geeigneter Größe zu verwenden und an der Stelle des Stents zu inflatieren. Die Ergebnisse sind mittels Angiografie zu bestätigen. Der Ballon ist so
oft wie nötig zu dilatieren, um die perfekte Dilatation des Stents zu erreichen.
• Nachdem der Stent nachdilatiert wurde, ist der Ballon zu entleeren und sorgfältig zu entfernen. Die Führung und der Führungskatheter sind sorgfältig
herauszunehmen.
8. Warnhinweise zu einer Wiederverwendung
Der Inhalt wird in einem durch Ethylenoxid (EO) erreichten STERILEN Zustand geliefert. Er darf nicht verwendet werden, wenn die sterile Barriere
beschädigt ist. Wenn Schäden entdeckt werden, müssen Sie einen Vertreter von Iberhospitex anrufen.
Das System darf nur für einen einzigen Patienten verwendet werden. Es darf nicht wiederverwendet, verarbeitet oder resterilisiert werden. Die
Wiederverwendung, Wiederverarbeitung oder Resterilisation kann der strukturellen Integrität der Vorrichtung schaden und/oder deren Defekt verursachen,
was dem Patienten gleichzeitig Läsionen, Krankheit oder Tod bringen kann. Die Wiederverwendung, Wiederverarbeitung oder Resterilisation kann
ebenso die Gefahr einer Verunreinigung der Vorrichtung implizieren und/oder dem Patienten eine Infektion oder verschleppte Infektion durch

verursachen. Die Verunreinigung der Vorrichtung kann Läsionen, Krankheit oder Tod des Patienten bewirken.
9. Garantie
Iberhospitex ist nicht für Schäden an Patienten verantwortlich, die durch eine fehlerhafte Verwendung des Katheters oder des Stents entstehen,
weshalb keine Reklamationen mit Ausnahme der hier aufgeführten akzeptiert werden. Iberhospitex übernimmt unter keinen Umständen die Verantwortung
für irgendeinen verursachten Schaden, mit Ausnahme jener, die gesetzlich ausdrücklich festgelegt sind.

1. Description du dispositif
Le système de stent coronaire à élution de simvastatine IRIST SMALL est un stent à élution de médicaments expansible par ballonnet pré-monté se
composant de :
• Un stent Apolo Small en acier inoxydable 316 L de classe médicale intégrant un médicament (une formule de simvastatine contenue dans une
couche de polymère) Cette couche se compose d'un porteur polymère chargé de 0,36 µg/mm2 de simvastatine dans un système à libération lente,
avec une quantité nominale maximale de 56 µg dans le stent de grande taille (2,75 mm x 36 mm).
• Un cathéter à ballonnet de dilatation à échange rapide semi-extensible avec support pour haute pression.
• Le ballonnet possède des marqueurs radio-opaques pour la mise en place précise du stent et est conçu pour fournir un segment qui, grâce à
l'application d'une pression spécifique, peut se dilater à un diamètre et à une longueur définis.
• Le stent est disponible en différentes longueurs (9, 14, 19, 23, 28 ou 36 mm) et différents diamètres (2,0 ; 2,25 ; 2,50 ou 2,75 mm).
Description du médicament
Le stent faisant partie du système de stent coronaire IRIST SMALL est un stent recouvert d'une formule de médicament et de polymère composée
de simvastatine (ingrédient actif) et du vecteur polymère P-5 (ingrédient non actif).
Simvastatine
Le composant pharmaceutique actif du stent IRIST SMALL est la simvastatine. La simvastatine est une poudre cristalline blanche ou presque blanche,
dont la formule moléculaire est C25H38O5 et le poids moléculaire 418,6. Il s'agit d'un médicament utilisé dans le traitement de l'hypercholestérolémie
et dans la prévention des maladies cardiovasculaires.
Vecteur polymère P-5
Le polymère P-5 est un ingrédient inactif du stent IRIST SMALL. Il s'agit d'un copolymère acrylique élastomère, très flexible et hydrophobe. Ce polymère
est mélangé au simvastatine, puis appliqué sur les stents, ce qui forme une couche de médicament et de polymère sur toute la membrane luminale
et abluminale du stent.
2. Conseils d'utilisation
Le stent à élution de Simvastatine IRIST SMALL permet d'augmenter le diamètre luminal coronarien et de réduire la resténose se produisant à
l'intérieur et sur les bords du stent au cours du traitement de lésions de novo sur des artères coronaires dont le diamètre du vaisseau de référence
est de 2,0 mm à 2,75 mm.
Ce stent est indiqué pour les lésions d'une longueur inférieure à la longueur nominale du stent utilisé (9, 14, 19, 23, 28 ou 36 mm).
