Device Description The Durango device is an intervertebral body fusion device consisting of a PEEK-OPTIMA intervertebral spacer, titanium plate, and screws. The spacer has a generally rounded shape with various heights and footprints and has a hollowed out central area to accommodate autogenous bone graft. The upper and lower surfaces have a series of transverse grooves formed to improve stability and fixation once the device is inserted. The titanium plate has holes for receiving bone screws and a central hole for receiving a cover plate to prevent screw back-out. The Durango system is available in a variety of sizes and configurations to approximate anatomical variation in different vertebral levels and/or patient anatomy. The Durango system is provided non-sterile. Materials The Durango system consists of plates, screws, and intervertebral spacers. The plates and screws are made from an implant grade titanium alloy (Ti-6Al-4V ELI) meeting the requirements of ASTM F136-08 and ISO 5832-3. PEEK-OPTIMA® LT1 polymer is a polyaromatic semicrystalline thermoplastic, and meets the requirements of ASTM F-2026 and ISO 10993. Chemically the material has the following basic formula, (-C6H4-O-C6H4-O-C6H4-CO-)n, and is known generically as polyetheretherketone (PEEK). All PEEK-OPTIMA implants are made from this implant grade material with tantalum position markers meeting the requirements of ASTM F560. The PEEK-OPTIMA device is radiolucent. Titanium and cobalt chrome implants should never be used with stainless steel implants. Indications for Use When used as a lumbar intervertebral body fusion device, the Durango system is intended for spinal fusion procedures to be used with autogenous bone graft in skeletally mature patients with degenerative disc disease (“DDD”) at one or two contiguous spinal levels from L2-S1. DDD is defined as discogenic back pain with degeneration of the disc confirmed by history and radiographic studies. These patients should have had six months of non-operative treatment. These DDD patients may have had a previous non-fusion spinal surgery at the involved spinal level(s), and may have up to Grade 1 spondylolisthesis or retrolisthesis at the involved level(s). The Durango system is to be implanted via an anterior or posterior approach and is to be combined with supplemental fixation (except as noted below). Approved supplemental fixation systems include the Biomet Spinal Fixation System. The Durango standalone interbody implants, when used with the integrated fixation screws, do not require use of supplemental fixation. Contraindications Contraindications may be relative or absolute. The choice of a particular device must be carefully weighed against the patient’s overall evaluation. Circumstances listed below may reduce the chance of a successful outcome. Contraindications include, but are not limited to: • Allergy to PEEK-OPTIMA, titanium or cobalt chrome alloys, or foreign body sensitivity. Where material sensitivity is suspected, appropriate tests should be made prior to implantation. • Known or suspected infection/immune system incompetence. Acute or chronic infectious diseases of any etiology or localization. • Any abnormality present which affects the normal process of bone remodeling including, but not limited to, severe osteoporosis involving the spine, bone absorption, osteopenia, active infection at the site or certain metabolic disorders affecting osteogenesis. • Morbid Obesity. An overweight or obese patient can produce loads on the spinal system, which can lead to failure of the fixation of the device or failure of the device itself. • Any neuromuscular deficit which places an unusually heavy load on the device during the healing period. • Open Wounds. • Pregnancy. • Any other medical or surgical condition which would preclude the potential benefit of spinal surgery, such as the presence of congenital abnormalities, elevation of sedimentation rate unexplained by other diseases, elevation of the white blood count (WBC), or a marked left shift in the WBC differential count. • Any case requiring the mixing of components from two different systems. • Any case requiring the mixture of stainless steel with titanium, or stainless steel with cobalt chrome implant components. • Fever or leukocytosis. • Signs of local infection or inflammation. • Previous history of infection. • Prior fusion at the level to be treated. • Alcoholism or heavy smoking. • Senility, mental illness or substance abuse, of a severity that the patient may ignore certain necessary limitations and precautions in the use of the implant, leading to failure or other complications. • Any patient unwilling to follow postoperative instructions. • Inadequate tissue coverage over the operative site. Possible Complications Possible complications specific to the device may include: • Early or late implant bending, breakage, failure, loosening or movement/migration • Bone fracture • Allergic reaction to implant material • Other general complications associated with any spinal surgical procedure may include: Non-union or delayed union, pseudoarthrosis; pain; second surgery; bleeding; infection, early and late; tissue or nerve damage, including dural tears or other neurological problems; incisional complications; scar formation; damage to blood vessels and cardiovascular system compromise; changes in mental status; damage to internal organs and connective tissue; complications due to the use of bone grafting, including graft donor site complications; respiratory problems; reactions to anesthesia and/or death. Warnings Patients with previous spinal surgery at the levels to be treated may have different clinical outcomes than previous surgical outcomes. The risk of a device expulsion and migration is higher without the use of integrated fixation screws as indicated. Precautions • The Durango implants are for single use only. Never reuse any implant even if it appears unmarked or undamaged. Reuse of the implant components may result in reduced mechanical performance, malfunction, or failure of the device. Any implant implanted and then removed must be discarded. Use only new implants for each case. • Only experienced spinal surgeons should perform the implantation of this system with specific training in the use of vertebral implants. The surgical procedure is technically demanding and presents a risk of serious injury to the patient. • The Durango system is intended to be used only by surgeons specialized in spinal surgery and having thorough knowledge of vertebral anatomy, regional vertebral morphology, and the biomechanical principles of the spine. It is advised that the surgeon also be thoroughly familiar with the surgical techniques relative to the use of the device. • Based on the fatigue testing results, the physician/surgeon must consider the levels of implantation, patient weight, patient activity level, other patient conditions, etc. which may impact on the performance of the system. • Risks associated with neurosurgery, general surgery, orthopedic surgery, and the use of general anesthesia should be explained to the patient prior to surgery. It is recommended that the advantages and disadvantages of using implants, as well as alternative treatment methods, are explained to the patient. • Preoperatively: The surgeon must be fully conversant with all aspects of the surgical technique and know the indications and contra-indications of this type of implant. The surgeon must have acquainted himself before the operation with the specific technique for insertion of the product, which is available from the manufacturer. As part of the preoperative examination, the surgeon must check that no biological, biomechanical or other factors will affect the correct conduct of the operation and the postoperative period. An appropriate range of implant sizes must be available at the time of the operation. • Intraoperatively: The correct selection of the type and size of implant appropriate to the patient and the positioning of the implant are extremely important. • Postoperatively: Patients must be informed of the precautions to be taken in their everyday life to guarantee a maximum implant service life. It is recommended that regular postoperative follow-up is undertaken to detect early signs of failure of the implants and to consider the action to be taken. Deterioration of the device after bone consolidation cannot be considered to constitute a dysfunction or deterioration in the characteristics of the implants. The implant can be removed after bony healing. • Correct selection and placement of the implants is extremely important. Implant selection must be based upon the bone defect to be treated as well as the patient’s weight, height, occupation, or degree of physical activity. • Proper handling of the implant before and during the operation is crucial. • Use of the cover plate to prevent back-out of the screws is mandatory. • If a cover plate is disassembled from a plate, it must be discarded and not reused. • If a plate is disassembled from a PEEK-OPTIMA interbody implant, it must be discarded and not reused. • The Durango device must not be used with vertebral components or instruments from other manufacturers. • Before use, inspect all instrumentation for possible damage, wear, or non-function. Damaged or defective instruments should not be used or processed. Contact your local Biomet representative or distributor for repair or replacement. • The use of an instrument for tasks other than those for which they are indicated may result in damaged or broken instruments. • Do not apply excessive force or stress. Misuse can damage instruments or implants. • Perform a careful preoperative review to be sure that all necessary implant components are available and that the instrument set is complete and in working order prior to initiating surgery. • The Durango system has not been tested for safety and compatibility in the magnetic resonance (MR) environment. The Durango system has not been tested for heating or migration in the MR environment. • Mixing of dissimilar metals can accelerate or initiate the corrosion process. Titanium components must NOT be used together in building a construct that involves other implant materials. Titanium and cobalt chrome may be used together within the same construct. Directions for Use The information contained in this package insert is necessary but not sufficient for the use of this device. This information is not intended as a substitute for the professional judgment, skill and experience of the surgeon in: careful patient selection; preoperative planning; device selection; knowledge of the anatomy and biomechanics of the spine; understanding of the material and the mechanical characteristics of the implants used; training and skill in both spinal surgery and use of associated instruments for implantation; securing the patient’s cooperation in following an appropriately defined postoperative management program; and, conducting postoperative follow-up examinations. The Durango implant is surgically implanted via an anterior surgical approach. Review and inspect all instrumentation and implants prior to sterilization. Replace or add any needed components for the planned surgery. The primary surgeon must be fully experienced with the required spinal fusion techniques. Prepare, position, and drape the patient in the usual fashion. A standard anterior surgical approach is performed. The appropriate level discectomy and/or corpectomy are completed, and bone graft is placed. Record the lot numbers of the implants that were placed in the patient during surgery in the patients chart. Surgical Guidelines Preoperative Procedure The surgeon must be fully conversant with all aspects of the surgical technique and know the indications and contraindications of this type of implant. The surgeon must be acquainted before the operation with the specific technique for insertion of the product which is available from the manufacturer. As part of the preoperative examination, the surgeon must check that no biological, biomechanical or other factors will affect the correct conduct of the operation and the postoperative period. Only patients that meet the criteria described in the Indications for Use should be selected. Patient conditions and/or pre-dispositions such as those addressed in the aforementioned contraindications should not be selected. Care should be used in the handling and storage of the Durango system components. Implants and instruments should be protected