AMENDMENTS TO THE BIDDING DOCUMENTS AND
NEW BIDDING SCHEDULES
As discussed during the Pre-Bid Conference held on September 23, 2014, the
Bids and Awards Committee hereby issues the following amendments:
1.
Please note the amendments/additions in OMBUDSMAN BID FORM
NO. 1 (Technical Proposals) and Section VII. Technical Specifications which are
underscored and highlighted as follows:
The following statement shall be added as the last paragraph of the technical
specifications for each of the following items:
For Lot 1a. 1 set - IP Telephony Network Switch with Power Supply
b. 4 sets- 48V IP Phone Power Supply (bundle of four):
c. 2 sets- IP Phone with Power-Over-Ethernet (PoE) Injector
For Lot 21 set- Wi-Fi Tester with bundled Management Software
The last paragraph shall read as follows: “Authorized Distributor/
Partner/ Reseller of the products being offered.”
2. Please note the amendments/additions in Section III. Bid Data Sheet, Clause
29.4.(d) consisting of the list of licenses and permits relevant to the Project and other
post-qualification documents, which are underscored and highlighted as follows:

3. Prospective Bidders who have already purchased the bidding documents are
reminded to use the attached Amended Ombudsman Bid Form No. 1A (Lot 1) and
1B (Lot 2) - Technical Proposals in lieu of the bid forms previously issued.
4. All prospective bidders are hereby advised of the new bidding
schedules for the Opening of Bids due to the issuance of Presidential
Proclamation No. 875 declaring October 6, 2014 (original schedule for
opening of bids) as regular holiday.
