ID Internal identifier for the contact. Date Enter the date of the contact. Action Enter the type of contact (Visit, Phone Call, Letter, etc) Type Addition field for recording type of visit or letter. Visitor 1 Enter the name of the first visitor, caller, letter writer, etc. Page 15