Critères généraux d'inclusion
1. Eligible pour une intervention coronarienne transluminale percutanée (ICTP).
2. Candidat admissible pour un pontage aorto-coronarien (PAC).
Critères d'inclusion pour angiographie
1. La lésion à traiter se situe dans le vaisseau coronaire d'origine.
2. Sténose du diamètre de la lésion à traiter 50% (estimation visuelle) ou 50% (estimation par analyse angiographique quantitative automatique).
3. Contre-indications
L'implantation du système de stent IRIST SMALL est contre-indiquée et doit être exclue pour les patients répondant aux critères d'exclusion suivants :
Critères généraux d'exclusion
• Sensibilité connue au Simvastatine.
• Allergie connue à l'acier inoxydable.
• Allergie aux médicaments et/ou matériaux utilisés durant la procédure.
• Contre-indications au traitement anticoagulant et/ou antiplaquettaire.
• La fonction du ventricule gauche du patient est gravement compromise (< 30%).
• Implantation directe d'un stent.
• Le patient a subi un infarctus du myocarde au cours des 48h précédant l'utilisation du système IRIST SMALL.
• Choc cardiogénique.
• Présence connue ou probable de thrombus intraluminal.
• Resténose intrastent.
• Lésion en Y.
• Implantation directe d'un stent dans les greffons veineux saphènes.
Critères d'exclusion pour angiographie
• Lésions de plus de 50 % situées sur le tronc coronarien commun gauche non protégé.
• La lésion à traiter ou le vaisseau proximal sont gravement calcifiés.
• Occlusion totale chronique du vaisseau à traiter.
• La lésion à traiter est très sinueuse, non adaptée à l'introduction et au déploiement du stent.
4. Avertissements et précautions
Avertissements
• L'utilisation de ce produit présente des risques associés de thrombose subaigüe, de complications vasculaires ou d'épisodes hémorragiques. Par
conséquent, les patients doivent être soigneusement sélectionnés et le traitement antiplaquettaire double (AASS et clopidogrel ou ticlopidine) doit
être prescrit. Dans cet essai clinique, le traitement antiplaquettaire est administré durant 6 mois après la procédure, c'est pourquoi le traitement ne
pourra jamais être administré durant moins de 6 mois et devra, de préférence, l'être pendant au moins les 12 mois suivants le traitement chez les
patients présentant un faible risque d'hémorragie. L'aspirine devra être administrée sans interruption.
• Il est très important d'informer le patient de manière adéquate sur les recommandations antiplaquettaires après l'intervention. L'arrêt prématuré
de l'antiplaquettaire prescrit augmente le risque de thrombose suivie d'un infarctus du myocarde ou du décès du patient.
• Ce dispositif ne doit pas s'appliquer aux patients présentant une hypersensibilité confirmée au Simvastatine ou à l'acier inoxydable 316 L.
• L'apparition de resténoses postérieures peut nécessiter de dilater de nouveau le segment d'artère où le stent a été posé. Les conséquences de
la dilatation répétée du stent restent, à l'heure actuelle, inconnues.
• La superposition de stents Irist Small n'a pas été évaluée. Méanmoins, si leur positionnement implique de les mettre en contact, le matériau des
stents doit être de composition similaire.
• Ne pas utiliser de stents de compositions différentes pour dilater le même vaisseau, car le risque de thrombose augmente notamment dans les
artères de petit calibre.
• L'implantation du stent ne doit être réalisée que dans des hôpitaux équipés de systèmes d'urgence ou dans des hôpitaux dotés d'un accès facilité
à d'autres hôpitaux pouvant réaliser des opérations à cœur ouvert en cas de complication.
• L'implantation ne doit être réalisée que par des médecins très familiarisés et entraînés à la technique de réalisation d'angioplasties coronariennes
transluminales percutanées et de déploiement de stent.
Précautions
Manipulation du stent - Précautions
• Dispositif à un seul usage. NE PAS RESTERILISER. NE PAS RETRAITER. Respecter la date de péremption indiquée sur l'emballage.
• Avant toute utilisation, choisir correctement la taille du stent à utiliser et la vérifier.
• Ne pas retirer le stent du ballonnet porteur. Le stent n'est pas conçu pour être monté sur un autre ballonnet. Le retrait du stent de son ballonnet
introducteur peut endommager le stent et/ou son revêtement ou produire une embolisation de celui-ci.
• Une mauvaise manipulation peut distendre le contact du stent avec le ballonnet ainsi qu'endommager le revêtement.
• Retirer précautionneusement le cathéter de l'emballage, le placer sur le guide et le faire passer par l'adaptateur de la valve hémostatique et le
raccord du cathéter-guide, pour ne pas manipuler le stent ni le changer de place dans le dispositif introducteur.
• Ne pas utiliser d'air ni de produit gazeux pour gonfler le ballonnet.
• Ne pas utiliser de solvants organiques ni de produits à base d'huile (comme les produits de contraste Ethiodol ou Lipiodol, par exemple) ou de
produits très visqueux.