during storage especially from corrosive environments. The type of construct to be assembled for the case should be determined prior to beginning the surgery. The primary surgeon must be fully experienced with the required spinal fusion techniques. Based on the fatigue testing results, the surgeon should consider the levels of implantation, patient weight, patient activity level, other patient conditions, etc. which may have an impact on the performance of the Durango system components. An adequate inventory of implant sizes should be available at the time of surgery, including an excess of implant sizes expected to be used and implant sizes larger and smaller than those expected to fit the patient. Review and inspect all instrumentation and implants prior to sterilization. Replace or add any needed components for the planned surgery. Since mechanical parts are involved, the surgeon should be familiar with the various components before using the equipment, and should personally assemble each device to be used to verify that all parts and necessary instruments are present before the surgery begins. Note should be taken to confirm the surgery date with Torque Limiting Handle calibration expiration date. Damaged or defective instruments should not be used. Contact the manufacturer for repair or replacement instructions. All components should be cleaned and sterilized before use. Additional sterile components should be available in case of an unexpected need. Prepare, position, and drape the patient in the usual fashion. Operative Procedures The surgical placement of the Durango device is similar to osseous interbody fusion procedures using standard procedures. • Patient prep, surgical exposure and fusion procedure (disc space preparation, bone grafting, etc.) are performed per the surgeon’s standard techniques. • The surgeon exposes the appropriate level(s) to be instrumented, and prepares the site using manual surgical instruments as necessary. • Perform any necessary bone and tissue removal. • After preparing the intervertebral disc space, the disc spreader found in the Biomet Spine Disc Preparation case is used to achieve the desired level of distraction (Disc Spreaders are available in 2 mm increments from 8 mm to 20 mm). • Once the disc material is removed and the endplates are prepared at the appropriate level, an implant trial sizer is used to measure the disc space and determine the correct implant size. • Start implant size selection by insertion of the smallest trial height and length. Use progressively increasing height of the trials until the appropriate height distraction is achieved. Radiographic confirmation will determine the appropriate trial height and footprint for the anatomical conditions. • The correct size PEEK-OPTIMA body is selected. The corresponding plate is selected per the surgeon’s preference. The two pieces are assembled using the assembly set screw and calibrated torque handle. The PEEK-OPTIMA body is packed with autogenous bone graft in the hollowed out central area. • The implant is securely locked to the tip of the inserter with a threaded rod and introduced into the disc space and tapped into final position as necessary. Additional graft material placement may be completed at this time. The final position is confirmed using x-ray and radiographic markers. • The implant is secured to the anatomy with four bone screws. The type of screw used (fixed/variable and tip configuration) is based on surgeon preference. The preparation and screw placement is done in a sequential fashion, such that each screw is placed with a driver after its site has been prepared. Preparation is performed using guides, awls, and drills. • After proper placement of the implant and bone screws, the corresponding cover plate is secured to the plate with the cover plate driver and calibrated torque handle. • The construct is then inspected to confirm proper placement of all implants and the surgical site is closed using standard techniques. Postoperative Care Ensure that an analgesic, antibiotic, immobilization, and home care regimen is followed. Patients must be informed of the precautions to be taken in their everyday activities to guarantee a maximum implant service life. It is recommended that a routine postoperative follow-up(s) are undertaken to detect early signs of implant failure and to consider appropriate action. Deterioration of the device after bone consolidation cannot be considered to constitute a dysfunction or deterioration in the characteristics of the implant. A suitable rehabilitation program must be designed and implemented. The physician’s postoperative directions and warnings to the patient and the corresponding patient compliance are extremely important. Adequately instruct the patient in the appropriate postoperative care. The patient’s ability and willingness to follow instructions are some of the most important aspects of successful healing. Recommended advisories include: • Warn the patient to avoid falls or sudden jolts in spinal position. • Provide detailed instructions on the use and limitations of the device to the patient. • If partial weight-bearing is recommended or required prior to firm bony union, the patient must be warned that bending, loosening or breakage of the components are complications which can occur as a result of excessive or early weight bearing or excessive muscular activity. The risk of bending, loosening, or breakage of an internal fixation device during postoperative rehabilitation may be increased if the patient is active, or if the patient is debilitated, demented, or otherwise unable to use weight supporting devices. • To allow maximum chances for a successful surgical result, the patient or device must not be exposed to mechanical vibrations that may loosen the device construct. Warn the patient of this possibility and instruct them to limit and restrict physical activities, especially those that require lifting, twisting motions and any type of physically demanding sport participation. • Advise the patient not to smoke, utilize nicotine products, and consume alcohol, non-steroids, or antiinflammatory medications, such as aspirin, during the bone graft healing process. • Advise the patient of their inability to bend at the point of spinal fusion and instruct them to compensate for this permanent physical restriction in body motion. If a nonunion develops or if the components loosen, bend, and/or break- revise and/or remove the device(s) immediately before serious injury occurs. Failure to immobilize a delayed nonunion of bone will result in excessive and repeated stresses on the implant. By the mechanism of fatigue these stresses can cause eventual bending, loosening, or breakage of the device(s). Never reuse explanted surgical implants. Advise patients with implants that they should consider the administration of prophylactic antibiotics before receiving any subsequent surgery (such as dental procedures), for patients who have additional circumstances that increase their risk. The implantation of spinal fixation instrumentation must be performed only by experienced spinal surgeons with specific training in the use of such spinal systems. This is a technically demanding procedure presenting a risk of serious injury to the patient. Packaging Components are packaged in metal sterilization cases and/or sealed polyethylene bags. Check packages to ensure all components are intact upon receipt. Check all sets and confirm that all instruments and implants are present. Inspect all set components for functionality to ensure there is no damage prior to use. Immediately return any damaged packages or products to the manufacturer without using them. Handling and Storage The Durango implants and instruments must be handled, stored, and opened in such a way that they are protected from inadvertent damage or contamination. The Durango device does not have a labeled shelf life. The aging process has no observed affect on the mechanical performance of the materials. Verify the proper function of the specialized surgical instruments needed for the Durango system procedure prior to every surgical procedure. Cleaning and Decontamination Durango implants and instruments are supplied non-sterile. Before sterilization, implants and instruments must be cleaned using the following procedures. Caution: Certain cleaning solutions such as those containing formalin, glutaraldehyde, bleach, and/or other alkaline cleaners may damage some devices, particularly instruments. Such cleaning solutions should not be used. Note: Some instruments may require disassembly prior to cleaning. Machine Cleaning Instructions (Recommended) 1. Prepare cleaning detergent a. Prepare an enzymatic detergent, following the manufacturer’s instructions for preparation and use. b. Saline solutions should NOT be used, as saline has a corrosive effect on stainless steel. c. The detergent should have a near-neutral pH to prevent pitting and tarnishing. 2. Prepare devices for soaking a. To prevent injury, separate out sharp and pointed devices and handle with care. b. Disassemble devices with removable parts. c. Open hinged, toothed or threaded joints. d. Remove heavy or large debris using single-use, non-shedding wipes soaked in appropriate cleaning solution. 3. Clean and soak in bath a. Immerse devices in prepared bath. b. Brush all surfaces of the devices with a cleaning brush (do not use steel brushes) while they are submerged in water, ensuring that all visible soil is removed. c. Whenever applicable: i. Use a pipe cleaner and syringe to clean all cannulae, lumens, crevices, grooves and hard to reach areas. ii. Use a syringe to repeatedly apply rinsing solution under pressure to flush all cannulae, lumens, crevices, grooves and hard to reach areas. iii. Repeatedly operate/bend/articulate movable joints while cleaning. iv. Brush the inside of hollow spaces along their entire length. d. Allow devices to soak in detergent bath for the manufacturer’s recommended soaking time. 4. Load devices into washer a. Place devices so they do not collide during operation. b. Place heavy items at the bottom and hollow objects in the washing machine baskets. c. Ensure no part is obstructed by large objects. d. Place articulating instruments in the fully open position and cannulated instruments horizontally. e. Place disassembled instruments in the washing machine baskets. 5. Washing and drying cycles a. 2 minutes: Prewash with cold water; drain. b. 5 minutes: Detergent wash with hot water; drain. c. 2 minutes: Neutralize with neutral pH detergent; drain. d. 2 minutes: Rinse with hot water; drain. e. Dry with hot air at a maximum of 115°C. 6. Inspect a. Inspect the devices with the naked eye under normal lighting conditions to determine if all adherent visible soil (e.g., blood, protein substances and other debris) has been removed from surfaces, lumen, cannulae, crevices, serrations, threading, etc. b. If visible soil remains, repeat the cleaning procedure. Manual Cleaning Instruction 1. Prepare cleaning detergent a. Thoroughly rinse the devices under running water for a minimum of 1 minute. c. Thoroughly flush cannulae, lumens and holes. 5. Ultrasonic bath a. Prepare an ultrasonic bath containing a blood-dissolving detergent, following the manufacturer’s instructions for preparation and use. b. Cover/seal the devices during transport from the rinse to the ultrasonic bath to prevent contamination. c. Place devices in the ultrasonic bath. d. Ensure that the devices are completely submerged and do not overlap. e. Sonicate for 15 minutes. To avoid corrosion, do not exceed 15 minutes. 6. Rinse in sterile water a. Thoroughly rinse the devices with sterile purified water (i.e., RO or DI) for a minimum of 3 minutes. 7. Dry a. Dry the devices with single-use, non-shedding absorbent wipes and/or medical compressed air (e.g., interiors of cannulae). b. Be sure to completely dry the devices immediately after rinse to inhibit corrosion. 