Hand Hygiene
HOW AND WHEN TO CLEAN HANDS IN THE DENTAL SURGERY
Hands must be cleaned immediately before each and every episode
of dental patient care, after contact with saliva, blood or other bodily
fluid and before donning gloves. This prevents contamination of the
patient’s mouth and face with organisms carried on the dental team’s
hands.
Hands must be cleaned immediately after every episode of patient
care and following removal of gloves to minimise contamination of
the dental surgery environment and oneself.
Working bare below the elbow encourages effective hand hygiene.
Removal of rings and avoidance of nail polish reduces the carriage of
pathogens on the hands.
A standardised hand hygiene technique should be used to ensure
that all surfaces of the hands and wrists are exposed to the soap or
disinfectant and are thoroughly cleaned in a systematic manner.
15 to 30 seconds is recommended to clean hands and wrists when
using plain & antibacterial soaps and waterless products. Performing
surgical hand hygiene to clean hands, wrists and forearms requires 2
to 3 minutes.
When using soaps and water, dry hands thoroughly afterwards as
bacteria can proliferate on damp hands. Drying hands also helps
to maintain skin integrity and prevent skin damage. Microbes grow
faster and are shed in greater numbers from damaged skin.
Use a moisturiser regularly to protect the skin from dehydration and
damage.
HCAI Code of Practice and HTM 01-05 specifies training in hand
hygiene as an essential quality requirement for all existing staff and
new staff during their induction.
Motivation and compliance can be encouraged by following the
example set by senior members of the dental team, the use of posters
and regular hand hygiene audits with feedback.
Employ vigorous rubbing to create friction which physically removes
microbes. Duration of hand hygiene is also important.

THE IMPORTANCE OF GOOD HAND HYGIENE

Our hands are covered with many transient microbes most of which are harmless and live in harmony with our skin.
However, some of these bacteria are pathogenic and can cause harm to our patients and staff. Because of this, hand
hygiene is an important part of our daily environmental cleaning processes. If efficient hand hygiene procedures are not
performed adequately, then the risk of cross contamination in categorised risk areas is far greater, and poses a risk of
infection to patients and staff. The diagram above shows some examples of the techniques involved in this process.

Hand cream, preferably water-based,
should be used to avoid chapped
or cracking skin. Communal jars of
hand cream are not desirable as the
contents may become contaminated
and subsequently become an
infection risk. Ideally, wall-mounted
hand-cream dispensers with
disposable cartridges should be
used. Any staff that develop eczema,
dermatitis or any other skin condition
should seek advice from their
occupational health department or
general practitioner (GP) as soon as

PERSONAL PROTECTIVE EQUIPMENT (PPE)
PPE is part of Standard Precautions for infection prevention and
control in the dental surgery.

PPE should not be relied on for sole protection. PPE should be used
in combination with other preventive measures such as safe working
practices, adequate surgery ventilation and immunisation.

Sterile gloves should always be used for invasive surgery.
Heavy duty gloves rather than examination gloves should be worn
for manual cleaning dental instruments as they are less likely to
be punctured by sharp instruments and protect hands from irritant
chemicals.
To avoid contaminating oneself, remove gloves aseptically so that
the outside of the glove is not touched with the bare hand. Note that
gloves can develop tears during use and may leak, so hands are
required to be cleaned after disposing of gloves.

Goggles or visors should be worn during all types of dental treatment
or when manually cleaning instruments.

MASKS AND RESPIRATORY MASKS
Masks are single use PPE and recommended for all dental
procedures.
The main function of a mask is to provide barrier protection against
splatter. Most masks produce a poor facial seal and therefore do not
protect the wearer effectively from aerosol inhalation.
Some multi-layered masks with metal nose strips if adapted closely
to the face will provide up to 70% air filtration.
Try to avoid touching the outer surface of the mask, which may be
contaminated.
Remove the mask at the end of the patient’s treatment by breaking or
undoing the straps and lifting over the ears.