• Il est déconseillé de mettre le stent en contact avec quelque fluide que ce soit avant de le positionner car cela pourrait provoquer la libération du
médicament. Cependant, si cela est absolument nécessaire, humidifier le stent avec une solution saline isotonique / stérile, avec une durée de contact
de 1 minute au maximum.
• Si le stent ne peut pas se déployer, retirer doucement le système tout entier et ne pas manipuler le stent avec les mains.
Installation du stent - Précautions
• Ne pas préparer ou pré-gonfler le ballonnet de manière différente de celle indiquée tant que le stent n'est pas déployé. Suivre la technique de
purge du ballonnet décrite dans la section Instructions pour l'opérateur.
• L'implantation d'un stent peut conduire à la dissection du vaisseau distal ou proximal par rapport à la partie sténosée et peut entraîner une occlusion
sévère du vaisseau, ce qui engendrera la nécessité d'une intervention supplémentaire (par exemple, PAC, grande dilatation, installation de stents
supplémentaires, etc.)
• Il est conseillé de dilater préalablement et de manière suffisante la lésion à traiter avant d'implanter le stent. L'implantation directe du stent n'a
pas été testée.
• En cas de résistance inhabituelle lors de l'accès à la lésion avant implantation du stent, il faudra retirer le système de stent et de cathéter-guide
d'un seul bloc.
• Le dispositif doit être manipulé sous surveillance fluoroscopique haute qualité. Ne pas pousser ou retirer le cathéter si le ballonnet n'est pas
totalement dégonflé sous vide d'air. En cas de résistance durant la manipulation, en déterminer la cause avant de continuer.
• Ne pas déployer le stent avant d'avoir réussi à le placer correctement sur la sténose. Relever la position grâce aux marqueurs radio-opaques.
• Le positionnement du stent peut compromettre la perméabilité de la branche latérale.
• La pression du ballonnet ne doit pas dépasser la pression estimée de rupture. Il est conseillé d'utiliser un dispositif de régulation de la pression
afin d'éviter de dépasser la pression d'éclatement du ballonnet. Au moins 99,9 % des ballonnets (avec une fiabilité de 95 %) n'éclatent pas lorsqu'ils
atteignent le niveau (ou au-dessous) de pression maximale recommandée. L'utilisation de pressions plus élevées que celles spécifiées peut entraîner
la déchirure du ballonnet ainsi que d'éventuels dommages internes et une dissection.
• Les méthodes d'extraction du stent (guides supplémentaires, liens, forceps) peuvent provoquer un traumatisme vasculaire supplémentaire. Les
hémorragies, hématomes ou pseudo-anévrisme font partie des complications possibles.
Retrait du système de stent - Précautions
• Si le stent ne peut pas se déployer, retirer doucement le système tout entier, surtout lors du retrait vers le cathéter-guide, afin d'éviter que le stent
s'accroche à la pointe du cathéter-guide.
• En cas de résistance, retirer le système tout entier y compris le cathéter-guide, d'un seul bloc. Cette opération doit être réalisée par visualisation
fluoroscopique directe. Dans ce cas, suivre les étapes suivantes :
- Ne pas retirer le système de positionnement à l'intérieur du cathéter-guide.
- Placer le marqueur radio-opaque proximal du ballonnet face à l'extrémité distale du cathéter-guide.
- Pousser le guide dans l'anatomie coronarienne le plus loin possible.
- Appuyer sur la valve hémostatique rotative et retirer tout l'ensemble doucement, en un seul bloc.
- Le non-respect de ces étapes ou une pression excessive sur le système de stent peuvent provoquer une fuite ou endommager le stent ou les
composants du système de stent (le ballonnet, par exemple).
Post-implantation - Précautions
• Prendre un maximum de précautions pour croiser un stent récemment déployé avec un cathéter à ultrasons intravasculaire ou un cathéter à
ballonnet afin de ne pas altérer la géométrie du stent ou du revêtement.
• Après l'implantation récente d'un stent, éviter les procédures de résonance magnétique sur le patient afin de réduire le risque de mouvement du
stent. Il est nécessaire d'attendre 2 à 3 mois afin de s'assurer de l'endothélisation du stent.
• Il est nécessaire de prescrire un traitement double antiplaquettaire (AAS et clopidogrel ou ticlopidine) au moins durant les 6 mois suivant le traitement
et, de préférence, durant 12 mois aux patients présentant un faible risque d'hémorragie afin de réduire le risque de thrombose au niveau de la zone
du stent. L'aspirine devra être administrée sans interruption.
Interactions médicamenteuses
• Les éventuelles interactions du simvastatine avec les médicaments administrés simultanément n'ont pas été étudiées. Les interactions avec des
médicaments des niveaux systémiques du Simvastatine sont détaillées dans la fiche technique du produit mais, si l'on prend en compte que le stent
IRIST SMALL présente des niveaux de Simvastatine 1000 fois inférieurs aux doses utilisées dans les applications systémiques du médicament, ces
interactions sont imprévisibles.