8. Inspect a. Inspect the devices with the naked eye under normal lighting conditions to determine if all adherent visible soil (e.g., blood, protein substances and other debris) has been removed from surfaces, lumen, cannulae, crevices, serrations, threading, etc. b. If visible soil remains, repeat the cleaning procedure. Sterilization The Durango implants and instruments are provided non-sterile and must be cleaned and sterilized prior to use. The recommended sterilization process is steam autoclave sterilization, using the parameters listed in the tables below. Use of a wrap (US only: FDA cleared wrap) is recommended to maintain sterility prior to use. The recommended sterilization cycles have been validated to assure a Sterility Assurance Level (SAL) of at least 10-6. Italiano ISTRUZIONI PER L’USO DEL SISTEMA DURANGO Scopo Il sistema Durango è un dispositivo autonomo di fusione intersomatica e sostituzione dei corpi vertebrali per la fusioneintersomatica lombare anteriore (anterior lumbar interbody fusion, ALIF) facente parte del sistema di fusioneintersomatica e sostituzione di corpi vertebrali (vertebral body replacement, VBR) Breckenridge (conosciuto anche come sistema Breckenridge). Descrizione del dispositivo Il dispositivo Durango è un dispositivi di fusione intersomatica composto da uno spaziatore intervertebrale in PEEK-OPTIMA, placca in titanio e viti. Lo spaziatore generalmente presenta forma arrotondata di altezze e impronte diverse con zona centrale forata per alloggiare un innesto osseo autogeno. Le superfici superiore e inferiore presentano una serie di scanalature trasversali che migliorano la stabilità e la fissazione dopo l’inserimento del dispositivo. La placca in titano dispone di fori per viti ossee e un foro centrale per una placca di copertura che impedisce la fuoriuscita delle viti. Il sistema Durango è disponibile in svariate misure e configurazioni per adattarsi alle variazioni anatomiche dei diversi livelli vertebrali e/o dell’anatomia del paziente. Il sistema Durango è fornito non sterile. Materiali Il sistema Durango comprende placche, viti e spaziatori intervertebrali. Le placche e le viti sono realizzate con lega di titanio (Ti-6Al-4V ELI) per implantologia che soddisfa i requisiti delle norme ASTM F136-08 e ISO 5832-3. Il polimero PEEK-OPTIMA® LT1 è una materia termoplastica semicristallina poliaromatica che soddisfa i requisiti delle norme ASTM F-2026 e ISO 10993. Questo materiale con formula chimica di base (-C6H4-O-C6H4-O-C6H4-CO-)n è generalmente noto con il nome polietereterchetone (PEEK). Tutti gli impianti in PEEK-OPTIMA sono realizzati con questo materiale specifico per l’implantologia con marker di posizione in tantalio a norma ASTM F560. Il dispositivo in PEEK-OPTIMA è radiotrasparente. Non utilizzare mai impianti di titanio e cromo-cobalto insieme ad impianti in acciaio inossidabile. Indicazioni per l’uso Quando viene usato come dispositivo di fusione intersomatica lombare, il sistema Durango è destinato all’uso nelle procedure di fusione vertebrale con innesto osseo autogeno in pazienti scheletricamente maturi affetti da discopatia degenerativa su uno o due livelli contigui da L2 a S1. Per discopatia degenerativa si intende dolore dorsale di natura discogenica con degenerazione del disco confermata da anamnesi e indagini radiografiche. I pazienti devono essere stati sottoposti a sei mesi di trattamenti non chirurgici. Possono essere stati sottoposti a precedente intervento chirurgico vertebrale non di fusione dei livelli interessati e possono presentare eventuale spondilolistesi fino al grado I o retrolistesi su tali livelli. Il dispositivo Durango deve essere impiantato con approccio anteriore o posteriore e abbinato a dispositivi di fissazione supplementari (tranne quanto indicato sotto). I dispositivi approvati includono il sistema di fissazione spinale Biomet. Quando vengono usati con viti di fissazione integrate, gli impianti intersomatici autonomi Durango non richiedono dispositivi di fissazione supplementari. Controindicazioni Le controindicazioni possono essere relative o assolute. La scelta di un particolare dispositivo deve essere attentamente soppesata sulla base di una valutazione globale del paziente. Le circostanze elencate di seguito possono ridurre le probabilità di esito positivo dell’intervento. Segue un elenco non esaustivo delle possibili controindicazioni. • Allergia al PEEK-OPTIMA, alle leghe di titanio o cobalto-cromo, o sensibilità ai corpi estranei. In caso di sospetta sensibilità ai materiali, si dovranno eseguire test idonei prima dell’impianto. • Infezione nota o sospetta/incompetenza del sistema immunitario. Patologie infettive acute o croniche di qualsiasi eziologia o localizzazione. • Qualsiasi anomalia presente in grado di influire sul normale processo di rimodellamento osseo comprese, tra l’altro, le seguenti: grave osteoporosi con coinvolgimento della colonna vertebrale, assorbimento osseo, osteopenia, infezioni attive nel sito dell’impianto o specifici disordini metabolici con effetto sull’osteogenesi. • Obesità morbigena. Nei pazienti in sovrappeso o obesi possono generarsi carichi eccessivi sul sistema spinale, con conseguente mancato fissaggio del dispositivo o cedimento del dispositivo stesso. • Qualsiasi deficit neuromuscolare che provochi un carico eccezionalmente gravoso sul dispositivo durante il periodo di guarigione. • Ferite aperte. • Stato di gravidanza. • Qualsiasi altra condizione medica o chirurgica che possa precludere i potenziali benefici della chirurgia spinale, come la presenza di anomalie congenite, l’incremento del tasso di sedimentazione non attribuibile ad altre patologie, l’incremento della conta dei globuli bianchi o un marcato spostamento verso sinistra nella conta differenziale dei globuli bianchi. • Qualsiasi situazione che richieda l’impiego congiunto di componenti di due sistemi diversi. • Qualsiasi situazione che richieda l’impiego congiunto di componenti implantari in acciaio inossidabile e titanio, o acciaio inossidabile e cobalto-cromo. • Febbre o leucocitosi. • Segni di infezione o infiammazione localizzate. • Pregresse infezioni documentate. • Precedente fusione al livello da trattare. • Alcolismo o tabagismo grave. • Senilità, malattia mentale o abuso di sostanze di gravità tale da impedire al paziente di attenersi alle limitazioni o alle precauzioni necessarie nell’uso dell’impianto, con conseguente rottura o altre complicazioni. • Pazienti incapaci o non disposti ad aderire alle istruzioni postoperatorie. • Inadeguata copertura tissutale sul sito operatorio. Possibili complicanze Le possibili complicanze correlate specificatamente al dispositivo possono comprendere: • Precoce o tardiva curvatura, rottura, fallimento, allentamento o spostamento/migrazione dell’impianto • Frattura ossea • Reazione allergica ai materiali dell’impianto • Tra le altre complicazioni generali associate all’intervento di chirurgia spinale vi possono essere: mancata o ritardata unione, pseudoartrosi; dolore; ripetizione dell’intervento chirurgico; sanguinamento; infezione, precoce e successiva; lesioni dei tessuti o dei nervi, tra cui lacerazione durale o altri problemi neurologici; complicazioni correlate all’incisione; formazione di cicatrice; lesioni a vasi sanguigni e compromissione del sistema cardiovascolare; alterazioni dello stato mentale; lesioni agli organi interni e al tessuto connettivo; complicazioni correlate all’uso di innesto osseo, comprese complicazioni della sede del prelievo dell’innesto; problemi respiratori; reazioni all’anestesia e/o morte. Avvertenze I pazienti che sono stati precedentemente sottoposti a chirurgia spinale nell’area da trattare potrebbero presentare risultati clinici diversi rispetto a quelli dei precedenti interventi. Il rischio di espulsione e migrazione del dispositivo è maggiore in assenza di viti di fissazione integrate come indicato. Precauzioni • Gli impianti del sistema Durango sono esclusivamente monouso. Non riutilizzare mai un impianto, anche quando non presenta segni o danni. Il riutilizzo di un impianto può comportare un funzionamento meccanico inadeguato, anomalie di funzionamento o rottura del dispositivo. I dispositivi impiantati e successivamente espiantati devono essere gettati. Usare solo impianti nuovi per ogni caso. • L’impianto di questo sistema deve essere effettuato esclusivamente da chirurghi con esperienza nelle tecniche chirurgiche vertebrali e con addestramento specifico nell’uso degli impianti vertebrali. Si tratta di una procedura molto impegnativa a livello tecnico, che presenta il rischio di gravi lesioni per il paziente. • Il sistema Durango è riservato solamente ai chirurghi specializzati nella chirurgia vertebrale e in possesso di una conoscenza approfondita dell’anatomia, della morfologia regionale e dei principi biomeccanici della colonna vertebrale. Si consiglia ai chirurghi di acquisire inoltre una completa familiarità con le tecniche chirurgiche relative all’uso del dispositivo. • In base ai risultati delle prove di fatica, il medico/chirurgo deve considerare i livelli di impianto, il peso del paziente e i suoi livelli di attività, oltre ad altre condizioni del paziente che possano influire sulle prestazioni del sistema. • Prima della procedura, spiegare al paziente i rischi correlati con gli interventi di neurochirurgia, chirurgia generale, chirurgia ortopedica e con la somministrazione di anestesia generale. Si consiglia di esporre inoltre al paziente i vantaggi e gli svantaggi dell’uso degli impianti e dei metodi di trattamento alternativi. • Fase preoperatoria - Il chirurgo deve avere completa dimestichezza con tutti gli aspetti della tecnica chirurgica ed essere al corrente delle indicazioni e controindicazioni relative a questo tipo di impianto. Prima dell’intervento, il chirurgo dovrà essersi documentato in merito alla particolare tecnica di inserimento dell’impianto, recuperando le informazioni dal produttore. Come parte dell’esame preoperatorio, il chirurgo deve verificare che non vi siano fattori biologici, biomeccanici o di altro tipo che influiscano sul corretto andamento dell’intervento e della fase postoperatoria. Al momento dell’intervento si dovrà disporre di un’adeguata gamma di impianti di diverse dimensioni. • Fase intraoperatoria - La scelta corretta del tipo e della dimensione dell’impianto in funzione del paziente, nonché il posizionamento dell’impianto stesso sono fattori di estrema importanza. • Fase postoperatoria - I pazienti devono essere informati in merito alle precauzioni da osservare nella vita quotidiana allo scopo di garantire la massima vita utile dell’impianto. Si consiglia di effettuare un regolare follow-up post-operatorio allo scopo di individuare precocemente eventuali segni di fallimento degli impianti e pianificare le misure da adottare. Il deterioramento del dispositivo dopo il consolidamento osseo non deve essere considerato una disfunzione o un deterioramento delle caratteristiche dell’impianto. L’impianto può essere rimosso una volta completata la guarigione dell’osso. • La scelta e il posizionamento corretti degli impianti sono di fondamentale importanza. Scegliere l’impianto in base al difetto osseo da trattare, considerando inoltre il peso, l’altezza, l’occupazione o il grado di attività fisica del paziente. • La manipolazione corretta dell’impianto prima e durante l’intervento è essenziale. • L’uso della placca di copertura che impedisce la fuoriuscita delle viti è obbligatorio. • Se una placca di copertura viene smontata da una placca, deve essere gettata e non riutilizzata. • Se una placca viene smontata da un impianto intersomatico in PEEK-OPTIMA, deve essere gettata e non riutilizzata. • Il dispositivo Durango non deve essere usato con componenti o strumenti vertebrali di altri produttori. • Prima di usare i dispositivi, verificare che non siano danneggiati, usurati e che siano funzionanti. Non usare né sterilizzare strumenti danneggiati o difettosi. Contattare il rappresentante o il distributore Biomet locale per eventuali riparazioni o sostituzioni. • L’uso di uno strumento per scopi diversi da quelli per cui è stato progettato potrebbe danneggiare o rompere la strumentazione. • Non applicare eccessiva forza o pressione. L’uso improprio può danneggiare gli strumenti o gli impianti. • Prima di iniziare l’intervento, eseguire un’attenta analisi preoperatoria per accertarsi che siano disponibili tutti i componenti necessari per l’impianto e che il set di strumenti sia completo e pronto per l’uso. • La sicurezza e la compatibilità del sistema Durango nell’ambiente di risonanza magnetica (RM) non sono state verificate. Non sono stati condotti test relativi al riscaldamento o alla migrazione del sistema Durango in un ambiente di risonanza magnetica. • L’utilizzo contemporaneo di metalli diversi può accelerare o innescare il processo di corrosione. I componenti in titanio NON devono essere usati per l’allestimento di strutture in cui siano presenti altri materiali implantari. Il titanio e il cobalto-cromo possono essere usati insieme all’interno della stessa struttura. Indicazioni