Manual cleaning is still accepted as a method under HTM 01-05: 2013
Edition allowing you to comply with essential quality requirements.
However, it is the least effective method of cleaning instruments, the
hardest to validate, and carries an increased risk of ‘sharps injury’.
WHEN IS IT PERFORMED?
Manual cleaning must be carried out in dedicated sinks that are not

Ultrasonic Cleaning TESTING & VALIDATION
WHAT IS IT?
HTM 01-05: 2013 Edition has a structured test schedule for checking,
testing and validating ultrasonic cleaners which should be adhered to
if no manufacturer guidelines have been provided.
Validation is performed to ensure that the different elements of the
process are functioning correctly. This ranges from simple checks to
make sure that the machine can clean soiled instruments effectively,
to more complicated checks to demonstrate that the ultrasonic
activity within the bath is adequate.
For a testing and validation schedule specific to your ultrasonic
cleaner which takes into consideration HTM 01-05 and manufacturer
recommendations, create your own Validation Test Schedule from the
Isopharm website.
WHEN IS IT PERFORMED?
HTM 01-05 requires that the equipment used during the
decontamination processes is regularly checked by the practice daily
and weekly, and a qualified validation engineer either quarterly or
annually in accordance with machine manufacturers guidance.

Washer Disinfectors TESTING & VALIDATION
WHAT IS IT?
HTM 01-05: 2013 Edition has a structured test schedule for checking,
testing and validating washer disinfectors which should be adhered to
if no manufacturer guidelines have been provided.
Validation is performed to ensure that the different elements of the
process are functioning correctly. This ranges from simple checks to
make sure that the machine can clean soiled instruments effectively,
to more complicated engineer checks to demonstrate that the
process is operating at the correct disinfection time and temperature.
For a testing and validation schedule specific to your ultrasonic
cleaner which takes into consideration HTM 01-05 and manufacturer
recommendations, create your own Validation Test Schedule from the
Isopharm website.
WHEN IS IT PERFORMED?
HTM 01-05 requires that the equipment used during the
decontamination processes are regularly checked by the practice
daily and weekly, and a qualified validation engineer either quarterly

This is a test that ensures that the cycle is
able to adequately clean a heavily soiled
load. Only the test soil recommend by the
manufacturer should be used. HTM 01-05 has
cleaning efficacy test as a daily and quarterly
requirements, however there is a difference
in how they are performed. Daily cleaning
efficacy is a visual check that requires the
use of an inspection magnification lamp.
Quarterly cleaning efficacy requires a test
soil such as the products listed. Some
manufacturer’s recommend the quarterly test
method, with use of a test soil, is carried out
more frequently.

Instruments Inspection
WHAT IS IT?
The cleaning of instruments is an important part of the
decontamination cycle. If this is not done correctly then the rest of the
process will fail.
HTM 01-05: 2103 Edition specifies that each instrument must be
visually inspected after cleaning, and further recommends the use
of an illuminated magnifier to improve the ease and efficiency of the
inspection.
If an instrument is not clean, then it can not be sterilised.
Instruments that are blunt, bent, damaged or show any signs of
pitting or other corrosion should be discarded or, if appropriate, sent
for repair.
WHEN IS IT PERFORMED?
Visual inspection should be carried out after the cleaning process but
before sterilisation.
The reason why a visual inspection is required before sterilisation
is to make sure that the cleaning process is removing staining and
debris. If an instrument is not properly cleaned it may impede the
sterilisation process.

Sterilisers TESTING & VALIDATION
WHAT IS IT?
Sterilisation by means of steam (moist heat) is the recommended
method for processing most dental instruments.
that sterility will be achieved, whatever steriliser is used, due to the
presence of lubricating materials.
There are three types of small steam steriliser available. Each has
specific operating conditions and load limitations.
Type B Steriliser: Vacuum
Type N Steriliser: Non-Vacuum
Air removal is critical to enable steam to condense on instruments
and effect sterilisation.
Also known as Vacuum, Vacuum Assisted, and Porous Load sterilisers.
Also known as Gravity, Downward Displacement, and Bowl &
Instrument sterilisers.
In a Type B steriliser air removal is active due to vacuum assistance,
allowing appropriately wrapped items to be processed as well as
lumen devices and hand pieces. The air is actively removed, and the
steam forced into any lumens allowing for the sterilisation of hollow
devices.
Air removal is critical to enable steam to condense on instruments
and effect sterilisation. In a Type N steriliser air removal is passive
and for this reason packaged instruments cannot be sterilised in
these machines.
Devices with narrow lumens, including hand pieces, should not
normally be sterilised in Type N sterilisers because the steam will not
be forced into the lumen due to passive air removal.
However, currently HTM 01-05: 2013 Edition permits the use of a
Type N steriliser for processing lubricated hand pieces.
Type S Steriliser: Vacuum
This type of steriliser is designed to reprocess specific load types.
The manufacturer of the steriliser must define exactly which load or
instrument types are compatible.
Type S sterilisers should be used strictly in accordance with
manufacturer instructions.
With regards to lubrication, HTM 01-05 states that it is unlikely

manufacturer safety checks and HTM 01-05 tests have been carried
out on the steriliser.