• Dans le cas spécifique d'une administration associée d'une statine par voie orale, les calculs réalisés démontrent qu'il n'est pas possible de prévoir
l'augmentation du risque de toxicité chez les patients traités avec le Stent IRIST SMALL et recevant de plus des statines par voie orale.
Grossesse
• Ce produit n'a pas été testé chez les femmes enceintes ou chez les hommes souhaitant avoir des enfants car il est recommandé aux patients
recevant le dispositif d'éviter toute grossesse/procréation. Bien qu'il n'y ait aucune contre-indication, les risques et effets sur la reproduction restent
méconnus.
5. Effets indésirables éventuels

Inspection avant utilisation
• Inspection soigneuse de l'emballage avant son ouverture. Ne pas utiliser après la « date de péremption ». En cas de dommages éventuels sur
l'emballage, ne pas utiliser le produit et contacter Iberhospitex pour savoir comment résoudre le problème. Ne pas utiliser en cas de défaut.
• Si, au cours de son utilisation, le corps proximal en acier inoxydable s'est dédoublé ou coudé, ne pas continuer d'utiliser le cathéter.

Préparation
• Préparer le dispositif de gonflage avec le produit de contraste recommandé selon les instructions du fabricant. Purge l'air du système.
• Sélectionner correctement le stent en ce qui concerne la longueur et le diamètre. Le diamètre du ballonnet (stent) ne doit pas dépasser le diamètre
de l'artère à dilater avant et après la sténose.
• Laisser le cathéter dans son étui de protection. Retirer l'extrémité distale de l'anneau de protection pour voir le stent.
• Vérifier que le stent se trouve fixé entre les marques du ballonnet et ne pas le manipuler avec les doigts car cela pourrait l'endommager.
• Préparer le cathéter de dilatation pour la purge. Remplir la seringue de 20 cc équipée d'un luer lock avec 3 cc de la solution de contraste appropriée.
MISE EN GARDE : Ne pas utiliser la technique de gonflage précédente pour purger le lumen du ballonnet. Ne pas utiliser d'air ni d'autre produit
gazeux pour gonfler le ballonnet. Ne pas éliminer la pression négative du dispositif de gonflage.
• Connecter la seringue au lumen du ballonnet, orienter le cathéter avec la pointe distale et le ballonnet verticaux tournés vers le bas.
• Appliquer une pression négative et aspirer durant 5 secondes. Pousser le piston doucement.
• Retirer la seringue du lumen de gonflage du ballonnet et retirer tout l'air.
• Reconnecter la seringue au cathéter et maintenir une pression négative dans le ballonnet jusqu'à ce qu'il arrête de renvoyer de l'air dans le dispositif.
Libérer la pression de la seringue jusqu'au point neutre et la déconnecter.
• Connecter le dispositif de gonflage au port de gonflage du cathéter sans introduire d'air dans le système.
MISE EN GARDE : Eviter d'appliquer une pression positive au système de libération, maintenir le dispositif de gonflage à la pression ambiante.
Technique d'insertion
• Introduire le cathéter-guide selon les techniques Standard.
• Dilater préalablement la lésion avec un ballonnet de diamètre adéquat ayant une relation de 1:1 avec le diamètre du vaisseau. Cela fait, laisser le
guide en position et retirer le cathéter à ballonnet.
Positionnement du stent
• Pousser le système de libération dans le guide jusqu'à la zone souhaitée. Placer le stent sur la lésion à l'aide des marques radio-opaques du
ballonnet de libération. EN CAS DE RESISTANCE DURANT L'INSERTION DU STENT, en déterminer la cause avant de poursuivre.
• Gonfler le système de stent en étendant le stent à une pression qui permette de garantir la bonne mise en place du stent contre la paroi artérielle.
Utiliser le tableau des pressions de déploiement. Ne pas dépasser la pression maximale recommandée.
• Lorsque le stent est gonflé et en position, dégonfler le ballonnet et le retirer doucement par le guide.
• Confirmer la position correcte du stent en réalisant une angiographie par le cathéter-guide. Pour des résultats optimaux, la totalité du segment
artériel sténosé doit être couvert par le stent. Pour une expansion optimale, le stent doit rester complètement en contact avec la paroi artérielle. Il
est nécessaire de prendre toutes les mesures nécessaires pour vérifier que le stent est assez dilaté.
Post-dilatation
• Il peut être nécessaire de post-dilater le stent une fois celui-ci positionné Pour cela, utiliser le cathéter de libération ou un autre ballonnet de taille
adéquate et le gonfler à la place du stent. Confirmer les résultats à l'aide d'une angiographie. Dilater le ballonnet autant de fois que nécessaire jusqu'à
obtenir la dilatation parfaite du stent.