per l’uso Le informazioni contenute in questo foglio illustrativo sono necessarie all’uso del dispositivo, ma non sono sufficienti. Queste informazioni non intendono sostituire il giudizio professionale, le capacità e l’esperienza del chirurgo per quanto riguarda: attenta selezione dei pazienti, pianificazione preoperatoria, scelta del dispositivo, conoscenza dell’anatomia e della biomeccanica vertebrale, conoscenza dei materiali e delle caratteristiche meccaniche degli impianti usati, addestramento e abilità nella chirurgia vertebrale e nell’uso degli strumenti necessari per l’impianto, assicurarsi la collaborazione del paziente nell’osservanza del programma specifico di gestione postoperatoria e conduzione degli esami di follow-up postoperatorio. Il dispositivo Durango viene impiantato chirurgicamente con approccio anteriore. Verificare e ispezionare tutta la strumentazione e gli impianti prima della sterilizzazione. Sostituire o aggiungere i componenti eventualmente necessari per l’intervento pianificato. Il chirurgo responsabile dell’équipe deve aver maturato una solida esperienza nelle necessarie tecniche di fusione vertebrale. Preparare, posizionare e coprire con teli il paziente secondo le prassi usuali. Procedere con l’approccio chirurgico anteriore standard. Completare la discectomia e/o la corpectomia sul livello appropriato e posizionare l’innesto osseo. Registrare i numeri di lotto degli impianti inseriti nel paziente durante l’intervento chirurgico nella scheda del paziente. Linee guida chirurgiche Procedura preoperatoria Il chirurgo deve avere completa dimestichezza con tutti gli aspetti della tecnica chirurgica ed essere al corrente delle indicazioni e controindicazioni relative a questo tipo di impianto. Prima dell’intervento, il chirurgo dovrà essersi documentato in merito alla particolare tecnica di inserimento dell’impianto, recuperando le informazioni dal produttore. Come parte dell’esame preoperatorio, il chirurgo deve verificare che non vi siano fattori biologici, biomeccanici o di altro tipo che influiscano sul corretto andamento dell’intervento e della fase postoperatoria. È necessario selezionare solo i pazienti che soddisfano i criteri descritti nella sezione Indicazioni per l’uso. Evitare pazienti con le condizioni e/o le predisposizioni elencate nella sezione Controindicazioni. Manipolare e conservare con cura i componenti del sistema Durango. Durante l’immagazzinaggio, gli impianti e gli strumenti devono essere protetti in particolar modo da eventuali agenti corrosivi. Il tipo di struttura da assemblare per il caso specifico deve essere determinato prima dell’intervento. Il chirurgo responsabile dell’équipe deve aver maturato una solida esperienza nelle necessarie tecniche di fusione vertebrale. In base ai risultati dei test di fatica, il chirurgo deve considerare i livelli di impianto, il peso del paziente e i suoi livelli di attività, altre condizioni mediche del paziente, ecc. che possono influire sulle prestazioni dei componenti del sistema Durango. Al momento dell’intervento chirurgico, tenere a disposizione una scorta adeguata di impianti di dimensioni diverse, inclusi impianti di riserva delle dimensioni che si prevede di usare e impianti di misure più grandi e più piccole rispetto a quelle che si pensa siano adatte al paziente. Verificare e ispezionare tutta la strumentazione e gli impianti prima della sterilizzazione. Sostituire o aggiungere i componenti eventualmente necessari per l’intervento pianificato. Poiché il sistema comprende parti meccaniche, il chirurgo dovrà familiarizzarsi con i vari componenti prima di usare il dispositivo e assemblare personalmente ciascun impianto per verificare che siano presenti tutte le parti e gli strumenti necessari prima di dare il via alla procedura. Assicurarsi che la data dell’intervento non sia successiva alla data di scadenza della calibrazione dell’impugnatura dinamometrica. Non usare strumenti danneggiati o difettosi. Contattare il produttore per istruzioni relative alla riparazione o alla sostituzione. Prima dell’uso, pulire e sterilizzare tutti i componenti. Tenere a disposizione ulteriori componenti sterili, in caso di necessità impreviste. Preparare, posizionare e coprire con teli il paziente secondo le prassi usuali. Procedure operatorie L’intervento chirurgico di impianto del dispositivo Durango è simile alle procedure di fusione ossea intersomatica eseguite secondo le tecniche convenzionali. • La preparazione del paziente, l’esposizione chirurgica e la procedura di fusione (preparazione dello spazio discale, innesto osseo, ecc.) vanno eseguite secondo le tecniche standard preferite dal chirurgo. • Il chirurgo espone il livello o i livelli interessati all’uso della strumentazione e prepara il sito usando gli strumenti chirurgici manuali necessari. • Procedere all’eventuale escissione di tessuto osseo e molle secondo la necessità. • Dopo la preparazione dello spazio del disco intervertebrale, usare il distrattore per disco incluso nella cassetta per la preparazione dei dischi Biomet Spine per conseguire il livello di distrazione desiderato (i distrattori per disco sono disponibili nelle misure da 8 mm a 20 mm, in incrementi di 2 mm). • Dopo l’asportazione del tessuto discale e la preparazione dei piatti vertebrali del livello appropriato, usare un impianto di dimensionamento di prova per misurare lo spazio discale e determinare la corretta misura dell’impianto. • Iniziare la scelta delle dimensioni dell’impianto inserendo il componente di prova più piccolo per altezza e lunghezza. Aumentare progressivamente l’altezza dei componenti di prova fino a raggiungere l’altezza di distrazione appropriata. Confermare mediante radiografia che l’altezza e l’impronta del componente di prova sono quelle adeguate alle condizioni anatomiche. • Scegliere il corpo in PEEK-OPTIMA delle dimensioni giuste. Selezionare la placca corrispondente in base alle preferenze del chirurgo. I due componenti vanno assemblati usando l’apposita vite di bloccaggio e l’impugnatura dinamometrica calibrata. Riempire con innesto osseo autogeno l’area centrale forata del corpo in PEEK-OPTIMA. • Fissare saldamente l’impianto alla punta dell’inseritore con una barra filettata e introdurlo nello spazio discale, collocandolo nella posizione finale con dei colpetti, secondo la necessità. A questo punto si può completare l’inserimento di ulteriore materiale di innesto. Confermare radiograficamente il posizionamento definitivo basandosi sui marker radiografici. • Fissare l’impianto all’anatomia con quattro viti ossee. Il tipo di viti usate (fisse o variabili e con diverse configurazioni della punta) dipende dalla preferenza del chirurgo. Preparare e posizionare le viti in sequenza, in modo che ogni vite venga inserita con l’avvitatore dopo averne preparato il sito di introduzione. La preparazione viene eseguita con guide, punteruoli e trapani. • Dopo il posizionamento corretto dell’impianto e delle viti ossee, fissare alla placca la corrispondente placca di copertura con l’apposito avvitatore e l’impugnatura dinamometrica calibrata. • Ispezionare quindi il costrutto per confermare la corretta collocazione di tutti gli impianti e procedere alla sutura del sito chirurgico secondo tecniche standard. Cure postoperatorie Assicurarsi che venga seguito un opportuno regime comprendente analgesici, antibiotici, immobilizzazione e cure domiciliari. I pazienti devono essere informati in merito alle precauzioni da osservare nell’attività quotidiana allo scopo di garantire la massima vita utile dell’impianto. Si consiglia di effettuare un regolare follow-up postoperatorio allo scopo di individuare precocemente eventuali segni di fallimento degli impianti e pianificare le misure più opportune da adottare. Il deterioramento del dispositivo dopo il consolidamento osseo non può essere considerato una disfunzione o un decadimento delle caratteristiche dell’impianto. Dovrà inoltre essere pianificato e implementato un adeguato programma riabilitativo. Le indicazioni e le avvertenze postoperatorie fornite dal medico al paziente, nonché la compliance di quest’ultimo, sono di importanza fondamentale. Istruire in modo adeguato il paziente affinché si attenga al trattamento postoperatorio indicato. La capacità e la volontà del paziente di seguire le istruzioni sono tra i fattori fondamentali per una guarigione rapida ed efficace. Le istruzioni per il paziente comprendono quanto segue. • Avvertire il paziente dell’importanza di evitare cadute o movimenti bruschi a carico della colonna vertebrale. • Fornire al paziente istruzioni dettagliate in merito all’uso e alle limitazioni del dispositivo. • Laddove fosse consigliabile o necessario un carico parziale per consentire una salda unione dell’osso, il paziente dovrà essere informato che la curvatura, l’allentamento o la rottura dei componenti del sistema rappresentano complicanze possibili in conseguenza di un carico eccessivo o precoce o di un’eccessiva attività muscolare. Die Patienten müssen auf die Vorsichtsmaßnahmen, die sie im Alltag befolgen müssen, hingewiesen werden, um eine maximale Lebensdauer des Implantats zu garantieren. Es wird empfohlen, sich nach der Operation regelmäßigen Nachuntersuchungen zu unterziehen, um frühe Anzeichen eines Implantatversagens zu erkennen und geeignete Maßnahmen in Betracht zu ziehen. Eine Beeinträchtigung des Systems nach Knochenkonsolidierung kann nicht als Fehlfunktion oder Verschlechterung der Implantateigenschaften gewertet werden. Es muss ein geeignetes Rehabilitationsprogramm erstellt und umgesetzt werden. Die dem Patienten vom Arzt erläuterten postoperativen Anweisungen und Warnhinweise sowie deren Einhaltung durch den Patienten sind von größter Bedeutung. Dem Patienten die geeignete postoperative Versorgung auf angemessene Art und Weise erläutern. Fähigkeit und Bereitschaft des Patienten zur Befolgung der Anweisungen sind zwei der wichtigsten Aspekte für eine erfolgreiche Heilung. Die empfohlenen Anweisungen umfassen u.a. Folgende: • Der Patient ist vor den Gefahren von Stürzen oder ruckartigen Stößen im Bereich der Wirbelsäule zu warnen. • Der Patient muss ausführlich über die Verwendung und die Einschränkungen des Implantats informiert werden. • Falls eine Teilbelastung vor Festigung der Knochenfusion empfohlen oder erforderlich ist, muss der Patient auf die Komplikationen durch Verbiegen, Ablösung oder Bruch der Komponenten aufgrund von übermäßiger oder frühzeitiger Belastung bzw. übermäßigen Muskelaktivitäten hingewiesen werden. Ein erhöhtes Verbiege-, Ablösungs- und Bruchrisiko einer internen Fixation während der postoperativen Rehabilitation besteht vor allem bei aktiven Patienten, bei geschwächten, dementen oder anderweitig eingeschränkten Patienten, die keine sonstigen Hilfsmittel zur Gewichtsabstützung verwenden. • Um die Aussichten auf ein erfolgreiches Ergebnis der Operation zu maximieren, sollte der Patient sich keinen mechanischen Vibrationen aussetzen, die ein Ablösen der Systemkonstruktion bewirken könnten. Der Patient muss vor den möglichen Konsequenzen gewarnt und angewiesen werden, körperliche Aktivitäten einzuschränken. Dies gilt insbesondere für das Heben von Gegenständen, das Verdrehen des Körpers und jegliche sportliche Betätigung. • Der Patient sollte angewiesen werden, während des Heilungsprozesses für das Knochentransplantat nicht zu rauchen, keine Nikotinprodukte zu verwenden, keinen Alkohol zu trinken und keine nicht-steroidalen oder entzündungshemmenden Medikamente (z.B. Aspirin) einzunehmen. • Der Patient muss darauf hingewiesen werden, dass die Bewegungsmöglichkeit im Bereich der Wirbelsäulenfusion unterbunden ist. Außerdem muss der Patient über Verhaltensweisen unterrichtet werden, mit denen diese permanente Einschränkung der Bewegungsfreiheit kompensiert werden kann. Falls keine Fusion erzielt wird oder die Komponenten sich lösen, verbiegen und/oder brechen, muss das Gerät bzw. müssen die Geräte sofort korrigiert und/oder entfernt werden, bevor ernsthafte Verletzungen entstehen. Wird bei einer verzögerten Fusion in den Knochen keine Immobilisierung bewirkt, wird das Implantat übermäßigen und wiederholten Belastungen ausgesetzt. Infolge von Ermüdung können diese Belastungen zu einem Verbiegen, Ablösen oder zum Bruch des Systems bzw. der Systeme führen. Explantierte chirurgische Implantate dürfen nicht wiederverwendet werden! Die Patienten mit Implantaten unterrichten, dass vor jedem weiteren chirurgischen Eingriff (wie bei Operationen im Dentalbereich) die prophylaktische Verabreichung von Antibiotika in