Regardless of the type of steriliser used, the critical parameters for
sterilisation remain the same:
Temperature - The preferred temperature is 134ºC to 137ºC.
Time - A minimum of three (3) minutes holding time once the load has
reached the temperature of 134ºC.
Pressure - Approximately 2.2 Bar. The pressure is associated with the
development of steam at 134ºC and is typically recorded in addition to
the temperature.

The daily tests should be performed by the operator
or user and will normally consist of: 1. Steam
penetration test - HELIX or BOWIE DICK tests
(vacuum steriliser only), 2. an automatic control test
(all benchtop steriliser) in line with manufacturers’
instructions, 3. a record of the temperature and
pressure achieved at the daily test, to ensure this
is satisfactory before the autoclave is used for
sterilising instruments.

HELPING YOU CHOOSE THE RIGHT PRODUCT
The Differences between Helix Devices
Helix devices do look very similar
to each other. They are made up of
a holder for a test strip that is held
sealed at the end of a length of tube.
Where the Helix will differ is with the
length of tubing.
A Helix for use with a type B steriliser
will have a tube length of 150cm
(1.5 meters). This length is directly
associated with an EN standard for the
validation of type B sterilisers.
A Helix for use with a type S steriliser
does not have a governing EN. It is
the manufacturer of the steriliser who
dictates the length of tubing based
on their specific machine. The most
common length of tube for type S is
30cm (0.3 Meters).
Another difference between Helix is
the price. The characteristics of a type
B Helix are written in the specific EN
document so it can be manufactured
by anyone with the ability to do so.
This means that there are many
types on the market which results in
competitive pricing and reduced cost
for the end user.
The ability to manufacture a
type S Helix is with the machine
manufacturer. This means that,
no matter where the device was
purchased from, it will have come
from the manufacturer of the machine.
As there is no competition for the
product, the price is at a premium.

TRANSPORTING CONTAMINATED INSTRUMENTS
Once instruments are contaminated and require decontamination, they
need to be taken to the decontamination area.
This process needs to be carried out safely and securely in order to
prevent contamination, to protect the instruments and to protect the
staff member handling the transportation.
It is advisable to use transport boxes even if decontamination is
performed in the patient treatment area as contaminated instruments
should always be stored in a secure manner.
HTM 01-05 requests that dedicated transport boxes are used which
are:

TRANSPORTING & STORING DECONTAMINATED INSTRUMENTS
Instruments that are pouched before sterilisation, or immediately
after sterilisation may be stored for the periods defined in HTM 01-05:
2013 Edition.
Storage facilities should provide ease of finding the correct instrument
with minimal handling and have an effective stock rotation, First In
First Out.
HTM 01-05 2013 Edition defines the storage time of a pouched
instrument(s) to be 1 year providing the pouch is not compromised
(damaged in any way) This is regardless of the type of steriliser used
within your dental practice.
As part of practice procedures, protocols should be put in place to
check the expiry dates of instruments.
The storage time can also be dictated locally. At the time of going
to print, local requirements for the storage of instruments that are
pouched either before or immediately after sterilisation are:
England: 1 Year
Wales: Wrapped instruments can be stored for one year. If using a
non-vacuum steriliser it is recommended that wrapped instruments
stored for more than one month should be reprocessed.
Scotland: 60 Days
It is expected that the pouch should be dated when closed. Best
practice would be to put both the sterilisation date and the expiry
date for ease of reference. Should only one date be used it is widely
recognised by industry advisers and auditors such as the CQC that it
is the expiry date that is written on the pouch.
Where unwrapped instruments are to be used in the same day as
they are sterilised, they should be stored covered to protect from
contamination by aerosols in clean and dry conditions until used.
Instruments stored in this way must be reprocessed if they are not
used within that same day.
Instruments stored un-pouched in the surgery should be kept on
lidded trays or in covered drawers as far away from the dental chair
as reasonably practicable.
Instruments should not be stored on open work surfaces, particularly
in clinical areas. It is important that practices have well developed
protocols and procedures in place to prevent contamination of these
instruments by ensuring that those required for a particular patient
are removed from their protected environment before treatment
commences. This eliminates the need to open cupboard doors or
drawers during patient treatment. If an instrument does need to be
retrieved from a cupboard or drawer during treatment, the practice
should have protocols in place to prevent contamination and to ensure
that staff hands are clean and that new gloves are donned before
handling unwrapped sterilised instruments.
Unwrapped instruments in the clinical area can be stored up to
1 day. All instruments, regardless if they have been used require
reprocessing at the end of the working day, or the beginning of the
next working day.
HTM 01-05 2013 Edition has introduced an additional storage option
which allows dental practices to store un-pouched instruments
in a non clinical area. This can be either an unused surgery or the
clean area of a separate decontamination room. Instruments that
are stored in this area can be kept for 1 week without the need to
be reprocessed. Instruments should still be stored dry and protected
from contamination.
To minimise reprocessing, only set out the instruments that are
reasonably expected to be used on a particular treatment. All
instruments set out for a treatment must be reprocessed even if they
have not been used.
Storage facilities must ensure that the instrument packs are protected
from contamination and protected from moisture.
Damp packaging will render the pack permeable to microorganisms.

