Completing the Form
Move through the choices on the list by using the scroll bar or entering the first letters of
your selection. Make your selection by double-clicking on the appropriate choice.
Your selection displays in that item and the cursor moves to the next item.
If your union is affiliated with a national or international union and you are not using prefill data, you must select your entries for Item 4, Affiliation or Organization Name, and
Item 5, Designation, from the drop-down lists. See Union Identification – Items 4 – 7 in
this User Guide.