Erwägung gezogen wird, insbesondere für Patienten, deren Risiko aufgrund sonstiger Umstände erhöht ist. Die Implantation eines Instrumentariums zur Wirbelsäulenfixation darf nur von erfahrenen Wirbelsäulenchirurgen durchgeführt werden, die über eine spezielle Ausbildung im Umgang mit derartigen Wirbelsäulensystemen verfügen. Es handelt sich um ein technisch anspruchsvolles Verfahren, das ein Risiko von schwerwiegenden Verletzungen des Patienten birgt. Verpackung Die Komponenten sind in Sterilisationsdosen aus Metall und/oder versiegelten Polyäthylen-Beuteln verpackt. Packungen nach Erhalt überprüfen, um sicherzustellen, dass sämtliche Komponenten fehlerfrei sind. Alle Sets überprüfen und bestätigen, dass sämtliche Instrumente und Implantate vorliegen. Alle Komponenten des Sets auf Funktionalität überprüfen, um vor der Verwendung sicherzustellen, dass keinerlei Schaden vorliegt. Beschädigte Packungen oder Produkte sofort an den Hersteller zurücksenden, ohne sie zu gebrauchen. Handhabung und Lagerung Die Implantate und Instrumente des Durango-Systems müssen vorsichtig gehandhabt, gelagert und geöffnet werden, sodass sie nicht unbeabsichtigt beschädigt oder kontaminiert werden können. Das Durango-Implantat verfügt über keine bezeichnete Haltbarkeit. Der Alterungsprozess hat keine festgestellten Auswirkungen auf die mechanische Leistung der Materialien. Die ordnungsgemäße Funktion der für das Durango-System benötigten chirurgischen Spezialinstrumente ist vor jedem Operationsverfahren zu überprüfen. Reinigung und Dekontamination Implantate und Instrumente des Durango-Systems werden nicht-steril geliefert. Vor der Sterilisation müssen die Implantate und Instrumente unter Einsatz der folgenden Verfahren gereinigt werden. Vorsicht: Bestimmte Reinigungslösungen, wie beispielsweise Reinigungsmittel, die Formalin, Glutaraldehyd, Bleichmittel und/oder andere alkalische Reinigungsstoffe enthalten, können manche Komponenten und besonders die Instrumente beschädigen. Reinigungslösungen dieser Art sollten nicht verwendet werden. Hinweis: Einige Instrumente müssen vor dem Reinigen ggf. auseinandergenommen werden. Reinigungsanweisungen – Waschmaschine (empfohlen) 1. Reinigungslösung zubereiten a. Eine Enzymreinigungslösung gemäß Herstelleranweisung bezüglich Zubereitung und Verwendung vorbereiten. b. KEINE Salzlösung verwenden, da Salz bei rostfreiem Edelstahl zu Korrosion führen kann. c. Der pH-Wert der Reinigungslösung sollte nahezu neutral sein, sodass Lochfraß und Anlaufen vermieden werden. 2. Instrumente für das Tauchbad vorbereiten a. Um Verletzungen zu vermeiden, scharfe und spitze Instrumente aussortieren und mit besonderer Sorgfalt handhaben. b. Instrumente mit abnehmbaren Teilen auseinandernehmen. c. Scharnier-, Zahn- oder Gewindegelenke öffnen. d. Schwere oder großflächige Verunreinigungen mit in der Reinigungslösung getränkten, fusselfreien Einweg-Reinigungstüchern entfernen. 3. Im Tauchbad reinigen a. Die Instrumente in das vorbereitete Tauchbad einlegen. b. Während die Instrumente im Tauchbad liegen, alle Oberflächen mit einer Bürste reinigen (keine Stahlbürste verwenden), dabei darauf achten, dass alle sichtbaren Verschmutzungen entfernt werden. c. Sofern zutreffend: i. Alle Kanülen, Lumen, Spalten, Rillen und schwer erreichbaren Bereiche mit einem Pfeifenreiniger und einer Spritze reinigen. ii. Alle Kanülen, Lumina, Spalten, Rillen und schwer erreichbaren Bereiche wiederholt mit Druck spülen, für die Spüllösung eine Spritze verwenden. iii. Bewegliche Gelenke während der Reinigung wiederholt betätigen bzw. drehen bzw. auf- und zuklappen. iv. Hohlrauminnenflächen in ihrer gesamten Länge ausbürsten. d. Die Instrumente für die vom Hersteller empfohlene Einweichzeit im Tauchbad liegen lassen. 4. Instrumente in die Waschmaschine legen a. Die Instrumente so in die Maschine legen, dass sie während des Waschvorgangs nicht aufeinander prallen. b. Schwere Gegenstände unten in die Maschine legen, hohle Gegenstände in die Waschmaschinenkörbe legen. c. Sicherstellen, dass die einzelnen Bestandteile der Instrumente nicht durch große Gegenstände blockiert werden. d. Gelenkinstrumente vollständig geöffnet in die Maschine legen, kanülierte Instrumente horizontal in die Maschine legen. e. Auseinandergenommene Instrumente in die Waschmaschinenkörbe legen. 5. Wasch- und Trockenzyklen a. 2 Minuten: Mit kaltem Wasser vorwaschen; abtropfen lassen. b. 5 Minuten: Mit heißem Wasser und Reinigungslösung waschen; abtropfen lassen. c. 2 Minuten: Mit einer Reinigungslösung mit neutralem pH-Wert neutralisieren; abtropfen lassen. d. 2 Minuten: Mit heißem Wasser spülen; abtropfen lassen. e. Mit Heißluft (max. 115 ˚C) trocknen. 6. Sichtprüfen a. Die Instrumente bei normaler Beleuchtung mit bloßem Auge sichtprüfen, um festzustellen, ob evtl. haftengebliebene sichtbare Verschmutzungen (z.B. Blut, eiweißhaltige Substanzen oder andere Verunreinigungen) von allen Oberflächen, Lumina, Kanülen, Spalten, gezackten Rändern, Gewinden usw. entfernt worden sind. b. Wenn sichtbare Verschmutzungen zurückgeblieben sind, die Reinigung wiederholen. Reinigungsanweisungen - Handwäsche 1. Reinigungslösung zubereiten a. Bewegliche Gelenke während der Reinigung wiederholt betätigen bzw. drehen bzw. auf- und zuklappen. iv. Hohlrauminnenflächen in ihrer gesamten Länge ausbürsten. d. Die Instrumente für die vom Hersteller empfohlene Einweichzeit im Tauchbad liegen lassen. 4. Spülen a. Die Instrumente aus dem Tauchbad nehmen. b. Die Instrumente mindestens eine Minute lang unter fließendem Wasser abspülen. c. Kanülen, Lumen und Löcher gründlich ausspülen. 5. Ultraschallbad a. Ein Ultraschallbad mit einer blutlösenden Reinigungslösung vorbereiten, dabei Herstelleranweisung bezüglich Zubereitung und Verwendung beachten. b. Die Instrumente während des Transports vom Spülvorgang zum Ultraschallbad abdecken/versiegeln, um eine Kontaminierung zu vermeiden. c. Die Instrumente in das Ultraschallbad einlegen. d. Dafür sorgen, dass die Instrumente vollständig in das Ultraschallbad eingetaucht sind und nicht übereinander liegen. e. 15 Minuten lang im Ultraschallbad behandeln. Um Korrosion zu vermeiden, darf die angegebene Dauer von 15 Minuten nicht überschritten werden. 6. Mit sterilem Wasser spülen a. Die Instrumente mindestens 3 Minuten lang mit gereinigtem, sterilem Wasser (d.h. RO- oder DIWasser) abspülen. 7. Trocknen a. Die Instrumente mit fusselfreien Einwegtüchern und/oder medizinischer Druckluft (z.B. Kanüleninnenräume) trocknen. b. Zur Vermeidung von Korrosion darauf achten, dass die Instrumente sofort nach dem Spülen vollständig getrocknet werden. 8. Sichtprüfen a. Die Instrumente bei normaler Beleuchtung mit bloßem Auge sichtprüfen, um festzustellen, ob evtl. haftengebliebene sichtbare Verschmutzungen (z.B. Blut, eiweißhaltige Substanzen oder andere Verunreinigungen) von allen Oberflächen, Lumina, Kanülen, Spalten, gezackten Rändern, Gewinden usw. entfernt worden sind. b. Wenn sichtbare Verschmutzungen zurückgeblieben sind, die Reinigung wiederholen. Sterilisierung Die Implantate und Instrumente des Durango-Systems werden nicht-steril geliefert und müssen vor dem Gebrauch gereinigt und sterilisiert werden. Die Sterilisation sollte in einem Dampfautoklaven mit den in den nachstehenden Tabellen aufgeführten Parametern durchgeführt werden. Zur Erhaltung der Sterilität vor dem Gebrauch empfiehlt sich die Verwendung einer Sterilisationsverpackung (Nur USA: Sterilisationsverpackung mit FDAZulassung). Die empfohlenen Sterilisationszyklen wurden geprüft und gewährleisten einen SAL-Wert (Sterility Assurance Level) von mindestens 10-6. Español INSTRUCCIONES DE USO DEL SISTEMA DURANGO Propósito El sistema Durango es un dispositivo autónomo de fusión decuerpo intervertebral/reemplazo de cuerpo vertebral para la fusión intersomática lumbar anterior (anterior lumbar interbody fusion, ALIF), que forma parte del sistema de fusión de cuerpo intervertebral/reemplazo de cuerpo vertebral (vertebral body replacement, VBR) Breckenridge (conocido también como sistema Breckenridge). Descripción del dispositivo El dispositivo Durango es un dispositivo de fusión de cuerpo intervertebral que consiste en un espaciador intervertebral de PEEK-OPTIMA, una placa de titanio y tornillos. El espaciador tiene una forma generalmente redondeada con distintas alturas y bases, y tiene un área central hueca para la inserción de autoinjerto óseo. Las superficies superiores e inferiores tienen una serie de ranuras transversales creadas para mejorar la estabilidad y la fijación una vez introducido el dispositivo. La placa de titanio tiene agujeros para tornillos óseos y un agujero central para la colocación de una placa cobertora que impida el retroceso de los tornillos. El sistema Durango está disponible en una serie de tamaños y configuraciones para adaptarse a las variaciones anatómicas de los diferentes niveles vertebrales y/o a la anatomía del paciente. El sistema Durango se suministra sin esterilizar. Materiales El sistema Durango consta de placas, tornillos y espaciadores intervertebrales. PEEK-OPTIMA® LT1 es un polímero termoplástico, semicristalino y poliaromático que satisface los requisitos de las normas ASTM F-2026 e ISO 10993. Indicaciones de uso Cuando se utiliza como dispositivo de fusión de cuerpo intervertebral lumbar, el sistema Durango está indicado para usarse en procedimientos de fusión de la columna vertebral junto con autoinjerto óseo, en pacientes esqueléticamente maduros con discopatía degenerativa (DD) en uno o dos niveles espinales contiguos del segmento L2-S1. La DD se define como dolor de espalda discogénico con degeneración de disco confirmada por los antecedentes y los estudios radiográficos. Estos pacientes deberán haber tenido seis meses de tratamiento no quirúrgico. Estos pacientes con DD pueden haber tenido una cirugía anterior sin fusión de la columna vertebral en los niveles espinales afectados, y podrían tener espondilolistesis o retrolistesis hasta de grado 1 en los niveles afectados. El sistema Durango ha de implantarse mediante un abordaje anterior o posterior y combinarse con fijación complementaria (excepto según se indica a continuación). Contraindicaciones Las contraindicaciones pueden ser relativas o absolutas. La elección de un dispositivo particular debe sopesarse cuidadosamente con la evaluación general del paciente. Las circunstancias enumeradas a continuación pueden reducir las posibilidades de obtener un resultado satisfactorio. Las contraindicaciones incluyen, entre otras: • Una alergia a las aleaciones de PEEK-OPTIMA, titanio o cobalto-cromo, o sensibilidad a cuerpos extraños. Cuando exista una sospecha de sensibilidad a los materiales, deben realizarse las pruebas adecuadas antes de la implantación. • Incompetencia del sistema inmunitario/infección conocida o sospechada. Enfermedades infecciosas agudas o crónicas de cualquier etiología o ubicación. • Cualquier anomalía presente que afecte al proceso normal de remodelación ósea incluidas, entre otras, osteoporosis intensa que afecte a la columna, absorción ósea, osteopenia, infección activa en el sitio o determinados trastornos metabólicos que afectan a la osteogénesis. • Obesidad mórbida. Un paciente obeso o con sobrepeso puede generar cargas en el sistema espinal, que pueden provocar el fallo de la fijación del dispositivo o del propio dispositivo. • Cualquier deficiencia neuromuscular que someta a una carga inusualmente pesada al dispositivo durante el periodo de consolidación. • Heridas abiertas. • Embarazo. • Cualquier otra afección médica o quirúrgica que impediría el beneficio potencial de la cirugía de la columna vertebral, como la presencia de anomalías congénitas, elevación de la tasa de sedimentación no explicada por otras enfermedades, elevación del recuento de glóbulos blancos, o un marcado giro a la izquierda en el recuento diferencial de glóbulos blancos. • Cualquier caso que requiera mezclar componentes de dos sistemas diferentes. • Cualquier caso que requiera la mezcla de implantes de acero inoxidable con otros de titanio o de acero inoxidable con otros de cobalto-cromo. • Fiebre o leucocitosis. • Indicios de infección o inflamación local. • Antecedentes previos de infección. • Fusión anterior al nivel que se va a tratar. • Alcoholismo o tabaquismo intenso. • Senilidad, enfermedad mental o abuso de sustancias, de tal gravedad que el paciente puede hacer caso omiso de determinadas limitaciones y precauciones necesarias en el uso del implante, dando lugar al fallo del dispositivo u otras complicaciones. • Cualquier paciente que no tenga intención