• Cela fait, dégonfler le ballonnet et le retirer doucement. Retirer le guide et le cathéter-guide.
8. Avertissements sur la réutilisation
Le contenu s'administre de manière stérile à l'aide d'oxyde d'éthylène (OE). Ne pas utiliser si la barrière stérile est endommagée. En cas de dommages,
contacter le représentant de Iberhospitex.
À utiliser sur un seul patient. Ne pas réutiliser, retraiter ou restériliser. La réutilisation, le retraitement ou la restérilisation peuvent compromettre l'intégrité
structurelle du dispositif et/ou causer une défaillance, ce qui, à son tour, peut entraîner des lésions sur le patient, une maladie ou la mort. La réutilisation,
le retraitement ou la restérilisation peuvent également créer un risque de contamination du dispositif et/ou provoquer l'infection ou infection croisée
sur le patient, y compris mais non limité à la transmission de maladies infectieuses d'un patient à un autre. La contamination du dispositif peut provoquer
des lésions, des maladies ou la mort du patient.
9. Garantie
Iberhospitex ne peut être tenu responsable des dommages causés aux patients dus à la mauvaise utilisation du cathéter ou du stent et, par conséquent,

1. Descrição do dispositivo
O sistema de stent coronário com eluição de simvastatina, IRIST SMALL, é um stent de eluição de fármacos expansível por balão pré-montado,
composto por:
• Um stent Apolo Small de aço inoxidável 316 L para uso médico que inclui um fármaco (uma formulação de simvastatina contida num revestimento
polimérico). O revestimento é composto por um portador polimérico carregado com 0,36 µg/mm2 de simvastatina num sistema de libertação lenta,
com uma quantidade nominal máxima de 56 µg no stent de maior tamanho (2,75 mm x 36 mm).
• Cateter balão de dilatação de substituição rápida semidistensível com suporte de alta pressão.
• O balão tem marcadores radioopacos para a colocação precisa do stent, e foi concebido para proporcionar um segmento expansível a um diâmetro
e comprimento conhecidos com aplicação de uma pressão específica.
• O stent é fornecido em diferentes comprimentos (9, 14, 19, 23, 28 e 36 mm) e diâmetros (2,0; 2,25; 2,50 e 2,75 mm).
Descrição do fármaco
O stent que faz parte do sistema de stent coronário IRIST SMALL é um stent revestido com uma formulação de fármaco e polímero composta por
simvastatina (ingrediente activo) e polímero portador P-5 (ingrediente não activo).
Simvastatina
O componente farmacêutico activo do stent IRIST SMALL é a simvastatina. A simvastatina é um pó cristalino branco ou quase branco, com fórmula
molecular C25H38O5 e peso molecular 418,6. É um fármaco de uso aprovado para o tratamento da hipercolesterolemia e para a prevenção cardiovascular.
Portador polimérico P-5
O polímero P-5 é um ingrediente inactivo do stent IRIST SMALL. É um copolímero acrílico elastomérico, altamente flexível e hidrófobo. O polímero
é misturado com o fármaco simvastatina e depois aplicado nos stents, obtendo-se um revestimento de fármaco e polímero em toda a superfície
luminal e abluminal do stent.
2. Indicações e uso
O stent eluente de simvastatina IRIST SMALL é indicado para o melhoramento do diâmetro luminal coronário e reduzir a re-estenose que se verifica
no interior e nas bordas do stent durante o tratamento de lesões novas em artérias coronárias com diâmetro do vaso de referência de 2,0 mm a 2,75
mm.
O stent é indicado para lesões com comprimentos inferiores ao comprimento nominal do stent utilizado (9, 14, 19, 23, 28 ou 36 mm).
Critérios de inclusão geral
1. Elegível para intervenção coronária transluminal percutânea (ICTP).
2. Candidato aceitável para enxerto de derivação da artéria coronária (IDAC).
Critérios de inclusão para angiografia
1. A lesão a tratar está situada dentro do vaso coronário nativo.
2. Estenose do diâmetro da lesão a tratar 50% por estimativa visual ou 50% por estimativa mediante análise angiográfica quantitativa automática
(QCA).
3. Contra-indicações
A implantação do sistema Stent IRIST SMALL é contra-indicada e deve ser excluída em doentes que preencham algum dos critérios seguintes:
Critérios de exclusão geral:
• Sensibilidade conhecida ao simvastatina.
• Alergia conhecida ao aço inoxidável.
• Alergia aos medicamentos e/ou materiais utilizados durante o procedimento.
• Contra-indicações para a terapia de anticoagulação e/ou anti-plaquetária.
• A função do ventrículo esquerdo do doente está gravemente comprometida (< 30%).
• Implantação directa de um stent.
