This guide has been created to explain the format for completing each section and
highlight common errors.
Please note, you cannot save this form part completed. We recommend that you
type up any detailed answers (such as your Personal Statement) in a word
processor or notepad so you can copy and paste into this form. Items marked with
an * must be filled in.
1 Personal Details and Current School

4 Personal Statement
We recommend that you type up your Personal Statement in a word processor or notepad
so you can copy and paste into this form.
Your Personal Statement should be 1 side of A4 approx. and should explain why feel you
are a suitable candidate for Sixth Form at Arthur Mellows Village College.
Example Personal Statements are attached at the end of this document

If taking Further Mathematics, you must also take Mathematics.
Do not select more than 1 English option or Media
Applied Health and Social Care is available as both a Single and Double award. If
choosing as a Single option, select the Single option only, if selecting as a Double
award, please select both.

Please note, an offer of a place in Sixth Form will depend on:
 Having passed at least seven full subjects at grades A* to C in
GCSE examination (or equivalent) including four at grade B or
above. A vocational subject can count as a maximum of two
GCSE equivalents.
 A recommendation from your current school, as suitable for
successful Sixth Form study in four subjects of your choice, from
the range offered by the College.
 Having satisfied the entry requirements specified for each Sixth
Form course.
Internal candidates will be expected to bring their Achievement File
with you to an interview.
External candidates will be expected to bring example work for
appropriate subjects to their interview.
Final courses are always subject to minimum numbers and may not
run if not viable.