de seguir las instrucciones postoperatorias. • Cobertura tisular inadecuada sobre el sitio quirúrgico. Posibles complicaciones Entre las posibles complicaciones específicas del dispositivo se incluyen: • Doblamiento, rotura, fallo, aflojamiento o desplazamiento/migración del implante tempranos o tardíos • Fractura ósea • Reacción alérgica al material del implante • Otras complicaciones generales asociadas con cualquier procedimiento quirúrgico de la columna vertebral pueden incluir: falta de unión o unión retardada, pseudoartrosis, dolor, una segunda intervención, hemorragia, infección temprana y tardía, daño tisular o nervioso, incluidos los desgarros de la duramadre u otros problemas neurológicos, complicaciones en la incisión, formación de tejido cicatrizal, daño a los vasos sanguíneos y deterioro del sistema cardiovascular, cambios en el estado mental, daño a los órganos internos y al tejido conjuntivo, complicaciones debidas al uso de injertos óseos, incluidas las del sitio donante del injerto, problemas respiratorios, reacciones a la anestesia y/o muerte. Advertencias Los pacientes con una cirugía espinal previaen los niveles que van a tratarse podrían tener resultados clínicos diferentes a los de la intervención anterior. El riesgo de expulsión y migración del dispositivo es mayor sin el uso de tornillos de fijación integrados tal como se ha indicado. Precauciones • Los implantes Durango son para un solo uso. Nunca reutilice un implante aunque no presente marcas ni daños aparentes. La reutilización de los componentes de implante puede generar un rendimiento mecánico reducido, mal funcionamiento o fallo del dispositivo. Cualquier implante que se haya implantado y después extraído se debe desechar. Sólo use implantes nuevos para cada caso. • La implantación de este sistema deberán realizarla únicamente cirujanos especialistas en columna vertebral con experiencia, que hayan recibido formación concreta en el uso de implantes vertebrales. El procedimiento quirúrgico es exigente desde el punto de vista técnico y presenta un riesgo de lesión grave para el paciente. • El sistema Durango está diseñado para que lo utilicen sólo cirujanos especializados en cirugía de la columna vertebral, que cuenten con un conocimiento profundo de la anatomía vertebral, la morfología vertebral regional y los principios biomecánicos de la columna vertebral. Se aconseja que el cirujano esté además totalmente familiarizado con las técnicas quirúrgicas relativas al uso del dispositivo. • En base a los resultados de las pruebas de fatiga, el médico/cirujano debe considerar los niveles de implantación, el peso del paciente, el nivel de actividad del paciente, otras posibles afecciones, etc. que pueden afectar al rendimiento del sistema. • Antes de la intervención quirúrgica deberán explicársele al paciente los riesgos asociados con la neurocirugía, la cirugía general, la cirugía ortopédica y el uso de anestesia general. Se recomienda explicar al paciente las ventajas y desventajas del uso de implantes, así como los métodos de tratamiento alternativos. • Antes de la cirugía: El cirujano debe estar totalmente familiarizado con todos los aspectos de la técnica quirúrgica, y conocer las indicaciones y las contraindicaciones de este tipo de implante. El cirujano debe haberse familiarizado antes de la cirugía con la técnica de introducción del producto específica, que se puede obtener del fabricante. Como parte del examen preoperatorio, el cirujano debe comprobar que no exista ningún factor biomecánico, biológico o de otro tipo que pueda afectar a la correcta realización de la cirugía y al periodo postoperatorio. Se debe contar con un amplio abanico de tamaños de implante en el momento de la cirugía. • Durante la cirugía: La correcta selección del tamaño y el tipo de implante adecuados para el paciente y la colocación del implante son de extrema importancia. • Después de la cirugía: Se debe informar a los pacientes de las precauciones que deben adoptar en su vida diaria para garantizar una vida útil máxima del implante. Se recomienda realizar un seguimiento postoperatorio regular, para detectar signos tempranos de fallo de los implantes y considerar la medida a adoptar. El deterioro del dispositivo después de la consolidación ósea no puede considerarse que constituya una disfunción o deterioro de las características de los implantes. El implante puede extraerse después de la consolidación ósea. • La selección y colocación correctas de los implantes es sumamente importante. La selección del implante debe basarse en el defecto óseo que se va a tratar, así como en el peso, la altura, la ocupación o el grado de actividad física del paciente. • La manipulación adecuada del implante antes y durante la intervención quirúrgica es crucial. • El uso de la placa cobertora para impedir el retroceso de los tornillos es obligatorio. • Si una placa cobertora se desmonta de una placa, debe desecharse y no reutilizarse. • Si una placa se desmonta de un implante intercorporal de PEEK-OPTIMA, debe desecharse y no reutilizarse. • El dispositivo Durango no debe utilizarse con componentes vertebrales o instrumentos de otros fabricantes. • Antes de su uso, inspeccione todo el instrumental para ver si presenta señales de daño, desgaste o falta de funcionamiento. No se deben utilizar ni procesar instrumentos dañados o defectuosos. Póngase en contacto con su representante o distribuidor local de Biomet para la reparación o sustitución. • El uso de instrumentos para tareas distintas de aquellas para las que están indicados podría dar lugar al deterioro o rotura de los instrumentos. • No aplique una fuerza o tensión excesivas. El uso incorrecto puede dañar los instrumentos o los implantes. • Realice un examen preoperatorio detenido para asegurarse de que todos los componentes de implante necesarios estén disponibles y de que el juego de instrumentos esté completo y funcione bien antes de iniciar la cirugía. • No se ha comprobado la seguridad y compatibilidad del sistema Durango en entornos de resonancia magnética (RM). No se ha comprobado el calentamiento o migración del sistema Durango en entornos de RM. • La mezcla de metales distintos puede acelerar o iniciar el proceso de corrosión. Los componentes de titanio NO deben utilizarse juntos para crear un constructo en el que se utilicen otros materiales de implante. Los componentes de implante de titanio y cobalto-cromo pueden utilizarse conjuntamente dentro del mismo sistema. Instrucciones de uso La información que contiene este prospecto es necesaria pero no suficiente para usar este dispositivo. Esta información no está pensada para sustituir el juicio profesional, la destreza y la experiencia del cirujano en: la cuidadosa selección de los pacientes, la planificación preoperatoria, la selección de los dispositivos, el conocimiento de la anatomía y de la biomecánica de la columna vertebral, la comprensión del material y de las características mecánicas de los implantes usados, la formación y la destreza tanto en cirugía de la columna vertebral como en el uso de instrumentos asociados para la implantación, garantizar la cooperación del paciente en el seguimiento de un programa de manejo postoperatorio adecuadamente definido, y realizar exámenes de seguimiento postoperatorios. El implante Durango se implanta quirúrgicamente mediante un abordaje quirúrgico anterior. Revise e inspeccione todo el instrumental y los implantes antes de la esterilización. Sustituya o añada cualquier componente necesario para la cirugía planificada. El cirujano principal debe contar con experiencia completa en las técnicas de cirugía de la columna vertebral necesarias. Prepare, posicione y cubra al paciente de la forma habitual. Se realiza un abordaje quirúrgico anterior estándar. En el nivel adecuado, se completa la discectomía y/o corpectomía, y se coloca el injerto óseo. Anote los números de lote de los implantes que se colocaron en el paciente durante la intervención quirúrgica en la ficha del paciente. Pautas quirúrgicas Procedimiento preoperatorio El cirujano debe estar totalmente familiarizado con todos los aspectos de la técnica quirúrgica, y conocer las indicaciones y las contraindicaciones de este tipo de implante. El cirujano debe haberse familiarizado antes de la cirugía con la técnica de introducción del producto específica, que se puede obtener del fabricante. Como parte del examen preoperatorio, el cirujano debe comprobar que no exista ningún factor biomecánico, biológico o de otro tipo que pueda afectar a la correcta realización de la cirugía y al periodo postoperatorio. Sólo se debe seleccionar a aquellos pacientes que cumplan los criterios descritos en las indicaciones de uso. No se deben seleccionar pacientes con afecciones y/o predisposiciones como las mencionadas anteriormente en las contraindicaciones. Se debe tener cuidado al manipular y almacenar los componentes del sistema Durango. Los implantes y los instrumentos se deben proteger durante el almacenamiento, especialmente contra ambientes corrosivos. Antes de iniciar la cirugía debe determinarse el tipo de constructo que se va a montar. El cirujano principal debe contar con experiencia completa en las técnicas de cirugía de la columna vertebral necesarias. En base a los resultados de las pruebas de fatiga, el cirujano deberá considerar los niveles de implantación, el peso del paciente, su nivel de actividad, otras posibles afecciones, etc., que puedan afectar al rendimiento de los componentes del sistema Durango. En el momento de la cirugía debe haber un inventario adecuado de implantes de diferentes tamaños, incluido un exceso de implantes del tamaño que se espera usar e implantes de tamaños menores y mayores de los que se espera que se ajusten al paciente. Revise e inspeccione todo el instrumental y los implantes antes de la esterilización. Sustituya o añada cualquier componente necesario para la cirugía planificada. Como hay involucradas piezas mecánicas, el cirujano debe estar familiarizado con los diversos componentes antes de usar el equipo, y debe montar personalmente cada dispositivo que vaya a usar para verificar que tiene todas las piezas y que cuenta con todos los instrumentos necesarios antes de comenzar la cirugía. Se debe contrastar la fecha de la cirugía con la fecha de caducidad de la calibración del mango limitador de torsión. No se deben usar instrumentos dañados o defectuosos. Contacte con el fabricante para solicitar instrucciones sobre la reparación o reemplazo. Todos los componentes se deben limpiar y esterilizar antes de usarse. Debe haber disponibles componentes estériles adicionales para casos de necesidad inesperada. Prepare, posicione y cubra al paciente de la forma habitual. Procedimientos operatorios La colocación quirúrgica del dispositivo Durango es parecida a los procedimientos de fusión intersomática ósea cuando se realizan de la forma habitual. • La preparación del paciente, la exposición quirúrgica y el procedimiento de fusión (preparación del espacio discal, colocación del injerto óseo, etc.), se realizan empleando las técnicas habituales del cirujano. • El cirujano expone los niveles adecuados para instrumentar, y prepara el sitio con instrumentos quirúrgicos manuales según sea necesario. • Extirpe la cantidad de hueso y tejido necesaria. • Tras preparar el espacio discal intervertebral, se utiliza el separador de disco incluido en el estuche para preparación del disco de Biomet Spine, para lograr el nivel de distracción deseado (hay disponibles separadores de disco en incrementos de 2 mm desde 8 mm hasta 20 mm). • Una vez retirado el material discal y preparadas las placas terminales en el nivel adecuado, se utiliza un calibrador de prueba de implantes para medir el espacio discal y determinar el tamaño correcto del implante. • Inicie la selección del tamaño del implante mediante la inserción del componente de prueba de altura y longitud más pequeñas. Utilice componentes de prueba de altura progresivamente mayor hasta que se logre la distracción de altura adecuada. La confirmación radiográfica determinará la altura y base adecuadas del componente de prueba para las condiciones anatómicas. • Se selecciona el cuerpo de PEEK-OPTIMA del tamaño correcto. Se elige la placa correspondiente según la preferencia del cirujano. Las dos piezas se montan con el tornillo de fijación y el mango de torsión calibrado usados para el montaje. El área central hueca del cuerpo de PEEK-OPTIMA se rellena con autoinjerto óseo. • El implante se bloquea firmemente en la punta del insertador con una