• O doente sofreu de enfarte do miocárdio nas 48h anteriores ao procedimento com o IRIST SMALL.
• Choque cardiogénico.
• Presença conhecida ou provável de trombose intraluminal.
• Re-estenose intra-stent.
• Lesões na bifurcação.
• Implantação de um stent em enxertos da veia safena.
Critérios de exclusão para angiografia:
• Lesões superiores a 50% situadas no tronco coronário comum esquerdo não protegido.
• A lesão a tratar ou o vaso proximal estão gravemente calcificados.
• Oclusão total crónica do vaso em tratamento.
• A lesão a tratar tem uma tortuosidade grave, inadequada para a introdução e desdobramento do stent.
4. Advertências e precauções
Advertências
• A utilização deste produto envolve os riscos associados à trombose subaguda, complicações vasculares ou episódios de hemorragia. Por isso,
devem-se seleccionar cuidadosamente os doentes e prescrever o tratamento anti-plaquetário duplo (AAS e clopidogrel ou ticlopidina) No ensaio
clínico administrou-se o tratamento antiplaquetário durante 6 meses pós-procedimento, pelo que o tratamento nunca poderá ser inferior a 6 meses,
e convenientemente pelo menos durante 12 meses após o procedimento em doentes com baixo risco de sangramento. A aspirina deverá ser
administrada indefinidamente.
• É muito importante que se proporcionem as informações adequadas ao doente sobre as recomendações anti-plaquetárias posteriores à intervenção.
A cessação prematura do medicamento anti-plaquetário receitado aumentará o risco de trombose seguida de enfarte do miocárdio ou morte.
• Este dispositivo não deve ser aplicado em doentes com hipersensibilidade confirmada ao simvastatina ou ao aço inoxidável 316 L.
• O aparecimento posterior de re-estenose pode exigir que se volte a dilatar o segmento de artéria que contém o stent. Desconhece-se de momento
o resultado da dilatação repetida do stent.
• A solapamento do Stent IRIST Small não foi estudada. Quando for necessário mais de um stent, se a colocação envolver contacto entre os
mesmos, o material dos dois stents deve ser de composição similar.
• Não usar stents de composições diferentes para expandir o mesmo vaso porque o risco de trombose aumenta especialmente nas artérias de
calibre reduzido.
• A implantação do stent deve só deve ser efectuada em hospitais equipados com meios de emergência ou em hospitais com facilidade de acesso
a outros hospitais que possam efectuar operações de coração aberto caso surjam complicações.
• A implantação só deve ser efectuada por médicos altamente familiarizados e treinados na técnica de execução de angioplastias coronárias
transluminais percutâneas e desdobramento de stent.
Precauções
Manuseamento do stent - Precauções
• Dispositivo de um só uso. NÃO VOLTE A ESTERILIZAR. NÃO VOLTE A PROCESSAR. Respeite a data de caducidade da embalagem.
• Antes do seu uso, escolher correctamente o tamanho do stent a utilizar e o verifica.
• Abstenha-se de retirar o stent do balão portador em que é fornecido. O stent não foi concebido para ser montado em outro balão. A remoção do
stent do seu balão introdutor pode danificar o stent e/ou o seu revestimento ou provocar uma embolização do mesmo.
• O manuseamento inadequado pode afrouxar o contacto do stent com o balão e além disso danificar a cobertura.
• Preste especial atenção ao retirar o cateter da embalagem, colocá-lo sobre a guia e fazê-lo avançar através do adaptador da válvula hemostática
e do conector do cateter-guia, para não manusear o stent nem o deslocar de qualquer forma da sua posição no dispositivo introdutor.
• Não utilize ar nem meios gasosos para insuflar o balão.
• Não usar solventes orgânicos nem produtos fabricados com base oleosa (tal como os meios de contraste Ethiodol ou Lipiodol) nem com uma
viscosidade elevada.
• Não é recomendado o contacto do stent com qualquer fluido antes da colocação, dado que se poderia verificar libertação de fármaco. No entanto,
se for absolutamente necessário, irrigue o stent com solução salina isotónica/estéril, sendo o tempo máximo de contacto 1 minuto.
• Se o stent não puder ser desdobrado, retire cuidadosamente todo o sistema e não manuseie o stent com as mãos.
Colocação do stent - Precauções
• Não prepare nem pré-insufle o balão antes do desdobramento do stent de forma diferente da indicada. Siga a técnica de purga do balão descrita
na Secção de Instruções para o operador.
• O implante de um stent pode conduzir à dissecação do vaso distal ou proximal na porção estenosada e pode causar um fecho agudo do vaso
que exigirá uma intervenção adicional (ex. IDAC, maior dilatação, colocação de stents adicionais, etc.)