ULTRASONIC CLEANING PROCESS - WHAT IS IT?
HTM 01-05: 2013 Edition deems ultrasonic baths to be optional, they
are not required to meet either Essential Quality Requirement or Best
Practice.

WASHER DISINFECTOR PROCESS - WHAT IS IT?
Washer Disinfectors are the preferred method of cleaning instruments
and medical devices. Critical cycle parameters are fully controlled,
repeatable and a record of the cycle is usually available via printout
or memory card. The process is fully automated. Washer disinfectors
both clean and disinfect consecutively during a process cycle.
Washer disinfectors are a requirement to achieve Best Practice
compliance.
All Washer Disinfectors should be operated in accordance with the
manufacturer’s instructions and local protocols.
WASHER DISINFECTOR PROCESS - WHEN IS IT PERFORMED?
The washer disinfector process should be performed after any
required manufacturer safety checks and HTM 01-05 tests have been
carried out on the washer disinfector.
A typical washer disinfector cycle for instruments includes the
following five stages:
Flush
Removes ‘difficult’ gross contamination, including blood, tissue
debris, bone fragments and other fluid and solid debris. Latest
standards indicate that a water temperature of less than 45ºC is used
to prevent protein coagulation and fixing of soil to the instrument.
Wash
Removes any remaining soil. Mechanical and chemical processes
loosen and break up contamination adhering to the instrument
surface. Detergents used in this process must be specified by
the manufacturer as suitable for use in a washer-disinfector and
compatible with the quality of water used. Detergents should also be
compatible with the instruments being processed to avoid instrument
degradation including discolouration, staining, corrosion and pitting.

Rinse
Removes detergent used during the cleaning process. This stage
can contain several sub-stages. The quality of water to be used for
this stage is an important consideration in terms of ensuring a clean
unmarked product after sterilisation. Advice should be taken from
manufacturers with respect to the compatibility of the hardness or
quality of the water supply with the equipment and detergents used.
Thermal Disinfection
The temperature of the load is raised and held at the pre-set
disinfection temperature for the required disinfection holding time, for
example 80ºC for ten minutes or 90ºC for one minute.
Drying
Purges the load and chamber with heated air to remove residual
moisture.

Surface Cleaning
SURFACE CLEANING & DISINFECTION
Surface cleaning and disinfection is critical for effective infection
control measures in the dental surgery. The practice should have
a local protocol clearly outlining the surface and room-cleaning
schedules. This can be in the form of an infection control policy,
infection control statement and daily surgery log book checks. The
practice manager should carry out regular surgery spot checks.
Bacteria, viruses and fungi may be present in the surgery and
in particular on the work surfaces after treating a patient, so the
cleaning and disinfection of surfaces is paramount to patient safety.

Since the introduction of HTM 01-05 many dental practices have
been given the impression that the use of alcohol-based wipes is
prohibited: this is not the case.
HTM 01-05: 2013 Edition, Section 6.57
Providing that a dental practice has a risk assessment and procedure
on the use of alcohol based products, and their use does not conflict
with the above exert from HTM 01-05, then alcohol wipes can have a
place within the dental practice.

surfaces. If there is obvious blood contamination, the presence of
protein will compromise the efficacy of alcohol-based wipes.’