barra roscada, se introduce en el espacio discal y se le dan golpes suaves según sea necesario hasta colocarlo en su posición final. En este momento puede terminar de colocarse el material de injerto adicional. La posición final se confirma utilizando rayos X y marcadores radiográficos. • El implante se fija a la anatomía con cuatro tornillos óseos. El tipo de tornillo utilizado (de tipo fijo/variable y la configuración de la punta) depende de la preferencia del cirujano. La preparación y la colocación de los tornillos se hacen en una forma secuencial, de manera que cada tornillo se coloca con ayuda de un destornillador después de que se haya preparado su lugar. La preparación se realiza usando guías, punzones y taladros. • Una vez colocados correctamente el implante y los tornillos óseos, se fija la placa cobertora correspondiente a la placa con el destornillador de placa cobertora y el mango de torsión calibrado. • El constructo se examina entonces para confirmar que todos los implantes se han colocado correctamente y el sitio quirúrgico se cierra mediante las técnicas habituales. Atención postoperatoria Asegúrese de que se siga un régimen de analgésicos, antibióticos, inmovilización y cuidado domiciliario. Se debe informar a los pacientes de las precauciones que deben adoptar en su vida diaria para garantizar una vida útil máxima del implante. Se recomienda realizar un seguimiento postoperatorio regular, para detectar indicios tempranos de fallo de los implantes y considerar la medida a adoptar. El deterioro del dispositivo después de la consolidación ósea no puede considerarse que constituya una disfunción o deterioro de las características de los implantes. Será necesario diseñar y llevar a cabo un programa de rehabilitación adecuado. Las instrucciones y las advertencias postoperatorias que el médico le da al paciente y el correspondiente cumplimiento por el paciente son sumamente importantes. Déle al paciente las indicaciones de cuidado postoperatorio adecuadas. La capacidad y disposición del paciente para cumplir con las instrucciones son algunos de los aspectos más importantes de una consolidación satisfactoria. Los avisos recomendados son: • Advierta al paciente que evite las caídas o las sacudidas bruscas en la posición espinal. • Proporcione al paciente instrucciones pormenorizadas sobre el uso y las limitaciones del dispositivo. • Si se recomienda o se requiere la carga de peso parcial antes de la unión ósea firme, debe advertirse al paciente de las complicaciones que pueden derivarse de la carga de peso excesiva o temprana o de la actividad muscular excesiva, a saber, el doblamiento, aflojamiento o rotura de los componentes. El riesgo de doblamiento, aflojamiento o rotura de un dispositivo de fijación interno durante la rehabilitación postoperatoria puede aumentar si el paciente es activo; o, si el paciente está debilitado, demenciado o no puede usar por alguna otra razón dispositivos de soporte del peso. • Para obtener la máxima probabilidad de un resultado quirúrgico satisfactorio, el paciente o el dispositivo no se deben exponer a vibraciones mecánicas que pudiesen aflojar el dispositivo. Advierta al paciente de esta posibilidad e indíquele que limite y restrinja las actividades físicas, especialmente las que requieran levantar objetos, girar el cuerpo y participar en cualquier tipo de deporte físicamente exigente. • Se debe aconsejar al paciente que no fume, no consuma productos con nicotina, no consuma alcohol ni use medicamentos antiinflamatorios no esteroideos como aspirina durante el proceso de consolidación del injerto óseo. • Se debe indicar al paciente su incapacidad para doblarse al nivel de la fusión espinal y enseñarle a compensar esta restricción física permanente en sus movimientos corporales. Si se desarrolla pseudoartrosis o los componentes se aflojan, doblan o rompen, revise y/o extraiga inmediatamente los dispositivos antes de que se produzca una lesión grave. Si no se inmoviliza una pseudoartrosis demorada del hueso, se aplicarán fuerzas excesivas y repetidas sobre el implante. Debido a la fatiga de materiales, estas fuerzas pueden causar el eventual doblamiento, aflojamiento o rotura de los dispositivos. Nunca reutilice los implantes quirúrgicos explantados. Aconseje a los pacientes con implantes que consideren tomar antibióticos profilácticos antes de someterse a cualquier otro tipo de cirugía (por ej., procedimientos dentales), ya que corren mayor riesgo por sus circunstancias especiales. La implantación del instrumental de fijación espinal deben realizarla sólo cirujanos de columna vertebral con experiencia, que cuenten con formación concreta en el uso de estos sistemas espinales. Se trata de un procedimiento exigente desde el punto de vista técnico que presenta un riesgo de lesión grave para el paciente. Envase Los componentes vienen en estuches metálicos de esterilización y/o en bolsas de polietileno precintadas. Tras su recepción, revise los envases para asegurarse de que todos los componentes estén intactos. Revise todos los juegos y confirme que todos los instrumentos y todos los implantes están presentes. Antes de utilizarlos, examine todos los componentes de cada juego y compruebe su funcionalidad para asegurarse de que no presentan daños. Devuelva inmediatamente al fabricante cualquier envase o producto dañado sin utilizarlo. Manipulación y almacenamiento Los implantes e instrumentos Durango deben manipularse, almacenarse y abrirse de forma tal que estén protegidos del daño o la contaminación por accidente. El dispositivo Durango no tiene una vida de almacenamiento marcada. No se ha observado ningún efecto del proceso de envejecimiento en el rendimiento mecánico de los materiales. Antes de cada procedimiento, verifique el funcionamiento correcto de los instrumentos quirúrgicos especializados que se necesitan para el procedimiento con el sistema Durango. Limpieza y descontaminación Los implantes e instrumentos Durango se suministran sin esterilizar. Antes de la esterilización, los implantes e instrumentos deben limpiarse siguiendo los procedimientos descritos a continuación. Precaución: Ciertas soluciones de limpieza, como las que contienen formalina, glutaraldehído, lejía y/u otros limpiadores alcalinos pueden dañar algunos dispositivos, especialmente los instrumentos. No deben usarse estas soluciones de limpieza. Nota: Puede ser necesario desmontar algunos instrumentos antes de la limpieza. Instrucciones de limpieza a máquina (recomendada) 1. Prepare el detergente de limpieza a. Prepare un detergente enzimático siguiendo las instrucciones del fabricante sobre su preparación y uso. b. NO se deben usar soluciones salinas porque son corrosivas para el acero inoxidable. c. El detergente debe tener un pH casi neutro para evitar las picaduras y el deslustre. 2. Prepare los dispositivos para remojarlos a. Para evitar lesiones, aparte los dispositivos afilados y puntiagudos, y manéjelos con cuidado. b. Desmonte los dispositivos que tengan partes desprendibles. c. Abra las uniones articuladas, dentadas o roscadas. d. Elimine los residuos pesados o grandes con toallitas de un solo uso que no suelten pelusa, empapadas con la solución de limpieza apropiada. 3. Limpie y remoje en un baño a. Sumerja los dispositivos en el baño preparado. b. Restriegue todas las superficies de los dispositivos con un cepillo de limpieza (no use cepillos con cerdas de acero) mientras están sumergidos en agua, asegurándose de eliminar toda la suciedad visible. c. Cuando corresponda: i. Use un escobillón para tubos y una jeringa para limpiar todas las cánulas, aberturas, hendiduras, ranuras y áreas de acceso difícil. ii. Use una jeringa para aplicar repetidamente solución de enjuague bajo presión para irrigar todas las cánulas, aberturas, hendiduras, ranuras y áreas de acceso difícil. iii. Mientras limpia las articulaciones opérelas, flexiónelas y muévalas repetidamente. iv. Cepille el interior de los espacios vacíos en toda su longitud. d. Permita que los dispositivos se remojen en un baño de detergente durante el periodo que recomiende el fabricante. 4. Coloque los dispositivos en la lavadora a. Coloque los dispositivos de manera que no choquen entre sí durante el ciclo de limpieza. b. Ponga los artículos pesados en la parte inferior y los objetos huecos en las canastas de la lavadora. c. Asegúrese de que ninguna pieza quede obstruida por objetos grandes. d. Coloque los instrumentos articulados en posición totalmente abierta y los instrumentos canulados en posición horizontal. e. Coloque los instrumentos desmontados en las canastas de la lavadora. 5. Ciclos de lavado y secado a. 2 minutos: Realice un lavado previo con agua fría; drene. b. 5 minutos: Lave con detergente y agua caliente; drene. c. 2 minutos: Neutralice con detergente con pH neutro; drene. d. 2 minutos: Enjuague con agua caliente; drene. e. Seque con aire caliente a una temperatura máxima de 115 ˚C. 6. Inspeccione a. Inspeccione los dispositivos a simple vista bajo condiciones normales de iluminación para determinar si se eliminó la suciedad visible pegajosa (p. ej., sangre, proteínas y otros residuos) de las superficies, aberturas, cánulas, hendiduras, bordes dentados, roscas, etc. b. Si todavía queda suciedad visible, repita el procedimiento de limpieza. Instrucciones de lavado manual 1. Prepare el detergente de limpieza a. Prepare un detergente enzimático siguiendo las instrucciones del fabricante sobre su preparación y uso. b. NO se deben usar soluciones salinas porque son corrosivas para el acero inoxidable. c. El detergente debe tener un pH casi neutro para evitar las picaduras y el deslustre. 2. Prepare los dispositivos para remojarlos a. Para evitar lesiones, aparte los dispositivos afilados y puntiagudos, y manéjelos con cuidado. b. Desmonte los dispositivos que tengan partes desprendibles. c. Abra las uniones articuladas, dentadas o roscadas. d. Elimine los residuos pesados o grandes con toallitas de un solo uso que no suelten pelusa, empapadas con la solución de limpieza apropiada. 3. Limpie y remoje en un baño a. Sumerja los dispositivos en el baño preparado. b. Restriegue todas las superficies de los dispositivos con un cepillo de limpieza (no use cepillos con cerdas de acero) mientras están sumergidos en agua, asegurándose de eliminar toda la suciedad visible. c. Cuando corresponda: i. Use un escobillón para tubos y una jeringa para limpiar todas las cánulas, aberturas, hendiduras, ranuras y áreas de acceso difícil. ii. Use una jeringa para aplicar repetidamente solución de enjuague bajo presión para irrigar todas las cánulas, aberturas, hendiduras, ranuras y áreas de acceso difícil. iii. Mientras limpia las articulaciones opérelas, flexiónelas y muévalas repetidamente. iv. Cepille el interior de los espacios vacíos en toda su longitud. d. Permita que los dispositivos se remojen en un baño de detergente durante el periodo que recomiende el fabricante. 4. Enjuague a. Saque los dispositivos del baño de remojo. b. Enjuague completamente los dispositivos bajo agua corriente durante 1 minuto como mínimo. c. Enjuague minuciosamente las cánulas, las aberturas y los agujeros. 5. Baño ultrasónico a. Prepare un baño ultrasónico con un detergente disolvente de la sangre siguiendo las instrucciones del fabricante sobre su preparación y uso. b. Cubra/selle los dispositivos durante su traslado del enjuague al baño ultrasónico para evitar su contaminación. c. Coloque los dispositivos en el baño ultrasónico. d. Asegúrese de que los dispositivos queden completamente sumergidos y que no se superpongan. e. Someta los dispositivos a ultrasonido durante 15 minutos. No exceda 15 minutos para evitar la corrosión. 6. Enjuague con agua estéril a. Enjuague minuciosamente los dispositivos con agua purificada estéril (p. ej., agua tratada por ósmosis inversa o agua desionizada) durante un mínimo de 3 minutos. 