• Recomenda-se que se dilate previamente e de forma suficiente a lesão a tratar antes da implantação do stent. Não foi avaliado o implante directo
de stent
• Se sentir uma resistência invulgar em qualquer momento durante o acesso à lesão antes da implantação do stent, o sistema de stent e cateterguia devem ser retirados como uma só unidade.
• O dispositivo deve ser manuseado sob observação fluoroscópica de alta qualidade. Não faça avançar nem retroceder o cateter salvo se o balão
estiver totalmente desinflado sob vácuo. Se durante o manuseamento encontrar resistência, determine a causa da mesma antes de prosseguir.
• Não desdobre o stent enquanto não tiver conseguido posicioná-lo correctamente na estenose. Observe a posição através das marcas radioopacas.
• A colocação do stent pode comprometer a permeabilidade de um ramo lateral.
• A pressão do balão não deveria exceder a pressão de ruptura prevista. Recomenda-se o uso de um dispositivo de controlo da pressão para evitar
ultrapassar a pressão de estalido do balão. Pelo menos 99,9% dos balões (com uma fiabilidade de 95%) não se romperão ao ser atingido o nível
(ou inferior ao mesmo), da pressão máxima recomendada. O uso de pressões mais elevadas do que as especificadas, pode causar ruptura do balão
e eventuais danos na íntima e dissecação.
• Os métodos de extracção do stent (guias adicionais, laços, fórceps), podem provocar um traumatismo vascular adicional. Estão incluídos nas
complicações a hemorragia e o hematoma ou pseudo-aneurisma.
Remoção do sistema de stent - Precauções
• Se o stent não puder ser desdobrado, retire cuidadosamente todo o sistema, especialmente quando for retirado para o cateter-guia para evitar
que o stent fique engatado na ponta do cateter-guia.
• Se notar alguma resistência, retire todo o sistema incluindo o cateter-guia, como se fossem uma só unidade. Isto deve ser feito sob visualização
fluoroscópica directa. Neste caso, siga estes passos:
- Abstenha-se de remover o sistema de posicionamento dentro do cateter-guia.
- Posicione a marca radioopaca proximal do balão de frente para a extremidade distal do cateter-guia.
- Faça avançar a guia na anatomia coronária até ao ponto mais distante possível.
- Aperte a válvula hemostática rotativa e retire cuidadosamente todo o conjunto como se fosse uma só unidade.
- Se estes passos não forem seguidos ou se aplicar uma força excessiva ao sistema de stent, pode provocar perda ou danos no stent ou nos
componentes do sistema de stent, como por exemplo, o balão.
Pós-implante - Precauções
• Tenha o máximo cuidado ao cruzar um stent recém-desdobrado com um cateter de ultra-som intravascular ou um cateter de balão, dado que a
geometria do stent ou da cobertura se pode alterar.
o Após a implantação recente de um stent evite procedimentos de ressonância magnética no doente para reduzir a possibilidade de movimento
do stent. Deve aguardar 2-3 meses para garantir a endotelização do stent.
o É necessário prescrever um tratamento duplo antiplaquetário (AAS e clopidogrel ou ticlopidina) pelo menos durante 6 meses após o procedimento
e recomendável durante 12 meses em doentes com baixo risco de sangramento, para reduzir o risco de trombose na zona do stent. A aspirina deverá
ser administrada indefinidamente.
Interacções medicamentosas
• Não foram investigadas as eventuais interacções do simvastatina com as medicações administradas de forma concomitante. As interacções com
medicamentos dos níveis sistémicos de simvastatina estão detalhados na ficha técnica do produto mas, se tivermos em atenção que o stent IRIST
SMALL tem níveis de simvastatina 1000 vezes inferiores aos das doses usadas em aplicações sistémicas do fármaco, tais interacções não são
previsíveis.
• No caso específico de administração conjunta de uma estatina por via oral, os cálculos efectuados demonstram que não é previsível o aumento

Gravidez
• Este produto não foi avaliado em mulheres grávidas ou em homens que desejem ter filhos, pelo que se recomenda aos receptores do dispositivo
que evitem a gravidez. Embora não tenha contra-indicações, os riscos e efeitos reprodutivos continuam a ser desconhecidos.
5. Eventuais episódios adversos
Os episódios adversos seguintes podem estar associados, entre outros, à colocação de um stent coronário nas artérias coronárias nativas:

• Transtornos da pele e do tecido subcutâneo
6. Forma de fornecimento
Estéril: Este produto está esterilizado com óxido de etileno e é apirogénio. De um só uso. Não o utilize se a embalagem não estiver integral.

7. Instruções para o operador
Inspecção antes do seu uso
• Inspeccione cuidadosamente a embalagem antes de a abrir. Não use depois da “data de caducidade”. Se se observarem eventuais danos na
embalagem não utilize o produto, e entre em contacto com a Iberhospitex para obter informações sobre a forma de o resolver. Não o utilize se observar
algum defeito.