The use of alcohol with dental instruments should therefore be

USING SPRAY BOTTLES IN A DENTAL PRACTICE
When using a spray it is best to spray directly into a dry wipe or
microfiber cloth to minimise aerosol. Never directly spray a visibly
contaminated area as this will create contaminated aerosol.
Spray bottles should be single use only and not refilled when empty.

GUIDANCE ON SPILLAGES
Spillages within a dental surgery are inevitable at some point. Careful
planning and protocols are required to ensure that they are dealt with
in a consistent approach that protects the patient, the employee and
the employer.
Under Fundamental Standards Regulation 12 section 2.b:
•
Providers must do all that is reasonably practicable to mitigate
risks. They should follow good practice guidance and must adopt
control measures to make sure the risk is as low as is reasonably
possible. They should review methods and measures and
amended them to address changing practice.
•
Providers should use risk assessments about the health, safety
and welfare of people who use their service to make required
adjustments. These adjustments may be to premises, equipment,
staff training, processes, and practices and can affect any aspect
of care and treatment.
It is important to have the following in place:
•
•
A policy and procedure outlining cleaning and disposal of a bodily
fluid or hazardous products in a safe and effective manner by
appropriately trained persons. It should be mindful of the control
of substances hazardous to health.
Staff training in the correct procedure for dealing with a spillage.
Ensure that they know where the spillage kit is kept, how it is
used, and the procedure for reporting a spillage.

Bodily Fluid Spillage Kit
The GV Health Standard Bodily Fluids Spill Kit contains four individual spill
cleaning packs, 2 for blood spills and 2 for urine and vomit spills. The kit is
suitable for safe, effective cleaning and disinfection of up to 4 biohazard spillages.
The small pack makes it very easy to keep ready for use in all areas and
ensure accessibility where it may be needed. The one use pack format prevents

Urine/Vomit Spill
GV Absorbent Granules have been developed for use on acidic spills such as urine
and vomit. The active ingredient Tricloscan is an organic anti-bacterial agent. With
a high absorbency to mass ratio, 100g of granules absorbs up to 7 litres of liquid.
Disinfectant wipes are provided to wipe down the surface and surrounding area
where spillages have occurred and remove any remaining residue.

For blood spillages, care should be taken to observe a protocol that ensures protection against infection. The use of hypochlorite at 1000 ppm available chlorine is recommended.
Hypochlorite should be made up either freshly using hypochlorite-generating tablets or at least weekly in clean containers. Contact times should be reasonably prolonged (not less
than five minutes). A higher available chlorine concentration of 10,000 ppm is useful, particularly for blood contamination. The process should be initiated quickly and care should be
taken to avoid corrosive damage to metal fittings etc. The use of alcohol within the same decontamination process is not advised.

• Cardiorespiratory arrest is rare in primary dental practice.
• There is a public expectation that dental practitioners and all
other dental care professionals should be competent in treating
cardiorespiratory arrest.
Give reassurance to the casualty.
• All primary care dental facilities should have a process for medical
risk-assessment of their patients.
• Specific resuscitation equipment should be available immediately
in all primary care dental premises. This equipment list should be
standardised throughout the UK.
• All clinical areas should have immediate access to an automated
external defibrillator (AED).
• Primary dental care providers, general dental practitioners and
all other dental healthcare professionals should undergo training
in cardiopulmonary resuscitation (CPR) including basic airway
management and the use of an AED.
Continue to administer oxygen at 15 litres
through a non-rebreathe mask.
Give the casualty 300mg of aspirin to
chew if:
• There should be regular practice and teaching using simulationbased cardiorespiratory arrest scenarios.

Lay the casualty down and raise their legs
above the level of their pelvis.
Place the casualty in a sitting position.
Leaning the casualty forward onto a table
or the back of a chair may help.
Encourage the casualty to cough, bending
them forward.
Give reassurance and stay with the casualty.
Ask the casualty if they suffer from
Asthma and if they have their inhaler
with them.
When the casualty feels a little better, give
them 200ml of Glucose drink.
Keep the casualty calm and
stay with them.
The casualty should begin to
improve rapidly within one minute.
Encourage the casualty to breath slowly
and deeply.
Allow the casualty to sit up when they
feel able to.
If the object has not been expelled, give up
to 5 abdominal thrusts.
For a baby under 1 year old, give up to 5
chest thrusts.
Never give abdominal thrusts.
Ask someone to dial 999 for an ambulance.
The casualty should begin to improve
rapidly within two minutes.
If there is no improvement or the casualty
loses consciousness, place in
the recovery position.

