Type of Database:
Select the version of SQL Server you have installed on your network
Server Name - required
The actual name of the machine that is hosting SQL Server
User ID - Optional
To connect using SQL Authentication, supply user name
To use Windows Authentication, leave blank
Password – Optional
To connect using SQL Authentication, supply the appropriate password
To use Windows authentication, leave blank
Save Connect Info
To save the parameter you just typed into this form, click on the “Save
Connect Info” button. This will save these parameters for re-use.