• Em qualquer momento durante a sua utilização, se verificar que o corpo proximal de aço inoxidável se dobrou ou encurvou, não continue a usar
o cateter.

Preparação
• Prepare o dispositivo de enchimento com o meio de contraste recomendado, de acordo com as instruções do fabricante. Purgue o ar do sistema.
• Seleccione correctamente o stent quanto a comprimento e diâmetro. O diâmetro do balão (stent) não deve exceder o diâmetro da artéria a dilatar
antes e depois da estenose.
• Deixe o cateter dentro do seu tubo de protecção. Retire a extremidade distal do aro de protecção para ver o stent.
• Certifique-se de que segura o stent entre as marcas do balão; não toque com os dedos, dado que se poderia danificar.
• Prepare o cateter de dilatação para a purga. Encha a seringa de 20 cc com fecho luer com 3 cc da solução de contraste apropriada.
ADVERTÊNCIA: Não utilize a técnica de enchimento prévio para purgar o lúmen do balão. Não use ar nem qualquer outro meio gasoso para encher
o balão. Não retire pressão negativa do dispositivo de enchimento.
• Conecte a seringa ao lúmen do balão, oriente o cateter com a ponta distal e o balão apontando verticalmente para baixo.
• Aplique pressão negativa e aspire durante 5 seg. Solte o êmbolo lentamente.
• Retire a seringa do lúmen de enchimento do balão e retire todo o ar.
• Volte a conectar a seringa ao cateter e a manter pressão negativa no balão até que deixe de restituir ar ao dispositivo. Liberte a pressão da seringa
até ao ponto neutro e desconecte-a.
• Conecte o dispositivo de enchimento ao bocal de enchimento do cateter sem introduzir ar no sistema.
ADVERTÊNCIA: Evite aplicar pressão positiva ao sistema de libertação; mantenha o dispositivo de enchimento à pressão ambiente.
Técnica de inserção
• Introduza o cateter-guia mediante técnicas Standard.
• Pré-dilate a lesão com um balão de diâmetro adequado que tenha uma relação de 1:1 com o diâmetro do vaso. Uma vez pré-dilatada a lesão,
deixe a guia em posição e retire o cateter-balão.
Posicionamento do stent
• Faça avançar o sistema de libertação através da guia até o posicionar no lugar adequado. Coloque o stent sobre a lesão utilizando as marcas
radioopacas do balão de libertação. CASO ENCONTRE RESISTÊNCIA DURANTE A INSERÇÃO DO STENT, não prossiga enquanto não determinar
a causa.
• Insufle o sistema de stent expandindo o stent até uma pressão tal que fique garantida a correcta aposição do stent contra a parede arterial. Utilize
a tabela de pressões de desdobramento. Não exceda a pressão máxima recomendada.
• Quando o stent tiver sido enchido e posicionado, desinsufle o balão e retire-o cuidadosamente através da guia.
• Confirme a posição correcta do stent, efectuando uma angiografia através do cateter-guia. Para obter resultados óptimos, todo o segmento arterial
estenosado deve estar coberto pelo stent. Uma óptima expansão exige que o stent fique totalmente posicionado em contacto com a parede arterial.
Devem-se tomar todas as medidas necessárias para se certificar de que o stent não está subdilatado.
Pós-dilatação
• Pode ser necessário pós-dilatar o stent uma vez posicionado. Para tal, utilize o cateter de libertação ou outro balão do tamanho adequado e insufleo em vez do stent. Confirme os resultados mediante angiografia. Dilate o balão todas as vezes necessárias até conseguir a perfeita dilatação do stent.
• Depois de ter o stent pós-diladado, esvazie o balão e retire-o cuidadosamente. Retire a guia e o cateter-guia.
8. Advertências sobre a reutilização
O conteúdo é fornecido ESTERILIZADO mediante óxido de etileno (OE). Não o use se a barreira estéril estiver danificada. Se encontrar danos, ligue
para o representante da Iberhospitex.
Para uso num só doente. Não volte a usar, processar ou esterilizar. A reutilização, o reprocessamento ou a reesterilização podem comprometer a
integridade estrutural do dispositivo e/ou causar a sua falha, o que por sua vez pode resultar em lesões no doente, doença ou morte. A reutilização,
reprocessamento ou reesterilização também podem constituir risco de contaminação do dispositivo e/ou causar infecção ou infecção cruzada no
doente, incluindo sem limitação a transmissão de doenças infecciosas de um doente para outro. A contaminação do dispositivo pode causar lesões,
doença ou morte do doente.
9. Garantia
A Iberhospitex não se considera responsável pelos danos causados a doentes por uma utilização errónea do cateter ou do stent; por isso, não aceitará
reclamações à excepção das aqui especificadas. A Iberhospitex não se considerará responsável, seja em que circunstância for, por qualquer dano
causado, à excepção dos especificamente dispostos na lei.