Dental Unit Waterlines
DENTAL UNIT WATERLINES
Dental Unit Waterlines (DUWL) can be contaminated by microbes
coming from three different sources:
-
-
Suck-back of oral bacteria via the dental handpiece.
-
From hands and the surgery environment during filling and
handling of the self-contained water bottles.
Incoming mains water which is a possible source of legionella
and mycobacteria spp.
KEY DISEASE CAUSING MICROBES IN DUWL
A variety of organisms are found in DUWL including bacteria, fungi
and protozoa. Of particular concern to health are the respiratory
disease causing bacteria such as legionella spp, atypical
mycobacterium spp and pseudomonas species.
The dental team is exposed to contaminated aerosols on a daily basis
over the long term, and patients are exposed in the short term during
treatments.
The high speed rotation of the airturbine aerosolises the DUWL
microbes providing a route of transmission particularly for respiratory
bacteria.
Contaminated aerosols can be inhaled (portal of entry described in
the chain of infection) by both the dental team and patients during
treatment.
The overall microbial quality is also of importance as high levels of
bacteria in DUWL have been associated with occupational asthma in
dentists.
As patients, including those who may be of increased susceptibility
to legionella, are likely to be exposed to aerosols during the course
of their dental treatment, and the water which produces that aerosol
is likely to be above 20°C, it is essential that the biofilm and bacteria
including legionella are not allowed to proliferate in the water system.
BIOFILM FORMATION IN THE DENTAL UNIT WATERLINES
In DUWL that are not treated with a biocide such as disinfectant,
Sterilox or a UV light system, the contaminating bacteria are able to
multiply and form a biofilm on the inner surface of the DUWL.

DUWL are made of micro-bore tubing that has a large surface
area to volume ratio.
- Overnight stagnation.
Biofilms form rapidly, and within a week start shedding large numbers
of bacteria into the waterline.
-
Relatively low levels of water usage.
Under the Health and Safety Act and associated regulations e.g. The
Control of Substances Hazardous to Health (COSHH) Regulations
2002 (as amended), a risk assessment must be carried out in order
to identify favourable conditions for bacteria proliferation within the
water systems. A combination of factors promote biofilm formation:

DUWL QUALITY AND MANAGEMENT
Water quality in dental premises is regulated under the ‘Code of
Practice: Legionnaires disease’, ‘The Control of Legionella Bacteria in
Water Systems (L8)’, ‘HTM 01-05’, and ‘HTM 04-01’.
A separate supply of sterile water or saline should be used to cool
rotary instruments used during minor oral surgery in order to prevent
surgical wound infections.
Water supply to dental unit waterlines (DUWLs) should be of drinking
water quality with a total viable count (TVC) expected to be between
100 and 200 cfu/ml of bacteria.
As DUWL can be contaminated via three different routes, a variety of
methods are required to prevent contamination and biofilm formation.
METHODS OF MAINTAINING WATER QUALITY
In order to prevent back-siphonage of clinical material into the
municipal water supply, all dental equipment that is supplied by mains
water must have a ‘Type A’ air gap separating it from the mains water.
A legionella risk assessment should be carried out on the DUWL.
The following combination of methods are used to maintain drinking
water quality in the DUWL:

Flush the DUWL at the start of every day for two minutes, and
between patients for 20-30 seconds. Manufacturer guidelines
should be followed for the end of day procedure. Flushing
reduces the number of free floating bacteria but has no impact
on biofilm formation. Therefore flushing is insufficient on its own
to control the bacterial count in the DUWL.

Use continuous or short purges of disinfection with a biocide
(daily or weekly according to the manufacturer’s instructions).
Only use a product that is compatible with your dental chair.