7. Seque a. Seque los dispositivos con toallitas absorbentes de un solo uso que no suelten pelusa y/o con aire comprimido de calidad médica (p. ej., el interior de las cánulas). b. Todos los derechos están reservados. m m m mm m m Objectif Le système Durango est un dispositif autonome d'arthrodèse intersomatique ALIF (arthrodèse intersomatique lombaire par voie antérieure)/de remplacement de corps vertébral qui fait partie du système d'arthrodèse intersomatique/de remplacement de corps vertébral (Vertebral Body Replacement, VBR) Breckenridge (aussi connu sous le nom de système Breckenridge). Description du dispositif Le dispositif Durango est un dispositif d’arthrodèse intersomatique constitué d’une entretoise intersomatique en PEEK-OPTIMA, d’une plaque de titane et de vis. L’entretoise a une forme globalement arrondie avec différentes hauteurs et superficies et comporte une zone centrale creuse pour recevoir une greffe osseuse autogène. Les surfaces supérieure et inférieure sont dotées d’une série de sillons transversaux dont la forme améliore la stabilité et la fixation après l’insertion du dispositif. La plaque de titane comporte des trous pour recevoir des vis à os et un orifice central pour recevoir une plaque de couverture qui empêche les vis de ressortir. Le dispositif Durango est disponible en plusieurs tailles et configurations pour s’adapter aux variations des différents niveaux vertébraux et/ou à l’anatomie du patient. Le système Durango est fourni non stérile. Matériaux Le système Durango est constitué de plaques, de vis et d’entretoises intersomatiques. Les plaques et les vis sont en alliage de titane (Ti-6Al-4V ELI) de qualité implant qui répond aux exigences des normes ASTM F136-08 et ISO 5832-3. Le polymère PEEK-OPTIMA® LT1 est un thermoplastique polyaromatique semi-cristallin qui répond aux exigences des normes ASTM F-2026 et ISO 10993. Chimiquement, la formule de base du matériau est (-C6H4-O-C6H4-O-C6H4-CO-)n, et il est connu sous le nom générique de polyétheréthercétone (PEEK). Tous les implants PEEK-OPTIMA sont composés de ce matériau de qualité « implant » et contiennent des marqueurs de position en tantale qui répondent aux exigences de la norme ASTM F560. Indications thérapeutiques En tant que dispositif d’arthrodèse intersomatique lombaire, l’implant Durango est destiné aux interventions d’arthrodèse rachidienne utilisant une greffe osseuse autogène chez des patients de maturité squelettique présentant une discopathie dégénérative (« DD ») à un ou deux niveaux contigus du rachis, de L2 à S1. La discopathie dégénérative est définie par la présence d’une douleur au dos d’origine discale associée à une dégénérescence du disque confirmée par les antécédents du patient et des études radiographiques. Les patients auront reçu un traitement non chirurgical pendant au moins six mois. Ces patients DD pourront avoir subi une intervention chirurgicale rachidienne non-arthrodésique à un ou plusieurs niveaux rachidiens, et peuvent présenter un spondylolisthésis ou un rétrolisthésis de grade 1 au(x) niveau(x) concerné(s). Le dispositif Durango est implanté par voie antérieure ou postérieure et doit être combiné à un système de fixation additionnel (sauf comme indiqué cidessous). Les systèmes de fixation additionnels approuvés comprennent le système de fixation rachidienne Biomet. Lorsqu’ils sont utilisés avec les vis de fixation intégrées, les implants intersomatiques autonomes Durango n’ont pas besoin de fixation additionnelle. Contre-indications Les contre-indications peuvent être relatives ou absolues. Le choix d’un dispositif particulier doit être soigneusement considéré en tenant compte de l’évaluation globale du patient. Les circonstances ci-dessous peuvent réduire les chances d’un résultat positif. Les contre-indications incluent, entre autres : • Une allergie au PEEK-OPTIMA, aux alliages de titane ou cobalt chrome, ou une sensibilité aux corps étrangers. Het risico van buigen, losraken of breken van een inwendig fixatiehulpmiddel tijdens postoperatieve revalidatie kan groter worden als de patiënt actief is of als de patiënt invalide of dement is of om andere redenen niet in staat is gewichtondersteunende hulpmiddelen te gebruiken. • Voor een optimale kans op een succesvol resultaat van de ingreep mag de patiënt of het hulpmiddel niet worden blootgesteld aan mechanische vibraties, waardoor de constructie van het hulpmiddel kan loskomen. Waarschuw de patiënt over deze mogelijkheid en draag de patiënt op om lichamelijke activiteiten te begrenzen en beperken, vooral activiteiten waarbij tillen en draaibewegingen nodig zijn en elke soort lichamelijk veeleisende sportbeoefening. • De patiënt moet aangeraden worden niet te roken, geen nicotineproducten te gebruiken, geen alcohol te drinken en geen niet-steroïde anti-inflammatoire middelen zoals aspirine te gebruiken tijdens het genezingsproces van het bottransplantaat. • De patiënt moet erop gewezen worden dat buigen niet mogelijk is op het punt van de spinale fusie en hij/zij moet leren compenseren voor deze permanente fysieke restrictie in de lichaamsbeweging. In geval van non-union of als de componenten losraken, verbuigen en/of breken, moeten de hulpmiddelen onmiddellijk worden gereviseerd en/of verwijderd voordat ernstig letsel optreedt. Als een vertraagde non-union van het bot niet wordt geïmmobiliseerd, zal het implantaat aan overmatige en herhaalde spanningen blootstaan. Door materiaalmoeheid kunnen deze spanningen uiteindelijk tot buigen, losraken of breken van het (de) hulpmiddel(en) leiden. Gebruik geëxplanteerde chirurgische implantaten nooit opnieuw. Adviseer patiënten met implantaten dat ze de toediening van profylactische antibiotica dienen te overwegen voordat ze een volgende ingreep krijgen (zoals tandheelkundige procedures) voor patiënten die extra omstandigheden hebben die hun risico vergroten. De implantatie van spinale fixatie-instrumentatie mag alleen worden uitgevoerd door chirurgen met ervaring in spinale operaties en met specifieke opleiding in het gebruik van dergelijke spinale systemen. Dit is een technisch veeleisende procedure die een risico van ernstig letsel aan de patiënt met zich meebrengt. Verpakking Onderdelen zijn verpakt in metalen sterilisatiedozen en/of verzegelde polyethyleen zakken. Controleer de verpakkingen om er zeker van te zijn dat alle onderdelen bij ontvangst intact zijn. Controleer alle sets en bevestig dat alle instrumenten en implantaten aanwezig zijn. Inspecteer vóór gebruik alle setonderdelen op functionaliteit om er zeker van te zijn dat er geen beschadiging is. Beschadigde verpakkingen of producten onmiddellijk ongebruikt aan de fabrikant retourneren. Hantering en opslag De Durango-implantaten en -instrumenten moeten zodanig worden gehanteerd, opgeslagen en geopend dat ze beschermd zijn tegen onbedoelde schade of verontreiniging. Het Durango-hulpmiddel heeft geen aangegeven houdbaarheidsdatum. Het verouderingsproces heeft geen waargenomen effect op de mechanische prestatie van de materialen. Verifieer het juiste functioneren van de gespecialiseerde chirurgische instrumenten nodig voor de Durango-systeem procedure vóór elke chirurgische ingreep. Reiniging en ontsmetting Durango-implantaten en -instrumenten worden niet-steriel geleverd. Vóór sterilisatie moeten implantaten en instrumenten worden gereinigd met gebruik van de onderstaande procedures. Let op: Bepaalde reinigingsoplossingen zoals oplossingen die formaline, glutaaraldehyde, bleekmiddel bevatten en/of andere alkalische reinigingsmiddelen kunnen sommige hulpmiddelen, waaronder in het bijzonder instrumenten, beschadigen. Dergelijke reinigingsoplossingen mogen niet worden gebruikt. NB: Sommige instrumenten moeten misschien vóór het reinigen worden gedemonteerd. Instructies voor machinereiniging (aanbevolen) 1. Gereedmaken van reinigingsdetergens a. Maak een enzymatisch detergens gereed volgens de instructies van de fabrikant voor bereiding en gebruik. b. Fysiologische zoutoplossingen mogen NIET worden gebruikt omdat deze een corrosief effect op roestvrij staal hebben. c. Het detergens moet een bijna neutrale pH-waarde hebben om pitvorming en verkleuring te voorkomen. 2. Gereedmaken van hulpmiddelen voor weken a. Houd scherpe en puntige hulpmiddelen apart en hanteer ze voorzichtig om letsel te voorkomen. b. Demonteer hulpmiddelen met afneembare onderdelen. c. Open verbindingen met scharnieren, tanden of schroefdraad. d. Verwijder zware of grote deeltjes met pluisvrije doekjes voor eenmalig gebruik die in een geschikte reinigingsoplossing zijn gedompeld. 3. In een bad reinigen en weken a. Dompel de hulpmiddelen onder in een gereedgemaakt bad. b. Borstel alle oppervlakken van de hulpmiddelen met een reinigingsborstel (gebruik geen stalen borstels) terwijl ze in water zijn ondergedompeld en controleer of al het zichtbare vuil is verwijderd. c. Waar van toepassing: i. Gebruik een pijpenrager en injectiespuit om alle canules, lumina, gleuven, groeven en moeilijk bereikbare plaatsen te reinigen. ii. Gebruik een injectiespuit om alle canules, lumina, gleuven, groeven en moeilijk bereikbare plaatsen herhaaldelijk met een spoeloplossing onder druk door te spoelen. iii. Zorg dat u tijdens het reinigen de beweegbare verbindingen herhaaldelijk in werking brengt, buigt en scharniert. iv. Borstel de binnenkant van holle ruimten over de volle lengte. d. Laat de hulpmiddelen in een detergensbad weken gedurende de door de fabrikant aanbevolen tijd. 4. Hulpmiddelen in de wasmachine laden a. Plaats de hulpmiddelen zodanig dat ze tijdens het wassen niet tegen elkaar stoten. b. Plaats zware voorwerpen op de bodem en holle voorwerpen in de mandjes van de wasmachine. c. Controleer of de onderdelen niet door grotere voorwerpen worden belemmerd. d. Plaats scharnierende instrumenten in de volledige open-stand en gecanuleerde instrumenten horizontaal. e. Plaats gedemonteerde instrumenten in de mandjes van de wasmachine. 5. Was- en droogcycli a. 2 minuten: Voorwassen met koud water; water weg laten lopen. b. 5 minuten: Wassen met detergens en heet water; water weg laten lopen. c. 2 minuten: Neutraliseren met detergens met neutrale pH; water weg laten lopen. d. 2 minuten: Afspoelen met heet water; water weg laten lopen. e. Drogen met hete lucht bij maximaal 115 ˚C. 6. Inspecteren a. Inspecteer de hulpmiddelen met het blote oog onder normale lichtcondities om te zien of al het klevende, zichtbare vuil (bijv. bloed, eiwitten en andere deeltjes) van de oppervlakken, lumina, canules, gleuven, kartelingen, schroefdraden enzovoort is verwijderd. b. Als er zichtbaar vuil achterblijft, moet de reinigingsprocedure worden herhaald. Instructies voor handmatig reinigen 1. Gereedmaken van reinigingsdetergens a. Maak een enzymatisch detergens gereed volgens de instructies van de fabrikant voor bereiding en gebruik. b. Gebruik een injectiespuit om alle canules, lumina, gleuven, groeven en moeilijk bereikbare plaatsen herhaaldelijk met een spoeloplossing onder druk door te spoelen. iii. Zorg dat u tijdens het reinigen de beweegbare verbindingen herhaaldelijk in werking brengt, buigt en scharniert. iv. Borstel de binnenkant van holle ruimten over de volle lengte. d. Laat de hulpmiddelen in een detergensbad weken gedurende de door de fabrikant aanbevolen tijd. 4. Spoelen a. Neem de hulpmiddelen uit het weekbad. b. Spoel de hulpmiddelen ten minste 1 minuut grondig onder stromend water. c. Spoel de canules, lumina en openingen grondig door. 5. Ultrasoonbad a. Maak een ultrasoonbad gereed met een detergens voor het oplossen van bloed, volgens de instructies van de fabrikant voor bereiding en gebruik. b. Breng de hulpmiddelen afgedekt/afgedicht over van de spoelplaats naar het ultrasoonbad om besmetting te voorkomen. c. Plaats de hulpmiddelen in het ultrasoonbad. d. Controleer of de hulpmiddelen volledig zijn ondergedompeld en elkaar niet overlappen. e. Sonificeer gedurende 15 minuten. Sonificeer niet langer dan 15 minuten om corrosie te voorkomen. 6. In steriel water spoelen a. Spoel de hulpmiddelen ten minste 3 minuten grondig met steriel, gezuiverd water (d.w.z. RO of DI). 7. Drogen a. Droog de hulpmiddelen met pluisvrije absorberende doekjes voor eenmalig gebruik en/of perslucht van medische kwaliteit (bijv. voor de binnenkant van canules). b. Onmiddellijk na het spoelen de hulpmiddelen volledig drogen om corrosie te voorkomen. 8. Inspecteren a. Inspecteer de hulpmiddelen met het blote oog onder normale lichtcondities om te zien of al het klevende, zichtbare vuil (bijv. bloed, eiwitten en andere deeltjes) van de oppervlakken, lumina, canules, gleuven, kartelingen, schroefdraden enzovoort is verwijderd. b. Als er zichtbaar vuil achterblijft, moet de reinigingsprocedure worden herhaald. Sterilisatie De Durango-implantaten en -instrumenten worden niet-steriel geleverd en moeten vóór gebruik worden gereinigd en gesteriliseerd. Het aanbevolen sterilisatieproces is met stoom in de autoclaaf, met gebruik van de parameters die in de onderstaande tabellen zijn vermeld. Het gebruik van een wikkel (alleen in de VS: door FDA goedgekeurde wikkel) wordt aanbevolen om steriliteit vóór gebruik te handhaven. De aanbevolen sterilisatiecycli zijn gevalideerd om een Sterility Assurance Level (SAL) van ten minste 10-6 te garanderen.