Service and maintain dental handpiece anti-retraction valves and
DUWL waterlines check valves.
-
Use distilled or reverse osmosis water in the self-contained water
bottle. Tap water is to be avoided as this will introduce respiratory
bacteria into the DUWL.
-
Disinfect reservoir bottles with a biocide before refilling to
prevent biofilm formation in both the bottle and the waterlines.
-
Drain down the DUWL at the end of the day. Self-contained water
bottles should be removed, disinfected with a compatible biocide,
flushed with distilled or reverse osmosis water and left open to
air dry. Store inverted overnight. If you are using a system where

the manufacturers don’t recommend the removal of the bottle
then this advice should be followed.

Flush dental chairs that are not used, or sparadically used, at
least twice a week.
Where a control scheme is put into place it is essential that the control
measures are validated to ensure that they are effective and that
ongoing monitoring takes place to verify their continued effectiveness.
Monitoring for microbiological parameters such as the heterotroph
count can be useful for validation.
In accordance to HTM 01-05 “All microbiological measurements
should be by approved methods and / or be carried out by United
Kingdom Accreditation Service (UKAS) accredited laboratories. Dip
slides are not acceptable” (Paragraph 19.68). The bacterial standard
required for the water being used in the DUWLs being that for drinking
water quality.
The frequency of these checks in a dental practice is to be risk
assessment led. The frequency for legionella in cooling water for
example is quarterly and this might be considered a reasonable
frequency for dental systems. TVC’s taken at the same time could be
used to help reassure the responsible person that the hygiene regime
is adequately maintaining the water quality within normal drinking
water parameters. Should legionella or other bacteria be found then
the correct response (borrowing from ‘L8’ principles) might be to:

and isopropanol and a number of other agents have
been shown to be effective in the removal of biofilm as

Sharing a keyboard with other team members can be difficult at
times, and poses a threat of germ proliferation.The keyboard may be
visibly dirty with dust and even sometimes crumbs embedded into
the keys. The washable keyboards allow you to wash the dirt away
with no worries about breaking it, allowing a clean work station at all
times.
CLINICAL AREAS
Clinical areas should have a keyboard, or keyboard cover and mouse
which can be disinfected. Cling film is non-compliant and should not
be used. The computer user should also remove their contaminated
gloves before using the mouse or keyboard.
WHAT IS A WASHABLE KEYBOARD?
A washable keyboard is not totally sealed because the case and keys
are the same as a standard keyboard. The electronics inside are
sealed so that the keyboard can be washed and/or flushed under a
tap. Of course, it can be flushed through with a disinfectant, but there
is no real control over what may be happening inside the case. This is
why the keyboard is ‘washable’ rather than ‘disinfectable’.
It is the type of keyboard that is ideal for use in practice reception
areas.
WHAT IS A DISINFECTABLE KEYBOARD?
A disinfectable keyboard is one that is completely sealed so that
all contaminants, whether particle or fluid, stay on the surface.

WHY?
The responsibility of ensuring impressions have been cleaned and
disinfected before dispatch to the dental laboratory lies solely with
the dentist.

HTM 01-05 states that impressions, prostheses and orthodontic
appliances must be decontaminated prior to and after placing in a
patients mouth.
Uncertainty of impression disinfection poses risk to both the health of
the receiving dental technician, and the disinfected impression itself.
If a disinfected impression comes into contact with a non disinfected
WHEN?
Decontamination of these devices is a multi-step process that
needs to be conducted in accordance with the device or material
manufacturer’s instructions.
There are two methods of disinfection: immersion and spraying.
Immersion in disinfectant (following the manufacturer’s
recommendations if available for dilution and duration) can be the
most effective method but may be compromised by the limited
working life of the disinfectant, which is affected by the frequency of
use and the presence of biological debris.
The procedure should be as follows
1. Immediately after removal from the mouth, any device should be
rinsed under clean running water. This process should continue
until the device is visibly clean from saliva, blood or debris.
Spraying to disinfect is not a preferred method due to the aerosols
that may be released. Spraying also does not guarantee that all
surface areas will be covered.
2. All devices should receive disinfection according to the
manufacturer’s instructions (Where these are available). This will
involve the use of specific cleaning materials, please visit www.
HTM01-05.com for a range of suitable options.
After disinfection, the device should again be thoroughly washed.
This process should occur before and after any device is placed in a
patient’s mouth.
If the device is to be returned to a supplier/ laboratory or in some
other fashion sent out of the practice, a label to indicate that a
decontamination process has been used should be affixed to the
package.
